LAUNDRY BUILDING INCIDENT REVIEW Tobin reported a decommissioned water main St. Croix County Health and Human Services Board Page 2 February 17, 2010 froze in the basement of the old laundry building and on 01/25/10 it broke and filled the basement of the building with approximately 90,000 gallons of water. He said with the help of New Richmond City Utilities Water Department and the Highway Department the water was removed from the basement within 48 hours. He reported Kristen Ziliak, Risk Manager was on site during this incident and the incident was reported to the Finance Committee. Be said 911 communications, IT and communication back up for New Richmond were housed in the basement in addition to Westfields Hospital antenna station. Westfields' equipment is working again and a new contract is being developed with Westfields regarding their antenna equipment. The equipment will be relocated to the upstairs of the building and no equipment will be housed in the basement. Wentz instructed Johnson to draft a letter of thanks to City Utilities (Rick Powers) for the assistance the Utilities Department supplied to remedy the incident. Tobin estimated a cost of $25,000 due to the incident. Wentz thanked Tobin for the quick response to the incident. FINANCIAL REPORT The following financial report for Health and Human Services through December, 2009 was distributed. Pempek said the State has taken back some of the overage for the Long Term Support contracts. She said there will be some additional take backs over the next four to six week period. The State reconciliation for Long Term Support accounts will take place between March and June, 2010 with the final reconciliation being done in June, 2010. Ron Kiesler asked about COP funding that was not recorded as revenue in the Behavioral Health budget and Pempek indicated that money was part of the State take back. Pempek said there are some additional clean ups to be done for year end but self- insured were moved to doubtful accounts at year end. NURSING HOME ADMINISTRATOR'S REPORT Robinson said the average monthly census for December was 66, with 7 Medicare, 37 Medicaid, 21 private pay and 1 Family Care resident. Robinson reported we are getting paid an average daily rate of $466.00 for Medicare so we are getting paid for the higher acuity of our residents. Staff documentation has improved and we are getting better credit for the work we are doing. The Medicare revenue is $21,812 /month above budget. St. Croix County Health and Human Services Board Page 7 February 17, 2010 Robinson is working with Tobin and Dave Simmons regarding room renovation. It was the consensus to delay any renovation for a couple of months until the results of the Phase II report is completed for the Nursing Home building. Tobin and Robinson will devise an interim plan to touch up some of the rooms and patch /paint some of the worst walls. Last month Robinson distributed a new Nursing Home brochure to the Board members. This brochure has been sent to all referring hospitals and discharge planners with follow up phone calls made to these facilities by Lisa Leahy. He said there are three open beds in the Nursing Home today. Peterson questioned if Westcap had been contacted as to their role in the Spring Valley facility project. Robinson indicated he is not sure where future planning will be until the completion of Phase II. Peterson instructed Robinson to contact Westcap to get general information regarding assistance available for assisted care apartments. COMMUNITY HEALTH IMPROVEMENT PLAN- PUBLIC HEALTH Wendy Kramer, along with Karen Hansen from the Hudson Hospital and Clinic gave a presentation on the Community Health Improvement Process. She reported the process was a collaboration of St. Croix County, Public Health and the hospital. The partnership between the hospital and public health arose out of a common need. The goal of the process is to identify critical health issues and prioritize health needs for the purpose of educating and mobilizing organizations to address top health priorities, garnering resources and planning actions to improve health and quality of life. The partnership used Healthiest Wisconsin 2010 as the framework for the process and the steering committee selected the top five (out of 11) health priorities to address. County Health Rankings results will be released today with booklets containing the rankings being issued in the near future. St. Croix County ranked second in health outcomes and eighth in health factors out of 73 counties. The second part of Community Health Improvement Process is to advance the plan. Hansen defined the Optimal Lifestyle Metric (OLM) which includes being physically active, not smoking, eating 5 fruits and vegetables each day, and drinking alcohol in moderation. She noted OLM adherence is low in the overall population (5 -80) but higher (approximately 10 %) among the employed population. Adults spend a substantial portion of their waking hours in the workplace. Frequent and sustained messages can reach employees and dependents. Companies benefit through healthy and more productive people. People benefit through lower medical costs and more productive lives. A meeting is planned for March 3, 2010 from 9:00 AM to noon at the Hudson Hospital Conference Center. Board members were invited to contact Hansen if they would like to attend. H1N1 UPDATE Kramer reported there has been low influenza activity statewide and in St. Croix County. No seasonal influenza has been detected at this time; however, lab testing continues, which indicates H1N1 is the predominant influenza virus. The EOC was closed January 25 and the Incident Command System was deactivated at that time. Public Health has additional H1N1 vaccination clinics scheduled for February 21 at the Government Center and February 25 at the Hudson United Methodist Church in addition to every Monday afternoon in the Public Health office in New Richmond. Kramer stressed the importance of getting vaccinated as it is not known if a third pandemic wave will strike. St. Croix County Health and Human Services Board Page 8 February 17, 2010 MEDICAL RECORDS STAFFING PLAN A move has begun towards consolidating medical records in the facility to have a centralized Medical Records Unit. Johnson reported Ardis Kelly has announced her retirement with today being her last day. With this retirement, plans for Public Health record to be managed by Medical Records and eventually moved to that department are being reviewed. Plans are underway to restructure and reorganize Medical Records, but nothing is finalized at this time. More information will be shared with the Board as it becomes available. The Board recessed from 9:40 - 9:50 AM. RECONSIDERATION OF TOBACCO -FREE GROUNDS RESOLUTION TO ALLOW SMOKING IN PERSONAL VEHICLES When the Tobacco -Free Grounds Resolution was introduced, Wentz apologized that she was not aware of wording in the union contract that states, "St. Croix County's facilities provide for a smoke -free environment. Employees must leave the facilities if they choose to smoke." It is the union's stance that the resolution changed the contract language without bargaining the change and do not believe a county board resolution trumps the union contract. A grievance has been filed on the Tobacco -Free Grounds Resolution. The Health and Human Services Board initiated the resolution and Johnson stated that he felt this should be addressed after reading of the action taken by the Finance Committee. Timmerman recommended negotiating with the union to try to reach agreement and then a possible future smoking resolution could be adjusted based upon those negotiations. Hartman recommended a meet and confer to possibly resolve the issue; however, implement non - enforcement with AFSCME employees only until the issue is resolved. Timmerman said the contract cannot be superseded by county resolution and no formal disciplinary action is occurring at this time. Concerns have been raised with county parks and jurors. Peterson recommended sending the issue to the Negotiating Committee for the County Board and that we follow the formal negotiation procedure. A tentative special meeting of the Health and Human Services Board was set for March 12, 2010 at 8:30 AM regarding the Tobacco -Free Grounds Resolution. Hartman, Funk and Whiting will try to meet and confer before the March 12th meeting. Rasmussen expressed concern to make a policy that is specific to those represented by the union as the resolution was created to look at the welfare of all people in St. Croix County. FAMILY CARE AUDIT REQUEST Beckman reported there will be an audit on Family Care and IRIS soon so there is direction on how to expand Family Care in the next budget process. This decision was made recently and as details of the audit process are identified, this will be shared with the committee. This is one mechanism to evaluate Family Care from a financial and operational perspective. STATE AND LOCAL EVALUATION PROCESS FOR ADRC, COMMUNITY HEALTH PARTNERSHIP AND SELF - DIRECT SERVICES (IRIS A provider forum was held in Madison on 01/22/10. Many different advocacy groups were involved and feedback was provided on Care Management Organizations (CMO's) and some issues that have developed. While this is not necessarily a formal State review, it appears to be part of an overall evaluation of long term care reform. One issue that came up at the forum was that some functional re- screens done for clients have had a large disparity when compared to the original functional screen. The impact of this can be significant for the consumer as a reduction in services could occur. St. Croix County Health and Human Services Board Page 9 February 17, 2010 This is also a concern for providers, such as St. Croix Industries, as it is anticipated that rates may be lowered or utilization of services may decrease. In addition, guardians are not always aware a re- screen is being done, so they do not have the opportunity to provide input. Some clients are being moved away from the areas they have been residing, some out of county, and have lost their community jobs in the process. Jennifer Borgh, an Ombudsman from Rice Lake, recently gave a presentation at the ADRC. Clients and guardians can contact her if there are problems with Family Care or IRIS, such as those discussed, and she will investigate the issue. The ADRC is also working on a customer service survey for clients. The State will also do a customer service survey with St. Croix County being designated as one of the pilot counties. The Managed Care Organization (MCO) do conduct random membership surveys for customer satisfaction. In addition, CHP has started a member advisory group meeting and will meet for the first time tomorrow. IRIS will soon start a state - sponsored survey also. Beckman said a new ADRC Senior and Disability Service Directory will be available in March. BADGERCARE PLUS BASIC PLAN Squyer reported the BadgerCare Core Plus Plan applications were closed in October, 2009. The State has been working on an alternate plan for people on the waiting list for the Core Plan. She reported 64,000 people applied for the Core plan and an additional 20,000 are on a waiting list. A press release from the Governor regarding the BadgerCare Plus Basic Plan was issued; however, this plan has not passed legislation. Squyer outlined some of the basics of the plan such as the income limit to qualify is below 200% of the Federal Poverty Level. The premium is $130 /month plus co- pays. If a premium payment is missed, the client is placed in restrictive re- enrollment for 12 months. Squyer reported Westfields Hospital opened a Health Station at Family Fresh on January 18 The walk -in clinic is open seven days a week. All major insurance is accepted and most services are available for around $55.00. ECONOMIC SUPPORT STAFFING One staff person left Economic Support for a position in Medical Records. A new staff person posted into the Economic Support position and is scheduled to start in that position on February 22. The new person will have to go through three months of training before they are able to taken on cases. The state has lowered the income limits for Energy Assistance so more clients can apply and lower amounts of assistance will be given. Since October 1 1,358 applications for Energy Assistance have been received and it is expected an additional 250 to 300 applications for crisis energy assistance will be received in the near future. The state has taken 780 cases but St. Croix County still receives the phone calls for those cases and Economic Support must continue to set up and pay for the drug testing for those cases. Child Care eligibility was being done by Work Force Connections (WFC) , along with the initial application and reviews. Now Economic Support has been instructed to do the reviews on 200 child care cases every 6 months and WFC will only do the initial application. All of this was added to Economic Support without any additional funding and has offset the additional staff time that was gained when child care certification was transferred to Family and Children's at the beginning of 2010. The case count for Economic Support as of today is 4,311. Overtime has been authorized for current staff, but the unit cannot wait for the budget process in June or July to hire an additional staff. St. Croix County Health and Human Services Board Page 10 February 17, 2010 Motion A motion was introduced by Rasmussen, seconded by Grant, and unanimously adopted: That the Board supports hiring an additional staff in Economic Support and to forward the request to the Finance Committee. PERSONNEL ISSUES NEW EMPLOYEES /VACANCIES Johnson announced Anne Salamone has filled a vacant Adult Community Support Services (ACSS) social work position. He said Ardis Kelly submitted her retirement effective today. He also announced Mary Squyer has announced her retirement effective May 21, 2010. SPRINGSTED JOB CLASSIFICATION AND SALARY STUDY Feedback from the HHS departments regarding the study has been channeled to Tammy Funk. Administration was given access to the job descriptions yesterday. Whiting said the information from Springsted has been received and nothing is in place yet for review of positions with Springsted. He said the study would provide a road map of how to go forward. Whiting said we will have to clarify who makes decisions, the pay plan, new positions and reclassification of positions. Timmerman evaluated data regarding study result and reported factual errors in the data. The Finance Committee is scheduled to meet February 18 in preparation for the full County Board meeting regarding the Springsted Study on February 23 rd . HUMAN SERVICES LOBBY DAY -WCA MEMORANDUM Johnson distributed information regarding Human Services Lobby Day to be held on March 4, 2010. The day is intended to educate members of the Legislature on the impacts of increasing mandates and decreased funding has had on counties' abilities to provide human services programs. Johnson plans to attend and invited Board members to contact him if they would like to attend. CLOSED SESSION PER WISCONSIN STATUTES 19.85 (1) (C) FOR DIRECTOR'S PERFORMANCE EVALUATION. RECONVENE IN OPEN SESSION Motion A motion was introduced by Peterson, seconded by Grant, That the Board enter into closed session per Wisconsin Statutes 19.85 (1) (c) for director's performance evaluation.
