Dear Heidi Madsen,
Community Development staff have reviewed the application to amend the Land Use Permit (LUP-
2017-a50) issued on February 14, 2018, for temporary occupancy pursuant to Chapter 1770 (3)(c)2,
for an already completed project including the construction of a single- family dwelling, attached
garage, and driveway The project involved land disturbance on slopes 20-249%In the Rural
Residential District, pursuant to Chapter s] 12hp, this requires a land use permit
The request for the addendum has been conditionally approved, based on the application
submission and the following findings
• Port construction site plans were submitted on April 1, 2019, with Conditional Use Permit
Application(CUP-2019-009) An after -the -fact addendum application was submitted on November
25, 2019, for land disturbance on slopes 20 24 9%,the project was completed in January 2018
The structure used for the temporary occupancy has been removed from the site
• There is setback encroachment a long the western property line with the principal structure
Permission has been granted to adjust the lot lice, this will require an exempt CSM and will be
completed separately
• The area around the land disturbance has been seeded and vegetation has been established
Bared on these findrngs, the Land Use Permit is approved and has been issued after -the -fast, with
the following condemns
1 Asper §1712(8)(H mitigation shall be maintained in perpetuity A Dormwster affidavit must be
corded with the St Croix County Register of Deeds Office
2 It is the apphequity responsibility to secure any other recurred local, sate or federal centrals) and
approvals) prior to land disturbance activity
3 Failure to comply with the terms or conditions above may result in the revocation of this permit by
the Zoning Administrator pursuant to Chapter 17 71
