The submittal described above has been reviewed for conformance with applicable Wisconsin Administrative
Codes and Wisconsin Statutes. The submittal has been CONDITIONALLY APPROVED. This system is to be
constructed and located in accordance with the enclosed approved plans and with any component manual(s)
referenced above. The owner, as defined in chapter 101.01(10), Wisconsin Statutes, is responsible for
compliance with all code requirements.
No person may engage in or work at plumbing in the state unless licensed to do so by the Department per
s.145.06, stats.
The following conditions shall be met during construction or installation and prior to occupancy or use:
Reminders
This approval recognizes the submitted plans as an Individual Site Design that does not conform to the
approved "In -Ground Soil Absorption Component Manual, SBD-10705-P (N.03/01, R 10/12)". That is,
the component manual does not allow the installation of in -ground systems on slopes >25%. This
proposal has the cells on a 26.7% slope. There is an existing dwelling on the property served by a failing
septic system which needs to be replaced. There is no way to avoid installing the POWTS in a steeply
sloping area. The installer shall follow the plan directives laid out in the approved plan. The installer is
responsible for installing this system in a safe and responsible manner to maintain the health and
safety of personnel and equipment. The installer is responsible for ensuring the installation shall
prevent seepage and breakout of wastewater moving laterally downslope of the cell.
a Verify and adjust for uniform distal head pressure at each lateral to ensure equal distribution.
• Two ball valves are being installed on force mains to both systems (the old and the new). Provide
instruction in the management plan about how to operate these valves and at what times they will be
turned. These valves must be accessible and protected from freezing. In addition, valves can greatly
impact head loss. Be sure the pump installed operates within it's designed specifications after
installation.
• Ensure the existing septic tank is watertight, structurally sound and baffles are in place. Any changes
made to the tank must be approved by the manufacturer.
• A sanitary permit must be obtained from the county where this project is located in accordance with the
requirements of Sec. 145.19, Wis. Stats.
• Inspection of the private sewage system installation is required. Arrangements for inspection shall be
made with the designated county official in accordance with the provisions of Sec. 145.20(2)(d), Wis.
Stats.
• A state approved effluent filter is required. Maintenance information must be given to the owner of
the tank explaining that periodic cleaning of the filter is required.
• A copy of the approved plans, specifications and this letter shall be on -site during construction and
open to inspection by authorized representatives of the Department which may include local
inspectors.
Owner Responsibilities
The current owner, and each subsequent owner, shall receive a copy of this letter. Owners shall also
receive a copy of the appropriate operation and maintenance manual(s) and be responsible for ensuring
that POWTS is operated and maintained in accordance with this chapter and the approved management
plan under s. SPS 383.54(1).
• In the event this soil absorption system or any of its component parts malfunctions so as to create a
health hazard, the property owner must follow the contingency plan as described in the approved plans.
• The owner is responsible for submitting a maintenance verification report acceptable to the county for
maintenance tracking purposes. Reports shall be submitted at intervals appropriate for the
component(s) utilized in the POWTS.
In granting this approval the Division of Industry Services reserves the right to require changes or
additions should conditions arise making them necessary for code compliance. As per state stats
101.12(2), nothing in this review shall relieve the designer of the responsibility for designing a safe
building, structure, or component.

and any others who are responsible for the installation, operation or maintenance of the POWTS.

EXISTING POWTS DESCRIPTION:
The existing soil absorption area consist of 3 Infiltrator Quick 4 trenches that are
dosed. The trenches have failed as evidenced by effluent seeping out around the
observation pipes when the pump cycles.
PROPOSED POWTS DESCRIPTION:
The existing soil absorption area is on a slope of greater than 25%. The
proposed replacement soil absorption area will lie on a 26.7% slope. Due to the
location of the well and a drainage easement on the property, this the best option
available. The new cells will be installed with approximately 48 inches of cover on
them. The depth of cover along with pressure distribution will keep the effluent
from leaking out Each lateral will be identical other than the elevation, which will
be dealt with by using ball valves to make all the squirt heights equal.
The trenches will be installed by shelving along the hillside to make the
installation safer. The back fill will be done with a track skid steer with greater
stability than most machines

PL-525 Effluent Filter
PI -525 Filter
The PL-525 Filter is rated for 10,000 GPD (gallons per day) making it one of the largest filters in its class. It has
525 linear feet of 1/16" filtration slots. Like the Polvlok PL-122, the Polylok PL-525 has an automatic shutoff ball
installed with every filter. When the filter is removed for cleaning, the ball will float up and temporarily shut off
the system so the effluent won't leave the tank.

1'I -SP Maintenance.
The PL-525 Effluent Filters will operate efficiently for
several years under normal conditions before requiring
cleaning. It is recommended that the filler be cleaned
every time the tank is pumped, or at least every three
years. if the installed filter contains an optional alarm,
the owner will be notified by an alarm when the filter
needs servicing. Servicing should be done by a certified
septic tank pumper or installer.
1. Locate the outlet of the septic tank.
2. Remove tank cover and pump tank if necessary.
3. Do not use plumbing when filter is removed.
4. Pull PL-525 cartridge out of the housing.
5. Hose off filter over the septic tank. Make sure all
solids fall back into septic tank.
6. Insert the filter cartridge back into the housing making
sum the filter is properly aligned and completely inserted.
7. Replace and secure septic tank cover.

Installation Instructions for
EZflow Systems in Wisconsin
Wisconsin Department of Commerce, Safety and Buildings
Division, has reviewed the specifications and/or plans for this
product and determined it to be in compliance with chapters
Comm 82 through 84, Wisconsin Admin. Code, and Chapters
145 and 160, Wisconsin Statutes. All sites must meet the Site
& Sod Conditions & Locations & Isolation distances as noted in
local regulations,
The approved products are 1203H (3-12' bundles with pipe in
center bundle in 5' or 10' lengths) and 1203HP (3-12' bundles
with pipe in each bundle in 5' or 10' lengths.
A single pipe bundle contains a four inch perforated pipe sur-
rounded by EPS aggregate and is held together with poly-
ehtylene netting. A single aggregate bundle contains aggregate
only and is held together with polyethylene netting.

Installation Instructions
The instructions for installation of EZflow products are given
below. This product must be installed in accordance with state
rules defined in chapters Comm 82 through 84, Wisconsin Ad-
ministrative Code, and Chapters 145 and 160, Wisconsin Stat-
utes, as well as the local health department's current design
manual.
1. After the local health department has determined sizing,
configuration, and layout for the EZflow systems, stake
or mark with paint the location of trenches and lines. Be
careful to set correct tan., invert pipe, header line or dis-
tribution box and trench bottom elevations before instal-
lation of pipe bundles.
2. Remove plastic EZflow shipping bags prior to placing
bundles in the trench(es). Remove any plastic bags in the
trench before system is covered.
3. This product must have geotextile fabnc that meets re-
quirements of s. Comm 84.30 (6) (g), Ws. Adm. Code,
installed directly on top of the product and extending
down along the sides of the product to a point at least six
inches from the bottom of product.
4 When installed In a trench, the trench should be dug to
a width of 36 inches. This not only saves labor in excava-
tion, but also provides better load -bearing capacity after
backfilling is complete.

5. The Absorption area (SF) necessary for a given site shall
be sized based on maximum daily sewage flow (GPD) and
the Permeability for the site. If certain criteria is met, the
EISA sizing can be used in Wsconsin, resulting in a 40%
smaller drainfield.
6. Place EZflow bundles) in the EZflow configuration ap-
proved by system design permit specified for the parbcu-
tar site. The top or center -most bundles containing pipe
are joined end to end with an internal pipe coupler. Any
additional aggregate only bundles that may be required,
• should be butted against the other aggregate -only bun -
dies and do not require any type of connection.
7. The top of each GEO cylinder contains a fitter fabric pre -
manufactured in between the netting and aggregate. The
fabric is inserted to prevent soil intrusion. The installer
shall make sure the the GEO is positioned upward and is
in contact with the fabric contained in the adjacent cylin-
der before backfilling.
8. The EZflow Drasnfield Systems should be installed in a
level trench In all directions (both across and along the
trench bottom) and should follow the contour of the ground
surface elevation (uniform depth), with all continuous
adjoining 10-foot cylindrical bundles placed end to end,
with central bundle distribution pipe interconnected,
without any dams, stepdowns or other water stops.
9. The trench top shall be graded such that water will not
pond. Backfill should be seeded or sodded immediately
after completion to reduce erosion.
10. EZflow EPS bundles are flexible and can fit ,n curved
trenches as may be necessary to avoid trees, boulders, or
other obstacles.
11_EPS aggregate is lighter than water, therefore, it might
be expected that natiral buoyancy forces would tend to
cause EZflow assemblies to float out of ground when
bonding occurs. Field experience has shown, however,
that this is not a problem when systems have a minimum
of 6' of soil cover as recommended by manufacturer.

The owner of this in -ground pressure system shall be responsible for its perpetual operation and maintenance pursuant to
requirements of SPS 382-384, Wisc. Admin. Code. Pursuant to SPS 383,52 (2), Wisc. Admin. Code, this system shall be
considered a human health hazard if not maintained in accordance with this approved management plan. Furthermore,
all inspection and maintenance activities shall be performed by a registered POWTS Maintalner in accordance with SPS
383.52 (3), Wisc. Admin. Code.

No product for chemical or physical restoration of the POWTS may be used unless approved by the department in
accordance with SPS 384, Wisc. Admin. Code.
Contingency Plan
In the event that any failed treatment component of this POWTS cannot be repaired, it shall be replaced pursuant to
a plan submitted to the appropriate agency for review and approval. A failed in -ground dispersal component may be
abandoned and replaced by a code -complying dispersal component in a pre -determined area of suitable soils.
System Abandonment
If use of this POWTS is discontinued, it shall be abandoned in accordance with SPS 383.33, Wisc. Admin. Code.
