Only four persons have start dates while nearly 30 persons have been referred. 6. Discuss transfer of children from CIPIB Waivers to the Children's Long Term Support (CLTS) Waiver. 7. Status report on Aging and Disability Resource Center Update by Brad Beckman, ADRC Manager ADJOURN (Items may be taken in different order) ** Additions /Changes /Corrections Copies to: County Clerk's office Committee Members Items for County Board approval must be submitted to the County Clerk's office by Wednesday morning preceding a County Board meeting. 11/6/2008 8:17 AM LONG TERM SUPPORT PLANNING COMMITTEE MEETING MINUTES November 20, 2008 MEMBERS PRESENT: James Craig, Keith Gregerson, Lorraine Jacobs, Ron Jansen, Melissa Walthal, Marge Wolske MEMBERS ABSENT: Bill Cranmer, Deb Ramacher MEMBERS EXCUSED: Richard Peterson STAFF PRESENT: Ron Lockwood, LTS Coordinator; Angie Magoon, LTS Program Specialist; Brad Beckman, ADRC Director Meeting called to order by Chair Gregerson at 1:30 p.m. Roll Call: Quorum present Motion to adopt agenda by Wolske, second by Jacobs. Motion carried. Motion to approve previous meeting's minutes by Wolske, second by Jansen — with the addition of Brad Beckman under staff present on previous minutes. Motion carried. Next Meeting Date and Time: Will be scheduled on an as needed basis. OLD AND NEW BUSINESS: 1. Financial Report: Handout by Lockwood through 9/30/08. Discussion. 2. MA Waiver Exceptional Expense /COP High Cost Requests: None 3. Nursing Home Relocation /Diversion Requests: All requests for relocations or diversions are being handled by CHP now. The county has no involvement. There have been 10 relocation referrals made and one placed. 4. Long Term Care Reform transition update: Handout by Lockwood. 48 persons have transitioned in September, 63 in October, and 59 in November. The plan is to move 40 -50 more persons effective 12/1/08. 294 Waiver participants will transfer to the managed care entity or the IRIS Waiver system between September 1, 2008 and February 1, 2009. 5. IRIS Waiver implementation: In St Croix County, about 10 -15% of current waiver participants are selecting the IRIS program that provides more autonomy for consumers regarding the selection of service providers and the kinds of services provided. 80% are choosing the Family Care model, with less then 10% choosing the Partnership model. IRIS 1 funds can carryover month to month if not utilized. Progress is slow. Currently only 4 persons have start dates while nearly 30 persons have been referred. 6. Transfer of children from CIP1B /1A Waivers to the Children's Long Term Support (CLTS) Waivers: Most transfers were effective October 1, 2008. The State will be reallocating CIP1A and CIP1B money to the Children's DD Waivers to support children's waiver services past the managed care conversion. 7. Brad Beckman, Director, Aging and Disability Resource Center — Status Report: Beckman reports things are going well. CHP attending a governing board meeting. Some persons with Family Care have been denied by local clinics and hospitals. There was some confusion but this is hoped to be worked out soon. Medicare Part D open enrollment is 11/15/08 — 12/31/08, where plan changes can also be made. Motion made to adjourn at 3:10 p.m. by Jacobs, second by Jansen. Motion carried.
