Community Development staff have reviewed the Land Use permit application for an addition to
an existing home, within a Shoreland and Shoreland-Wetland Overlay Districts The request has
been conditionally approved based on the application submission and the following findings
• A portion of the proposed land disturbance will occur on slopes 12-1999%
• The proposed land disturbance will not affect slopes greater than 25%measured over
horizontal distance of 50 feet
• The proposed structures meet setbacks to the property Ines and maximum 35-foot height
requirement
• Proposed land disturbance is within 300 it of the ordinary high- water mark(OHWM) of
navigable pond and near Shoreland-wetlands
• The proposed principal structure meets the]5-foot setback to the OHWM
• Vegetation will not be disturbed within 35-feet inland of the pond
According to FEMA Firm Map Panel No 55109CO210E, a portion of the property Is mapped
Zone A floodplam The project proposed is mapped outride of the floodplam
• The total impervious surface calculation within 300 feet of the OHWM resulted in
approximately 3 ]%, therefore mitigation is not required
• Erosion and sediment control plans have been submitted and meet Wisconsin Department
of Natural Resources Technical Standards Best management practices will be incorporated
• A Land Use Permit was previously issued for the construction of a Single Family Dwelling,
driveway and POWFS May P', 2017
Based on these findings, approval of the Land Use Permit is subject to the following conditions.
1 Prior to Construction, erosion control measures, such as silt fencing must be installed prior to all
land disturbance activity
2 Apre-construction inspection is required to validate setbacks and verify the installation of erosion

3 St Croix County reserves the right to require additional sediment and erosion control measures to
be installed if found necessary due to site -specific concerns and will be documented in an as -built
site plan
4 All temporary erosion control measures shall be left in place and maintained until the site has reached
a point of at least ]o% permanent vegetation. Permanent vegetation shall be established once final
grade is reached or as soon as applicable per plan
5 Apost-construction inspection is required prior to removing the temporary sediment and erosion
control measures to verify proper vegetation cover has been established Please call me at

6 It is the applicant's responsibility to secure any other required local, rote or federal perm i and
approvals) prior to land disturbance activity
Failure to comply with the terms or conditions above may result in the revocation of this permit by
the Zoning Administrator pursuant to Chapter 16 570 1.

ensure compliance with any other local, state, or federal permitting or regulations, including

permissions are required This permit is valid for one year, with the possibility of up to two (2) slx-
month extensors if the applicant submits the appropriate permit extension fee and documentation
to the Zoning Administrator A copy of the Land Use Permit placard should be submitted to the
town local Building Inspector upon applying for town building permits) The orange placard must
be posted on the)obsrte and visible from public view
