St. Croix County Health and Human Services Board Page 2 October 20, 2010 MINUTES Motion A motion was introduced by Schrank, seconded by Larson, and unanimously adopted: That the minutes of the 09/15/10 and 09/24/10 Health and Human Services Board meetings be approved as distributed BUSINESS PLAN REPORT FROM LARSONALLEN FOR ST. CROIX HEALTH CENTER Johnson highlighted that the development of a business plan for the Health Center has been discussed at the Ad Hoc Facility Planning Committee to compare costs for various facility options. Johnson also highlighted that questions were raised at the October 5 County Board meeting regarding nursing home planning and potential reduction in levy for the 2011 budget. The time frame to have this report completed was moved up so this information could be provided to the County Board at the October 2e meeting. Trenton Fast from LarsonAllen discussed several considerations that were reviewed when creating the business plan. This included the market demand analysis for senior housing that was done about a year ago and a market analysis of outpatient therapy, which is still in process. He looked at the organization and its ability to take on debt and created some projections that drive the affordability of a replacement facility, but this process did not include a full operational assessment or examine financial forecast. The report was created on assumptions such as consideration of the County tax levy ending in 2015. The baseline for projection through 2019 was the current 2011 budget request. He said the report reflects a baseline assumption there will be no significant change in Supplemental Payment Program (IGT) Revenue, there will be 1% to 2% annual increase in payment rates from all payors and there will be a 2% to 3% overall increase in operating expenses. Fast compared St. Croix County numbers with 15 other governmental homes in the area and while costs are higher at the Health Center, the staffing ratios for nursing are consistent with what would be recommended by LarsonAllen. Fast presented the following scenarios: • Current facility with 72 beds • Current facility with 72 beds plus 30 unit housing project (assisted living) • New nursing home facility with 72 beds • New nursing home facility with 50 beds • New nursing home facility with 72 beds plus 30 unit housing project (assisted living) • New nursing home facility with 50 beds plus 30 unit housing project (assisted living) Details of these projections may be viewed on the powerpoint presentation at the end of these minutes. Fast noted that none of the options move the Health Center off County levy. This report will be presented to the full County Board on 10/26/10. The Board recessed from 9:35 — 9:45 AM. FINANCIAL REPORTS Kusmirek indicated that the financial reports have a modified format. More work is needed on the reports as the whole year needs to be rebuilt as to what was filed and what still needs to be filed. Staff are working through these issues and will report back to the Board. The following financial report for Health and Human Services through August, 2010 was distributed. Kusmirek indicated the Cores and CARS reports have not been filed since July. Filing will be held until we can get a better handle on actual expenses that should be submitted on a monthly basis in an effort to stay away from only doing this as a year -end reconciliation process. She indicated a final reconciliation is usually done on March 31 st with payment received in July and efforts will be made to shorten the reconciliation timeline so the audit report can be completed earlier in the year. The `all other sourcess' line item was reallocated to more accurately reflect Medicaid/Medicare revenue. A one -time insurance dividend was also received and is noted under other financing sources. Meters to monitor utility usage have been installed in the Nursing Home, Health and Human Services and St. Croix Industries; however, it is not clear what the direct impact of these meters will be on the financial operations, but projections were made based upon prior audited amounts for utility expense. Allocation for utilities was previously configured on square footage and now will be done by meter readings. Tobin indicated Xcel installed the sub - meters. He said an audit of the meters was done and four meters were unaccounted for. Credits will be received for the farm and the tenant house. He indicated capital purchases for the Nursing Home have been put on hold. S 194 $ 359 Expected Precent of Budget 66.7% FINANCIAL DEPARTMENT WORK PRIORITIES: It is hoped to eventually have year end projections in all departments. However, at this time the priority is to work on State reporting of expenditures and adjust practice to reconcile this reporting on a monthly basis rather than at year end as discussed earlier. Additional priorities include changes in the budget format which will be done in conjunction with the auditors. AGED HEALTH AND HUMAN SERVICES RECEIVABLES REPORT: The aging Accounts Receivable report for Health and Human Services through August was reviewed. There were no significant changes in Accounts Receivables. HEALTH CENTER PRIVATE PAY SUBSIDY: Discussion regarding private pay subsidy as the Nursing Home private pay rate is less than daily operating expense. Fast covered the subsidy in his report. The subsidy is $46.20/day. Johnson indicated the Board may want to look at an increase in private pay rates to lower this margin but remain competitive. The private pay rate will increase to $185.38 effective 11/01/10. The Nursing Home averages 18 private pay residents per month. Private pay residents are charged an additional $15/day if they reside in a private room. Robinson offered to survey private pay rates again as many facilities indicated they would increase their rates in January. Nursing Home private pay rates will be discussed again in six months. St—Croix County Health and Human Services Board Page 8 October 20, 2010 NURSING HOME ADMINISTRATOR'S REPORT Robinson said the average monthly census for August was 63, with 9 Medicare, 34 Medicaid, 18 private pay, 1 insurance and 1 Family Care residents. The Medicare average daily rate paid is $460.00 compared to the budgeted amount of $426.00. Robinson said census is down which is consistent with other facilities in the area. In January Robinson reported to the Board regarding a self -report that was made to the State of Wisconsin regarding a resident that was not transferred by the care plan and the resident was injured. The situation was handled correctly with follow up actions by the Nursing Home and the State acknowledged that in their findings. In June Robinson reported a Level D citation was received. Last week a fine of $1,400 was received due to this citation; however, the fine would be reduced to $965 if paid in 10 days. Johnson approved the early payment and savings to the County. The State surveyors praised the facility for the self - reporting. Robinson said occupancy has been impacted by hospital swingbed stays. Robinson said the St. Croix Health Center remains a 5 -star quality facility due to the exceptional staff in the facility. The report was issued September 1, 2010. Robinson said the Nursing Home is within reasonable ranges per patient day (ppd) costs. Robinson reported the Nursing Home is managing operational, overtime and pool costs and the things that can be controlled are being controlled. Staffing is above state level based on acuity level of residents and resident needs. PERSONNEL ISSUES NEW EMPLOYEESNACANCIES Johnson distributed a vacancy report for October. Johnson reviewed the hiring freeze process established at the October 5, 2010 County Board meeting. Johnson indicated it was his understanding that temporary, on -call positions and contracted positions were outside the realm of the hiring process. Funk agreed with this explanation. Johnson said St. Croix Industries has a need for a contracted job developer position to provide positions in the community. There has been an increased push from the State to work to increase positions in the community. This position would work in concert with the current job developer to develop jobs in the community. It is the feeling this position would generate enough money to pay for this position. Johnson indicated the front desk receptionist went on FMLA on short notice and it is anticipated this person may not return. Johnson would like to move forward with filling this position permanently versus continuation with a long term temporary position. Motion A motion was introduced by Schrank, seconded by Berenschot, and unanimously adopted: That the Board approves going forward with the hiring process of the front desk receptionist position to the Administration Committee and County Board if this staff person does not return. The position will not be filled if the staff person returns. Johnson said a centralized registration unit is being created which will result in some shuffling of temporary staff. Johnson reviewed three CNA (Certified Nursing Assistant) positions open in the Nursing Home. One position is 30 %, one is 20% and one is 15 %. St. Croix County Health and Human Services Board Page 9 October 20, 2010 Motion A motion was introduced by Marzolf, seconded by Larson, and unanimously adopted: That the Board approves going forward with the hiring process for the open 30 %, 20 %, and 15% Nursing Home CNA positions. Johnson reviewed on -call positions open in the Nursing Home. It is less expensive to use on -call versus pool staff. Motion A motion was introduced by Marzolf, seconded by Schrank: That the Board approves filling the nine on -call positions in the Nursing Home. Shrank and Marzolf withdrew the motion. It was the consensus these positions do not need to go to Administration Committee as if the positions do not work, they receive no pay and have no benefits. HEALTH AND HUMAN SERVICES BYLAWS Johnson will be scheduling a meeting with Corporation Counsel regarding the Bylaws to assure that they are consistent with the new County Bylaws and the changes that resulted from downsizing the size of the County Board. Johnson recommended that the HHS bylaws be adjusted to include routine reporting from subcommittees. Johnson will get revisions to the Bylaws to Board members before the next meeting. ST. CROIX INDUSTRIES FUNDRAISERS St. Croix Industries held a fundraiser at Willow River Inn - Carbone's. It was very successful with over 350 people attending and clearing approximately $6,000 that will be used for client summer picnic, client holiday parry in December, and other client activities throughout the year. ST. CROIX INDUSTRIES PRELIMINARY VOCATIONAL AND TRANSPORTATION UTILIZATION BY COMMUNITY HEALTH PARTNERSHIP FOR 2011 Schroeder reported 85% of St. Croix Industries funding comes from Community Health Partners (CHP). Paul Cook is the CEO of CHP. Cook has met with Johnson and Schroeder regarding adjustments in utilization and participating in a risk - sharing agreement. CUP lost approximately $11 ,000,000 in 2010 and is anticipated will be short an additional $4,000,000 for 2011. The state apparently is no longer supporting a risk - sharing agreement with CUP in 2011 and cuts are needed. To balance the $4 million shortfall, service utilization needs to be cut as much as 65% for prevocational services. Clients now served five days a week could drop to two to three days a week with the cut. Cook assured that service adjustments would not impact health and safety, but quality of life of individuals and the ability to meet individual outcomes are a concern. If fiscal goals are not met, CHP could be decommissioned and the state will go to the back up plan, which is not yet in place by the Insurance Commission. The capitation rates CHP receives are based upon utilization rates in pilot counties. Consumers are being educated regarding their options. Johnson offered to create a resolution to go to County Board from the HHS Board to advocate for changes in funding. It was the consensus of the Board that we need to do what can be done to meet the needs of the consumers of St. Croix Industries. A resolution will be created in conjunction with Greg Timmerman. The Board requested a calculation of the impact on St. Croix Industries if CHP does not fund clients. St. Croix County Health and Human Services Board Page 10 October 20, 2010 Schroeder announced there is a candidate forum on 10/27/10 from 6:00 to 8:30 PM in the rotunda of Sacred Heart Hospital. Schroeder plans to attend. ST. CROIX INDUSTRIES INTEGRATED EMPLOYMENT TRAINING AWARD Staff from St. Croix Industries were recently recognized for completing Integrated Employment training. The YMCA was given the employer of the year for employing five individuals from St. Croix Industries. REPORT ON NATIONAL REHABILITATION ASSOCIATION CONFERENCE Skramstad reported on attending the National Rehabilitation Association Conference and various information received. She reported St. Croix Industries will be redefined due to changes being made. She reported on good collaboration with the schools with smooth transition between school and supported employment. Skramstad reported she is investigating grants available to start small businesses and also grants to promote new programming. Skramstad is part of the Leadership Council that will work on problems /solutions. Efforts will continue to be proactive for St. Croix Industries. REPORT ON ECHO CONFERENCE Wilson, Strenke and Dupre attended the ECHO conference. Wilson reported on EHR (electronic health record) and ECHO compliance for HIPAA/HITECH. She reported e- scribing will be available the first part of 2011 and will be implemented soon. Strenke reported electronic billing will change after the first of the year due to Federal mandate changes. The conference walked through the forms that will be available. These forms will interface with billing versus having manual billing logs. Updates are being made in ECHO so that more than one user can see client notes at one time. `Travel' ECHO will be implemented so staff can work in the field without an internet connection and when back in the office, information will be synchronized with our database. An audit function is now out with test database and will go live in the next month. This function allows staff the ability to audit who viewed or worked on files within the ECHO system. Other highlights of updates include concurrent documentation, electronic signatures on documents, searchable data, and optical recognition. ECHO is constantly changing and updating. Johnson has assigned Steve Kirt as project manager for the department for the transition to an EHR. 2011 BUDGET REVIEW Johnson said the County Board meets on 10/26 regarding the 2011 budget. He reported HRA contributions for non - represented employees changes from $1,300 to $1,200 in 2011. Health insurance increase from 2010 to 2011 is approximately $848,000 for Nursing Home, St. Croix Industries and Health and Human Services. Johnson will report final budget numbers at the next meeting. WIC INFRASTRUCTURE FUNDING WIC applied for and was awarded infrastructure funding in the amount of $23,766 from the State WIC Office to improve clinic setting and/or services A list of possible items to purchase with this money that are allowable was given to WIC. Reyzer provided the Board with a list of supplies being purchased. St..Croix County Health and Human Services Board Page 11 October 20, 2010 Motion A motion was introduced by Berenschot, seconded by Rasmussen, and unanimously adopted: That the Board approves accepting the WIC Infrastructure funds and plan as presented. Reyzer reported vaccination clinics have been offered to the schools. Public Health is holding a clinic at St. Croix Central next week and has been to New Richmond and Glenwood City. Public Health staff are still in discussion with Baldwin regarding a clinic. COMPUTER DONATION TO NAMI (NATIONAL ASSOCIATION FOR THE MENTALLY ILL): A request for a donation of a computer to NAMI was received. Kusmirek indicated County policy needs to be followed to dispose of assets.
