QUESTIONNAIRE
BACGROUND: The Human Resource Committee has recently noticed a drop in
attendance. This questionnaire is an effort to determine the best time,
Location, program etc. to endeavor to make the committee worthwhile.
DIRECTIONS: Please take a moment to read the questions below and indicate
what would be best for your situation. Please return to Tom Holland ASAP.
1. Currently, the committee meets every other month; the first Wednesday
from 1:30-3:30 pm. Please indicate what frequency and time you would prefer.
