The following conditions shall be met during construction or installation and prior to occupancy or use: • This system is to be constructed and located in accordance with the approved plans, and the "Mound Component Manual for Private Onsite Wastewater Systems Version 2.0" SBD- 10691 - P(N.0 1 /0 1). • The pressure network is to be constructed in accordance with publications SBD- 10706- P(N01 /O1) "Pressure Distribution Component Manual for Private Onsite Wastewater Treatment Systems - Version 2.0" and/or the sizing methods of publication "SSWMP Publication 9.6 Design of Pressure Distribution Networks for ST -SAS (01/81y. • A sanitary permit must be obtained from the county where this project is located in accordance with the requirements of Sec. 145.135 and 145.19, Wis. Stats. • Inspection of the private sewage system installation is required. Arrangements for inspection shall be made with the designated county official in accordance with the provisions of Sec. 145.20(2)(d), Wis. Stats. • The area within 15 feet horizontally below the system shall remain undisturbed. Vehicular traffic or soil compaction in this area is prohibited. • A state approved effluent filter is required. Maintenance information must be given to the owner of the tank explaining that periodic cleaning of the filter is required. Access to the filter for cleaning must be provided per Comm 84 product approval conditions. k �, 1? p WILLIAM C SCHUMAKER Page 2 8/13/04 • Comm 83.22(7) - A copy of the approved plans specifications and this letter shall be on -site during construction and open to inspection by authorized representatives of the Department which may include local inspectors. Owner Responsibilities: • The current owner, and each subsequent owner, shall receive a copy of this letter including instructions relating to proper use and maintenance of the system. Owners shall receive a copy of the appropriate operation and maintenance manual and/or owner's manual for the POWTS described in this approval. • Comm 83.52(1)(a) - The owner of a POWTS shall be responsible for ensuring that the operation and maintenance of the POWTS occurs in accordance with this chapter and the approved management plan under s. Comm 83.54(1). • Comm 83.52(2) - A POWTS that is not maintained in accordance with the approved management plan or as required under s. Comm 83.54(4) shall be considered a human health hazard. In the event this soil absorption system or any of its component parts malfunctions so as to create a health hazard, the property owner must follow the contingency plan as described in the approved plans. • The owner is responsible for submitting a maintenance verification report acceptable to the county for maintenance tracking purposes. Reports shall be submitted at intervals appropriate for the component(s) utilized in the POWTS. In granting this approval the Division of Safety & Buildings reserves the right to require changes or additions should conditions arise making them necessary for code compliance. As per state stats 101.12(2), nothing in this review shall relieve the designer of the responsibility for designing a safe building, structure, or component. Code General This system shall be operated in accordance with Comm 82 -84 is. Adm. Se tic Tank The septic tank shall be maintained by an individual certified to sE rvice septic tanks under s 281.48, Slats. The curtents of the sep.ic tan snali be disposed of in accordance with NR 113, Wis. Adm. Code. Tie operating condition of the septic tank and outlet filtei shall oa assesse j at least once every 3 years by inspection. The outlet filter shall be cleaned as necessary to ensure proper o eration. The filter cartridge should not be removed unless prc,visions are made to retain solids in the tank that may slough off the filter when re oved from its enclosure. If the filter is equipped with an alarm, the filter shale be serviced if the alarm is activated continuously. At noon the weather /lighting conditions were acceptable, with temp. 60 °F, clear, sunny and frost was not encountered in the test pit. Adam Schumaker and his excavator, Dwight Alwin, assisted with the soil determination. I set up the laser transit and recorded elevations for the benchmarks reported on the soil report and for test pits in the system area. Adam stated that he had excavated a couple of other test pits that were not used/reported because they would not support a conventional system. This cleared up confusion regarding system area location and I recorded taped measurements to document actual boring locations on the lot. Adam's original site plan showed B2 at 40 feet from the east fence line based on a scale of 1:40, but it actually measured 86 ft. west of the fence and benchmarks. Without the correct measurements, a plumber would not be able to identify the actual tested area. An amended copy of the soil report site plan is attached to show where both Gary Steel and Adam's borings are located. A new boring, B4, was excavated just west of B3, and the soil profile recorded on attached County on -site verification form. The pit depth only went to '50 ", to get into the sandy loam horizon in question. Although CST Schumaker agreed that the original report of strong, medium, subangular blocky structure was not accurate, he felt that a grade of weak structure might be considered. The soil, when chunks were taken out of the profile to hand texture, parted easily into "crumbs" that appeared at first to be subangular in shape. However, these were not true peds that broke apart along planes of weakness , but fragments created by handling. The soil when observed in the horizon did not have distinct units of structure and are reported as "massive" on the county on -site report. I Page Two — Lot 36 Red Pine Corner On -site The other two borings (B 1 & 132) described on Adam's report show the sandy loam horizon beginning at 17" — 20" below grade and a shallow conventional system elevation would bring wastewater in contact with these massive sandy loams. Code changes effective on 2/l/04 list the soil application rate for massive sandy loam as 0.2 gpd/ft which will require a 3,000 sq. ft. dispersal area for a 4 bedroom/600 GPD in- ground POWTS. The tested area is approximately 45' x 150' in size, which would just fit 4 cells containing 24 chambers, plus one to make the minimum required 97 chambers @ 31.1 ft /chamber and still maintain the 5 ft. setback from the south property line. The property owner will be notified of the on -site determination and will need to have a new POWTS plot plan prepared and submitted along with a new sanitary permit application. The original mound tested area should be preserved during construction so that it provides a viable replacement area. The original soil report by Gary Steel (11 /3/2000) documented massive sandy clay loams at approximately 41 inches, with a mound recommendation. A more recent soil report, dated 2/8/04, was completed by Adam Schumaker and indicated soil conditions were acceptable for a conventional septic system. This site visit was conducted as part of the sanitary permit application review process. As a result of this site investigation, my opinion is that the original soil report appears to more accurately evaluate conditions on this lot due to observations detailed below. The weather was overcast with good light conditions for color determination and no precipitation for >24 hours. Temperatures were in the upper 30's at 2:00 p.m. The more recently excavated pits were evident and taped measurements were taken to document actual locations on the lot. An amended copy of the soil report site plan is attached to show where both sets of borings are located. Due to saturated surface soils and standing water in the backfilled test pits, I did not dig any hand borings. The backfilled test pits had exposed soil from the horizons reported by CST Schumaker and I used this material to make observations of color, texture and structure. The silt loam l horizon and another horizon of gravelly ( >15% coarse fragments) sandy clay loam was identifiable, plus a 3rd sandy /loamy soil was visible. The soils had been frozen and then thawed. The third soil was too saturated to properly texture — it turned to mush when I tried to ribbon a sample. It felt more smooth than gritty, but again, it was too wet for an accurate determination. I am particularly concerned about the conflicting structure reported for soils in the 3 horizon; rather than observing strong subangular blocky peds (3msbk) with firm consistence, the clods of the soils appeared to have weak or no structure. If the structure in the 3 horizon were weak to massive, the soil application rate would be reduced. The structure and consistence for Adam's report was determined during frozen conditions (we had a big snowstorm on 2/2/04 and several days following that with subzero temperatures). Gary Steel's soil report was done in November 2000 when soil texture and structure would not be altered by freezing conditions. The soil colors I observed on the exposed soils were closer to 5 YR 4/4 than the 10 YR 3/6 reported for horizon 3, but that may have been a factor of light conditions and frozen soils in situ on the day of Adam's evaluation. However, Comm 85.20(4) requires that (a) soil color evaluation must be performed on days when light conditions permit accurate color determination and (b) frozen soil material shall be thawed prior to conducting evaluations for color, texture, structure, and consistence. I do not have any documentation that samples were taken home and thawed prior to their evaluation. The CST Schumaker's report was incomplete at the time of submittal: 1. There was no lot number, parcel number, or subdivision name on any of the pages to identify the site. (The only way I determined the site location was matching the owner's name from a phone call regarding the permit application status) 2. The site plan did not follow the 1 ": 40' scale indicated; based on actual site measurements, the borings were not the distances shown on the plot plan from either the west or south lot lines. Refer to the amended copy of the site plan that shows measured distances. 3. The benchmarks were at the same location as Gary Steel's report, however Adam's plot plan makes it look like they are farther from the southwest lot corner and south lot line. The PVC pipe and fence post benchmarks are still in place for use during installation of the POWTS. 4. A replacement area is not provided, unless the original mound area was preserved for future use. The POWTS plot plan accompanying the sanitary permit application was based on this incomplete soil evaluation report. The proposed conventional system would not meet setbacks to the south lot line and the mound area would be destroyed during installation (couldn't be used as a potential replacement). The plan also places the house /driveway within the 100 ft. building setback. The permit application for a conventional POWTS will be disapproved until an acceptable plan is submitted.
