PROGRAM HOURS - 10:00 a.m. - 2:00 p.m. on Fridays I10C.A2�ON -Woodville Senior Center 102 S . Main St., Woodville, WI 54028 IS 1`lIM PROGRAM FOR YOU? The answer is "yes" if you and the one you care for fit these descriptions: * FAMII�Y CAREGIVERS are spouses, adult children, relatives or unpaid friends who provide regular assistance to an older person who is forgetful. The program provides caregivers with temporary relief from caregiving--and offers opportunities for free time aimed at balancing caring tasks with personal well-being. * PROGRAM PARTICIPANTS are adults with mild to moderate levels of memory loss due to Alzheimer's disease, stroke, Parkinson's disease or related disorders. They are also persons who: * have no chronic or acute illness that is contagious --and no health program which limits ability to take.part in program activities. * are mobile --or capable of managing their own mobility and transfer needs with little assistance. * are bowel continent --and bladder continent or wearing protective devices. * are not harmful or abusive to themselves or others. DayAway Clulb offers members . opportunities to .enjoy new friendships in a safe, social setting. Activities --involving music, games, exercise, conversation, or crafts --are planned with individual needs and interests of group members in mind. And they are led by people who care. * Group Size & Staff Ratios - P The program serves no more than 10 A 1 * 3 ratio of staff to members is * Fees & Transportation - Fees of $15.00 per session help defray costs of enrollment, professional and volunteer staffing materials and meals. People who work with participants and family members values of individual dignity, respect and security in high regard. All program personnel receive extensive ni traing as well as on -site supervision. * TO APPLY, SIlVIPLY FOLLOW THESE STEPS: 1. Call the Department on Aging, 381-4360 or 1 (800) 372-2333 to ask question, request information or application forms, or to refer potential participants . 2. Caregiver completes.. application/enrollment forms. 3. Health care provider/doctor completes medical information form. 4.
