Community Development staff have reviewed the Land Use Permit application for the construction
of a Single -Family Dwelling, driveway and septic system involving land disturbance activities on
slopes 12 — 24 99% pursuant to the General Zoning Chapter 15415 and land disturbance arnviaes
greater than 10,000 square feet in the Shoreland Overlay District Chapter 76345(4)(a) The request
has been conditionally approved based on the application submission and the following findings
• Previously a Land Use permit LUP-2018-04$ had been issued and expired
• The proposed structures meet the setbacks to the property lines
• Erosion and Sediment Control plans have been submitted that meet Wisconsin Department 0f
Natural Resources Technical Standards Best Management Practices and will be incorporated
• The proposed Private Oni Wastewater Treatment System was approved October 8. 2018 and
expires October 8, 2020
• The Stormwater Management plans have been submitted that meet the performancestandards
described in Wisconsin Administrative Code FUR 151 Subchapter III, Non-Ag Performance Standards
Total land disturbance is less than l Acre
• No temporary occupancy at the site is to occur without a Temporary Occupancy Permit
Based on these findings, approval of the Land Use Permit is subject to the following conditions'
1 Prior to Construction, erosion control measures, such as silt fencing and a tracking pad at the
entrance must be installed prior to all land disturbance activity
2 Apre-construction inspection is required to validate setbacks and erosion control Please call me

3 Permanent vegetation shall be establ shed once final grade is reached or as soon as applicable per
plan
4 All temporary erosion control measures shall be left in place and maintained until the site has reached
a point of at least 70%permanent vegetation.

5 A post-constructson Inspection is required prior to removing the temporary sediment and erosion

6 It is the applicant's responsibility to secure any other required local, state or federal permil and
approvals) prior to land disturbance activity
Failure to comply with the terms or conditions above my result in the revocation of this permit by
the Zoning Administrator pursuant to Chapter 15 555
This approval is subject to the conditions listed above, it does not allow for any additional
construction, structures, or buildings beyond the limits of this request your information Sill remain
on file at the St. Croix County Community Development Department sure It is your responsibility
to ensure compliance with any other local, state, or federal permitting or regulations, including
contacting the Town of Star Prairie and the Department of Natural Resources to inquire if additional
permissions are required. This permit is valid for one year with the possibility of up totwo (2)six

month extensions if the applicant submits the appropriate permit extension fee and documentation
to the Zoning Administrator A copy of the Land Use Permit placard should be submitted to the
local Building Inspector for the town upon application for the building permit The orange placard
must be posted on thejob-site and visible from public view
