COUNCIL ON AGING AND DISABILTIES MEETING

CALL TO ORDER: Chair Nelson called the meeting to order at 9:00 a.m. Pledge of
Allegiance was said.
ROLL CALL: Supervisors: Al Schrank, Esther Wentz, Richard `Buzz' Marzolf, and Sharon
Norton- Bauman; Councilmembers: Sue Nelson, Ramsey Lee, Judy Harrison, Vera Grant,
Michelle Chevalier and Lois Burri. Also present Administrative Assistant Renea Aeschliman;
Caregiver Support Coordinator Nancy Abrahamson; Information & Assistance Specialist
Denise Gillen; Preventative/Nutrition Program Manager Randi Hanson; ADRC Interns Lorie
Jensen & Nancy Wiedmeyer; Health and Human Services Director Fred Johnson; ADRC
Manager Brad Beckman; Regional ADRC Quality Specialist Division of Long Term Care
Kathleen Solberg; Administrative Coordinator Chuck Whiting. Excused was Councilmember
Kristen Johnson.
ACTION OF PREVIOUS MINUTES: A motion to approve the minutes was made by
Supervisor Marzolf and seconded by Council member Grant; motion carried.
DATE OF NEXT MEETING: Thursday, June 17"' in the County Board Room of the St.
Croix County Government Center in Hudson. Council member Johnson will review vouchers.
1. Monthly Financial Report: Beckman distributed financial information for the ADRC and
the Aging programs through April 30, 2010.
2. Consideration of Claims & Vouchers: Council member Harrison reviewed the vouchers.
A motion to accept the vouchers as presented was made by Council member Harrison and
seconded by Supervisor Schrank; motion carried. Please see page 4 for a list of vouchers
reviewed by Harrison. Council member Johnson will review next month's vouchers.
3. ADRC Consumer Survey results: Student Intern Lorie Jensen presented a power point on
the results of the ADRC Consumer Survey. Handouts were given to all the Council members
and she explained the results.
4. ADRC State Agency pilot survey: Regional ADRC Quality Specialist Division of Long
Term Care Kathleen Solberg handed out a survey for each of the Council members to complete
and return to the new ADRC Intern Nancy Wiedmeyer at the next meeting. Wiedmeyer will
compile the information and return one survey to the state.
5. Presentation — Medical Records Coordinator Tammy Wilson: Wilson introduced

herself to the Council and explained the goals for the Medical Records Department. The
Department is currently working on implementing electronic records and the process for
retrieving records has been changed. A training on HIPPA & HITECH for all Health and
Human Services staff will be held on June 1 S` in Hudson.
6. Recotnition of Economic Support Supervisor Mary Sguyer: Manager Beckman
presented Supervisor Squyer with a plaque in appreciation for her hard work and
contributions to the Aging & Disability Resource Center.
7. Adn Updates:
Caregiver Support Coordinator Abrahamson informed the Council on the two preventative
programs being offered through the ADRC. Abrahamson recently received training on a
seven week falls prevention course called Stepping On. She will be teaching the course
with a physical therapist and peer leader. Participants can attend this course to learn how
to minimize the risk of falls through muscle and balance strengthening, assessing for
hazards in the home, and evaluating their nutrition and medications. Classes will be
offered beginning this summer and fall. The other training she attended was called Living
Well With Chronic Conditions. It is a six week course designed to help anyone who is a
caregiver or has a chronic condition to develop strategies for better self - management of the
disease.

• Interfaith Volunteer Needs Handout Council members received the most recent
volunteer needs.
• DUAway Adult Respite Program m Update Caregiver Coordinator Abrahamson
informed the Council there have been new referrals to the Hudson location. A
volunteer has been found to assist with the New Richmond location.
• Caregiver Conference: Abrahamson informed the Council preparation for the Annual
Caregiver Conference is underway. Brochures will hopefully be printed June /July and
mailed out shortly thereafter. The Conference will be on October 15`
• Transportation Program
■ The Somerset Senior Center to Minneapolis for the Fogey Follies on June 4 River
Falls to Minneapolis Swedish Institute on June 13`"; Glenwood City Senior Center to
Dunn County on June 20 A motion to approve the out of county trips was made by
Council member Lee and seconded by Supervisor Wentz•, motion carried.
• Senior Center/Nutrition Pro ram
■ Preventative Nutrition Program Manager Hanson informed the Council of a
collaborative effort between the ADRC and UW Extension. The UW Extension held a
train- the - trainer session on nutrition and workshops will be set up by those trainers at

our local Senior Nutrition Sites.
■ Annual County -Wide Card Tournament: Beckman informed the Council that the Card
Tournament for 2010 was completed yesterday in Amery. Scores need to be tallied yet
and the results will be shared with the Nutrition Sites and in the ADRC News. The
winning team will receive the traveling trophy at the Annual County -wide Senior Picnic.
■ CWAG Regional Meeting — wrap up: The District 6 meeting on April 29 at the
Baldwin Senior Center was very well attended. There were over 70 individuals in
attendance and lots of information was shared.
■ Teeing It Up for The Seniors Golf Tournament: Hanson shared flyers and information
on the upcoming fundraiser for the Congregate Nutrition Program. Individuals can
register by contacting R.andi Hanson at the ADRC. The $35.00 registration fee includes
18 holes and a cart at the Hammond Golf Course. There will also be a Silent
Auction/Raffle.
8. ADRC updates:
• Student Intern: Nancy Wiedmeyer introduced herself to the Council. Wiedmeyer will
be with the ADRC for 16 weeks completing her field placement for her Bachelor's
degree in Social Work through College of St. Scholastica in St. Paul, Minnesota.
• Information & Assistance Specialist — Update Information & Assistance Specialist
Denise Gillen reported that since August 2008, excluding former waiver clients, 13
individuals have chosen Community Health Partnership — Partnership Plan (not
including former Long Term Support Waiver clients), 94 have chosen Community
Health Partnership — Family Care Plan (not including former Long Term Support
Waiver clients), 110 clients have been referred to IRIS. One hundred forty -five
individuals are on the current waiting list. Pre - Admission Consultations are received
daily. There are currently three youth transitioning into adult services in process.
• Website Update: Beckman informed the Council that a staff member from the ADRC is
part of the work group currently working on the redesign of the County website. Once
the home page for the County is determined, then the ADRC will work on its page or
pages.
9. Correspondence
• Beckman read thank you notes /letters received by the ADRC.
• Beckman informed the Council of the Senior Health days that will be held on May 26`
at the local YMCAs and the ADRC will have a presence at each of them.
• Beckman informed the Council that the Annual Public meeting for Health and Human
Services will be held on June 16' at 6:00 p.m. at the Head Start in New Richmond.
