COUNCIL ON AGING AND DISABILTIES MEETING
January 21, 2010
County Board Room, Hudson
CALL TO ORDER: Chair Nelson called the meeting to order at 9:00 a.m. Pledge of Allegiance
was said.
ROLL CALL: Supervisors: Al Schrank, Gerald Peterson, Esther Wentz, and Sharon Norton -
Bauman; Councilmembers: Ramsey Lee, Kristen Johnson, Sue Nelson, Vera Grant, Michelle
Chevalier and Lois Bum. Also present were: Administrative Assistant Renea Aeschliman;
Community Services Coordinator Tracy Davis; Preventative/Nutrition Program Coordinator Randi
Hanson; Information & Assistance Specialist Kelly Weber; Caregiver Support Coordinator Nancy
Abrahamson; Health and Human Services Director Fred Johnson; ADRC Manager Brad Beckman;
Regional Transportation Coordinator Center for Independent Living for Western Wisconsin, Inc
Denise Larson; Transportation Specialist Center for Independent Living for Western Wisconsin, Inc
Dale Heimke; County Board Chairman Roger Rebholz; Charlene Sackett. Excused was
Councilmember Judy Harrison.
ACTION OF PREVIOUS MINUTES: A motion to approve the minutes as corrected was made
by Supervisor Norton - Bauman and seconded by Supervisor Peterson; motion carried.
DATE OF NEXT MEETING: Thursday, February 18 in the County Board Room of the St.
Croix County Government Center. Council member Burn will review vouchers.
1. Monthly Financial Report: Beckman handed out financial information for the ADRC and the
Aging programs through December 31, 2009.
2. Consideration of Claims and vouchers: Supervisor Wentz reviewed the vouchers. A motion to
accept the vouchers as presented was made by Supervisor Wentz and seconded by Council member
Lee; motion carried. Please see page 4 for a list of vouchers reviewed by Wentz. Council member
Burri will review next month's vouchers.
3. Center for Independent Living for Western Wisconsin, Inc: Regional Transportation
Coordinator Center for Independent Living for Western Wisconsin, Inc Denise Larson presented
information to the Council regarding the services and programs offered through the Center for
Independent Living for Western Wisconsin, Inc. ( CILWW). CILWW is a nonprofit agency with
offices in Menomonie and Rice Lake with another office opening in Eau Claire. Larson spoke on
the transportation programs through CILWW — New Freedom Transport and Voucher Program.
The transportation program provides rides using volunteers to medical, social, education
destinations. Persons receiving services are under age 60 with a disability. Rides are granted upon
availability of drivers. The transportation programs are funded by grants from the Federal New
Freedom Program, CILWW, other counties (Dunn, Pierce, Chippewa, and St. Croix) and the fares
of the participants with a budget in 2009 of $80,000. CILWW sent out a survey last year and found
that nights and weekend services are needed. CILWW has started working with the ADRC on the

trying to meet that need.
4. Presentation by Councilmember Chevalier: Council member Chevalier shared that she is part
of a group that formed in September to look at the mental health services in St. Croix County. She
stated the purpose of the group is to eliminate the us and them concept between the client and
therapist and become partners. The group would also like to establish peer support with training.
5. Aging Updates:
• 2009 Aging Statistics
o Beckman handed out information on the statistics for 2009. There were 26,781
congregate nutrition meals provided by 185 volunteers and 21,441 home - delivered meals.
There were 280 rides provided by 20 volunteers through the Volunteer Medical
Transportation program. The Nutrition Site Van Transportation program provided 15,495
one way trips to 290 persons needing rides to the nutrition sites, local medical
appointments, shopping, etc. The DayAway Adult Respite program provided 6,183 hours
of respite to caregivers. The Interfaith Volunteer Program currently has 175 active
volunteers providing a variety of services to 150 participants.

• Interfaith Volunteer Needs handout Caregiver Support Coordinator Abrahamson
informed the Council there is an urgent need for drivers to take individuals to dialysis,
therapy and other ongoing medical treatments.
• DayAway Adult Respite pro r�pdate Abrahamson reported there are currently
openings in Hudson. More volunteers are needed to assist with daily activities. DayAway
is open to anyone who is in need of socialization.
• AFCSP 2010 Funding The Alzheimer's Family Caregiver Support Program ( AFCSP)
funding for 2010 has been cut in half. AFCSP money is used for scholarships to
DayAway Adult Respite and 2010 the goal is to sponsor 7 clients.
• Transportation Program
° The Glenwood City Senior Center is requesting a van to go to Como Park on February
19 A motion to approve the out of county trip was made by Supervisor Schrank and
seconded by Supervisor Peterson; motion carried.
o Collaborative agreement between Center for Independent Living for Western Wisconsin
to provide weekend and night transportation for Elderly and Disabled consumers:
Community Services Coordinator Davis informed the Council a meeting between Denise
Larson from Center for Independent Living, Elijah, owner and proprietor of Kilimanjaro
Taxi, Tim Sheehan, Director of Center for Independent Living via the phone, and Davis
was held in January. The group discussed starting the program in Hudson with specific
hours and days. Kilimanjaro Taxi has agreed to be the provider with Center for
Independent Living Center as the coordinator. Once more information has been gathered,
the Transportation Advisory Committee will meet.
• Senior Center/Nutrition Program

Senior Center Highlights Preventative/Nutrition Program Coordinator Hanson reported
a SafeSery class is scheduled for January 22 for the Nutrition Program Managers,
Nutrition Program Cooks, and Nutrition Program Cooks Aides. The training is a
requirement for all staff serving or preparing food at the Nutrition Sites. The training
will be held in Hudson and will be put on by the Amy Ramsey, Nutrition/Prevention
Specialist, Bureau of Aging & Disability Resources and opened up to other
surrounding county agencies.
The First Aid/CPR training for February 5 th is full. The Nutrition Program Managers
and Van Drivers will be attending the training.
February 17 a Nutrition Program Managers meeting is scheduled at New Richmond.
On February 24 the Nutrition Advisory Council will meet in River Falls.
AARP tax assistance will be starting at the local senior centers in February.
Interviews for the Glenwood City Nutrition Program Manager are being scheduled. A
retirement party was held for Donna Zacharias celebrating the 17 years of dedicated
service the Department on Aging/ADRC.
■ Volunteer Appreciation Luncheon: Hanson informed the Council the Volunteer
Appreciation Luncheon will be held on Friday, April 23 at The Orchard in Baldwin.
Individuals who contributed a minimum of 20 hours in 2009 to the ADRC will receive
an invitation to the event and be honored. All Council members will be invited as well.
6. ADRC updates:
• Information & Assistance Specialist — update: Information & Assistance Specialist Kelly
Weber informed the Council that she and the other two I &A Specialists have been busy.
• Client Transition Update Beckman reported that since August 2008, excluding former
waiver clients, 29 individuals have chosen Community Health Partnership — Partnership Plan,
268 have chosen Community Health Partnership — Family Care Plan, 103 clients have been
referred to IRIS and 25 individuals have completed Nursing Home Relocation. One hundred
nineteen individuals from the waiting list have been served with one hundred thirty -five on the
current waiting list. Pre - Admission Consultations have ranged between 40 — 50 a month with
the majority of those not needing a consultation with an I &A Specialist.
The information & Assistance Specialists are working on a letter to the Special Education
Programs at the schools. The state requests that the school receive information on transitioning
students 17 years and 9 months into the adult programs.
Implementation of the agency /provider portion of the information and referral database,
BEACON will begin shortly with the assistance of GWAAR. The database is currently being
utilized by the I &A for client tracking. Once the agency /provider area information has been
entered the information will be available for all Health & Human Services and ADRC staff and
part of the ADRC website.
• ADRC/HHS Strategic Plan for 2010: Beckman informed the Council he is working with
Health and Human Services and an outside consultant, Duncan to get the action plan in place.
The action plan will break down who in the ADRC will be responsible for certain tasks within
the Strategic Plan. A copy of the information will be provided to the Council next month.

: 2010 Health & Wellness Expo March 27 Beckman informed the Council the 2nd annual
Health & Wellness Expo is scheduled for March 27 at the WITC in New Richmond. A
committee will be meeting to get the information out to prospective vendors and to advertise
the event.
• Service Directory: Beckman informed the Council that a rough draft of the directory was
presented to the committee and advertisement is being acquired by the printer. The directory
will be available before the Health & Wellness Expo in March.

Beckman read thank you's from Secret Santa recipients.
