Tank inspections must include a visual inspection of the tank(s) to identify any missing or broken hardware, identify any cracks or leaks, measure the volume of combined sludge and scum and to check for any back up or ponding of effluent on the ground surface. The dispersal cell(s) shall be visually inspected to check the effluent levels in the observation pipes and to check for any ponding of effluent on the ground surface. The ponding of effluent on the ground surface may indicate a failing condition and requires the immediate notification of the local regulatory authority. When the combined accumulation of sludge and scum in any tank equals one -third (Y or more of the tank volume, the entire contents of the tank shall be removed by a Septage Servicing Operator and disposed of in accordance with chapter NR 113, Wisconsin Administrative Code. All other services, including but not limited to the servicing of effluent filters, mechanical or pressurized components, pretreatment units, and any servicing at intervals of 512 months, shall be performed by a certified POWTS Maintainer. A service report shall be provided to the local regulatory authority within 10 days of completion of any service event. Page of Z START UP AND OPERATION For new construction, prior to use of the POWTS check treatment tank(s) for the presence of painting products or other chemicals that may impede the treatment process and /or damage the dispersal cell(s). If high concentrations are detected have the contents of the tankls) removed by a septage servicing operator prior to use. System start up shall not occur when soil conditions are frozen at the infiltrative surface. During power outages pump tanks may fill above normal highwater levels. When power is restored the excess wastewater will be discharged to the dispersal cell(s) in one large dose, overloading the cell(s) and may result in the backup or surface discharge of effluent. To avoid this situation have the contents of the pump tank removed by a Septage Servicing Operator prior to restoring power to the effluent pump or contact a Plumber or POWTS Maintainer to assist in manually operating the pump controls to restore normal levels within the pump tank. Do not drive or park vehicles over tanks and dispersal cells. Do not drive or park over, or otherwise disturb or compact, the area within 15 feet down slope of any mound or at -grade soil absorption area. Reduction or elimination of the following from the wastewater stream may improve the performance and prolong the life of the POWTS: antibiotics; baby wipes; cigarette butts; condoms; cotton swabs; degreasers; dental floss; diapers; disinfectants; fat; foundation drain (sump pump) water; fruit and vegetable peelings; gasoline; grease; herbicides; meat scraps; medications; oil; painting products; pesticides; sanitary napkins; tampons; and water softener brine. ABANDONMENT When the POWTS fails and /or is permanently taken out of service the following steps shall be taken to insure that the system is properly and safely abandoned in compliance with chapter Comm 83.33, Wisconsin Administrative Code: • All piping to tanks and pits shall be disconnected and the abandoned pipe openings sealed. • The contents of all tanks and pits shall be removed and properly disposed of by a Septage Servicing Operator. • After pumping, all tanks and pits shall be excavated and removed or their covers removed and the void space filled with soil, gravel or another inert solid material. CONTINGENCY PLAN If the POWTS fails and cannot be repaired the following measures have been, or:must be taken, to provide a code compliant replacement system: A suitable replacement area has been evaluated and may be utilized for the location of a replacement soil absorption system. The replacement area should be protected from disturbance and compaction and should not be infringed upon by required setbacks from existing and proposed structure, lot lines and wells. Failure to protect the replacement area will result in the need for a new soil and site evaluation to establish a suitable replacement area. Replacement systems must comply with the rules in effect at that time. ❑ A suitable replacement area is not available due to setback and /or soil limitations. Staff requested this information because the proposed building site and driveway varied from the ones approved by the Planning, Zoning and Parks Committee at the time of plat approval. Staff has reviewed the sketches submitted by Dave Nash of McCombs Frank Roos and Associates and found that a Special Exception will not be required for the construction since there are no slopes of 20 % or greater that will be disturbed during this project. However, since the construction will disturbed slopes in excess of 12 % you are required to submit an erosion control plan for the construction site (driveway and house). The following is the list should be part of the erosion control plan: 1. Narrative of the project. 2. A description of the Best Management Practices (BMP'S) (erosion mat, silt fence, triangular sift dikes, seed, rock lined channels, etc...) that will be used to stabilized the site during and after site construction. The BMP's can be found in the Wisconsin Construction Site Handbook. This description should discuss how the adjacent lots will be protected during and after construction. 3. Site plan showing the location of the BMP's, direction of storm water flow, soil stock pile locations, cut and fill areas, areas of concentrated flow, existing vegetation cover, culvert locations (if needed), property lines, septic area, building site, driveway location, driveway finished surface, etc... 4. Due to the steep slopes in this subdivision and on the adjacent lots, it is very important to provide a sound plan that will ensure there are not Stormwater or erosion problems after development. Please note that the developers are constructing a small Stormwater treatment pond on the north side of the shared driveway for lots 9 and 10. This pond might be able to be sized to accommodate the Stormwater runoff from the proposed drive and construction. Contact the developer and /or Dave Nash for further information. If a detention basin is proposed for the project, use the 2 -year, 24 -hour storm event for its calculations. 5) Indicate whether culverts will be incorporated into the design and construction of the driveway. If culverts are planned, please show location(s) and size. I strongly suggest that you consult and /or hire an engineer to design /review your plans to ensure the correct calculations, design methods, and materials are incorporated into your project. Pre - construction meeting: Once the final plan is approved by County staff and before grading is allowed at the site, a pre - construction meeting is required to be held at the site with staff from the Land and Water Conservation and Zoning Departments, grading contractor, and person /firm responsible for installing and maintaining all erosion control practices. Tree Cuttinq: Cutting trees this winter in the location of the driveway and home site is acceptable as long as heavy equipment is not used to clear and grub the site (i.e. bulldozers). Cutting the trees with a chainsaw and removing them with a truck or skidder would be the preferred method of tree removal. She has done a good job of showing proper erosion control around the home but the driveway may be a bit beyond her expertise. I strongly suggest they consult and /or hire an engineer to review their plans. 1) Please provide a grading plan, plan and profile drawing and at least two cross sections for the proposed driveway. The 101lowing narrative outlines our construction plans and schedule. We plain on building a walkout rambler on Lot 9, as these lots are marketed and sold as walkout lots. We would like to remove as few trees as possible to put in our driveway and the home. Lot 9 is a fully wooded and beautiful 4 acre .lot and we would like to keep it as undisturbed as possible. We will be using silt fence and straw bales as necessary to control any erasion, from the grading of the driveway and excavation of the foundation area of the 1101ne. There is it nice level area on the northwest corner of the lot, which works perfect for a home site. Since than area is level, that is where we will be stockpiling the excavated soil. We will be putting silt fence and possibly straw bales around the stockpiled soil to keep it stabilized, depending on what is necessary for the weather conditions at the time. No other adjacent lots will be disturbed either during or after construction, m we have adequate setbacks and the lot size is quite large (4 acres). The natural slope of* the land goes to the south and there is an existing driveway at the edge of the lot, outside of the subdivision area. The cut slopes of the walkout area will not be greater than a 4:1 slope. We wou p ud like the slope to mimic the natural area of the woods and blend in with native and natural landscaping. We will he using sod immediately upon final grading to eliminate any possibility of erosion to the surrounding woods.
