PUBLIC HEARING
June 19, 1997
The St. Croix County Health and Human Services Board conducted an "Open Hearing" at the
St. Croix County Health and Human Services/Health Center complex in New Richmond,
Wisconsin.

Chair Stephens, called the Hearing to order at 5:30 P.M., noting that the meeting was properly
and legally publicized. Chair Stephens welcomed everyone to the Hearing and invited everyone
to speak and asked that everyone sign the sheet. Stephens stated, this is the fifth consecutive
year of "0" budget of county allocations. Stephens stated, "We do the best we can with the
money we have to meet your needs. We will plead with the County Board to increase the
budget so we can meet your needs without cutting programs, to cut those programs is to hurt
people and that would be the last resort. "
Borup stated this is the fifth year freeze of county allocations. We are still receiving overhead
increases without any additional funds. We are going to approach the Finance Committee to ask
them to increase the allocation.
Chair Stephens read several letters from clients to support the need of services.
Mike G. , resident of the nursing home again gave the highest praise for the staff in the nursing
home. He stated the labor contract is overdue, the people deserve it, the people work hard here.
We need to maintain what we do have. Mike also states he met people in physical therapy that
say this is the best facility they have ever worked at.
Glen W . , stated he has a learning disabled son and job support is a big issue for him. He will
be a productive member of society but will need these program to help to make him a productive
citizen.
Jaye H. , stated her brother is developmental disabled and lives at a group home, works at St.
Croix Industries. His job means everything to him. She pleaded not to cut services, to continue
funding these programs, if not increase them.
John S., stated he is a taxpayer who believes taxes should be increased if that would ensure
programs will not be cut. He stated if County Board members could hear what the needs are
they would increase the budget, they just don't know what the programs are. If we do not
increase in the dollars for services or willing to have an increase in taxes to ensure individuals
who need services would get those services.
Wanda V., St. Croix County Disability Coalition, Inc., stated respite care is needed and there
is a lack of respite in St. Croix County. Respite care is to give the care giver a night off, to
give that person a rest from the daily schedule/routine.
Lori M., stated they have a son who goes to a respite care home every two weeks for 48 hours.
Their son needs special care and equipment. With a limited amount of money that will be up
in respite care in a couple of months. This is an excellent program and sure hopes to see it
continue.
Lee K. , stated it bothers him to see the waiting list in Long Term Support. He would like to
have the committees handle the contingency fund instead of all of the dollars going into one
fund. It would be nice to have each board handle their own contingency fund.
Ray M., Chairman Long Term Support, stated staff have done a remarkable job in getting the
waiting list down. They have qualified people and qualified care.

Debbie W., appeals to the County Board/Finance Committee members to visit homes to see the
tremendous care these children receive from families, to spend a day with the families. Those
families would be glad to share a day with them. She states, let there be no freeze.
Shelly W. and Mike W., are the parents of a child in a wheelchair. The families have no
preparation for a child of special needs. They are advocates for respite, if you take care of the
parents, they can take care of the child and you have no idea the toll it takes on the family.
Raelle J., has a 16 y/o son in a wheelchair and feels blessed when her son got involved with the
respite care program. Taking care of these children can utterly drain you. The parents exist
in a state of chronic exhaustion. She stated the respite care program is a God send. They also
are on the family support program. They provide support and services, their home was
modified with a ramp on the front of the house. With family support funding, there were funds
available to raise the roof on the mini van. She is grateful we have these programs. She
strongly advocates whatever funding is available.
Chair Stephens asked if there was anyone else who would like to give testimony.
Stephens stated "we work for you," and closed by saying tl-iank you very much for coming.

Chair Stephens, called the meeting to order at 6:30 P.M. noting the meeting was properly and

NEXT MEETING: The next Health and Human Services Board meeting is scheduled for
Thursday, July 10, 1997 at 8:00 a.m. at' the St. Croix County Health and Human
Services/Health Center complex. Olson will review the vouchers at 7:30 a.m.
AGENDA: MOTION: A motion was introduced by Berenschot, seconded by Bethke, and

meeting be approved as circulated.
Report by Brian Schoeneck-Wisconsin Association of Homes and Services for the A�in�: Chair
Stephens welcomed Schoeneck to the Health and Human Services board meeting. Schoeneck
joined us to answer any questions and discuss financial matters regarding the nursing home
industry and Long Term Care Redesign. Schoeneck distributed an overview: 1997-99 Wisconsin
Budget Medicaid Rate Increases for Nursing Homes as it pertains to St. Croix County. For
direct care, nursing homes are currently reimbursed for direct care costs up to maximums under
the nursing home reimbursement formula. The direct care maximums are established at 110%
of statewide average direct care costs . The Governor reduced the maximums to 102 % . The
Joint Finance Committee increased the maximums to 103 % at an estimated cost of $3.2 million.
Schoeneck also discussed the minimum occupancy standard for Nursing Home Reimbursement.
St. Croix Health Center's 1996 occupancy was 80.87%. To avoid penalties in the 7/1/97
Medicaid rates, the Health Center would need to' bank 14 beds which would reduce the licensed
bed capacity from 129 beds to 115 beds. The estimated Medicaid loss if St. Croix Health
Center remains at 129 beds versus bed banking 14 beds and operating at 115 beds is $188,000.
Note: A portion of this loss may be reimbursed thru the Intergovernmental Transfer Program
(ITP) for 97/98. Horne asked what advantage would there be to decertify, if you can be
reimbursed. It is hard to make a decision on the impact of the facility, the earliest this Board
could make any decision will be August/September. Stephens stated we have no idea where we
stand until they get the budget complete in September. Krieger stated we are being penalized
for being efficient.
Schoeneck had a list of the counties that decertified beds. Horne asked what the rationale was
for them decertifying the beds.
Stephens stated each individual bed holds value. With the growth and aging in our county, we
will certainly needs those beds. The nursing home holds more value with 129 beds than with
14 less.
Schoeneck stated the Long Term Care Redesign is up in the air at the present time and set back
until at least April. There are a lot of concerns tying acute care with it. There are a lot of
issues not addressed at this point. They will continue to revise the plan and move forward. The
implementation will take a number of years.
Stephens thanked Schoeneck for his presentation.
Wisconsin Works (W-2) State Start-up Funds: Fruit stated this is informational only, we are
close to ordering the equipment. The actual bids were lower than budgeted by $18,000. We
are ordering 17 PC's, 17 printers, and 1 lap top for the FEP's who will then be able to connect
into the system when out of the office. The terminals we have now were installed in 1990.
Stephens thanked Fruit for keeping the Board informed.
Women Infant and Children (WICI Pro rg am Update and Resolution to Support Continued State
Fundin Hansen stated State funding for WIC is projected to be cut 11 % in the next fiscal
year, resulting in projected waiting lists in 13 of 19 Western Wisconsin counties. WIC provides

nutritional counseling and specific foods only to pregnant, breast feeding or new mothers with
children under age 5 who meet the program's financial guidelines. With W-2 and welfare
reform, Administration is concerned about the future of this important program. A resolution
supporting no cuts in funding WIC is proposed. Olson questioned if any of the mothers who
receive WIC were smokers. Hansen stated there are about 35 % that do smoke upon admission,
but by the termination date the percentage is about 20%. Discussion followed.
MOTION: A motion was introduced by Bethke, seconded by Berenschot, Motion passed with
Olson and Horne opposing: That the Health and Human Services Board approve to pass the
resolution supporting no cuts in funding WIC.
Personnel Issues: Job description updates: Due to elimination of the Deputy Director's position
and further efforts to cut costs, the following job descriptions were reviewed: 1) Mental
Health/Alcohol and Drug Abuse Coordinator; 2) Long Term Support Coordinator; 3) Mental
Health/Alcohol and Drug Abuse Clinical Services Supervisor; and 4) Social Work Supervisors.
Borup stated these four job descriptions effect 5 people. Borup stated most departments of this
size have a Deputy Director. Karen Hall, grade 10 officer, with this job description change
would not be the Deputy Director, but would serve as Acting Director in the Director's absence.
MOTION: A motion was introduced by Horne, seconded by Olson, and unanimously adopted:
That the Health and Human Services Board approve the above listed job description updates to
go on to the Personnel Office for a "Hay" System Review.
Borup stated with the reduction of positions, we are looking at the 1998 budget to possibly
reinstate a vacated clerical support position. Borup stated in the last five years there have been
a lot of positions eliminated. The Board askedithat a list of all the positions eliminated over the
past five years be brought to the next meeting.
Health and Human Services/Nursing Home Report: Borup distributed a list of the Nursing
Home and Health &Human Services increases mandated to us. Union arbitration on 06/10/97
has resulted in:
1997: salary increase of 3 % + $.25/hour or $.15 first year, $.10 second year (depending
on Bargaining Unit)
1998: salary increase of 3 %
1999: salary increase of 3 %
We can't cut mandated services so this will be a big challenge for the County Board. Olson
requested a list of all the mandated and non -mandated services be brought to the next meeting
so we can distinguish between the two.
Krieger stated the nursing home census is at 105.
Borup stated he was contacted by Bill Rubins, Economic Development, requesting to purchase
acreage west and to the north for a railroad spur. Bethke stated the land may be in CRP,
approved the first 8 acres, denied 17 acres. We have 30 days to look at this. Borup to make

arrangements and get back to Bethke who will go with Borup.
Stephens asked if there were any other issues to bring before the Board.
Van Dyk is moving another herdsman here.
Olson stated our flag needs to be replaced before the centennial in August.
Horne stated he would like the W-2 Steering Committee to look at not meeting every two weeks
as he is not able to meet that often.
Horne asked about the computer system and if Accounts Receivable has been able to get the
statements out. Borup stated we think we have turned some corners, working with Computer
Services, we have hired a consultant for computers. The problem was in the dual processors
that were not configured properly. We have had no computer lock -ups in the last eight days.
Horne stated that someone needs to take responsibility to keep things running. Borup stated that
we are and the system is working better.
MOTION: A motion was introduced by Berenschot, seconded by Bethke, and unanimously
adopted: That the Health and Human Services Board meeting be adjourned.

RESOLUTION
WHEREAS, the State of Wisconsin Women, Infant and Children Supplemental Nutrition
Program (WIC) program is serving approximately 69% of the total eligible population
while there are federal funds available to provide food supplements for approximately
72% of the population, and,
WHEREAS, it is expected that the amount of funds available to cover the local costs
associated with client enrollment, nutrition counseling and distribution of WIC vouchers
in St. Croix County will be reduced in 19974998 by approximately 7.2% (or $7000); and
WHEREAS, in St. Croix County this means that there are approximately 50 pregnant
women, infants and breastfeeding mothers who will not receive food supplements even
though federal funds for food are available; and,
WHEREAS, for each $1.00 spent for WIC nutrition services for mothers and children
there are $3.00 saved in medical costs; and,
WHEREAS, the State of Wisconsin has provided funds in the past to assure participant
enrollment, education and distribution of all federal funds available for WIC food.
NOW THEREFORE BE IT RESOLVED, that the St. Croix County Department ofHealth
and Human Services Board hereby encourage Wisconsin Senator Alice Clausing,
Representative Sheila Harsdorf, Representative Joe Plouff, Governor Tommy Thompson,
US Senators Herb Kohl and Russ Feingold, and US Representative Ron Kind to make
$1,500,000 available to do health screening and nutrition counseling necessary to assure
the distribution of all the food made available in Wisconsin for the WIC program; and,
BE IT FURTHER RESOLVED, that this resolution be sent to other Wisconsin Counties
and that the Wisconsin Counties Association encourage efforts to fully maximize the use
of Federal funds for WIC in Wisconsin.
