Maintenance Required
The submittal described above has been reviewed for conformance with applicable Wisconsin Administrative Codes
and Wisconsin Statutes. The submittal has been CONDITIONALLY APPROVED. "this system is to be
constructed and located in accordance with the enclosed approved plans and with any component manual(s)
referenced above. The owner, as defined in chapter 101.01(10), Wisconsin Statutes, is responsible for compliance
with all code requirements.
No person may engage in or work at plumbing in the state unless licensed to do so by the Department per s.145.06,
stats.
The following conditions shall be met during construction or installation and prior to occupancy or use:
• It is recommended to fence off dicoersal area prior and durng con tit ruction to asoid duht rba nce,
compaction and use of the site.
• NN ith new construction, it is recommended not to acticale the pump in the dose tank until the tanks are
pumped prior to homeowner occupanc.N.
• NN astewater generated from contractors cleaning of equipment and tools and/or- left -over construction
products shall not be discharged into the drains discharging to the prisate onsite wastewater treatment
system (PONN Ts). NN aste generated shall be properly disposed of on -site or off site.
• Any tall grasses, leaves and shrubs shall be cut short and removed prior to tilling the surface for installation to
prevent matting under the dispersal area.
• Prior to construction of the dispersal area, check the moisture content of the soil to a depth of 8 inches.
Smearing and compacting of wet soil will result in reducing the infiltration capacity of the soil. Proper soil
moisture content can be determined by rolling a soil sample between the hands. If it rolls into a 1/4- inch wire,
the site is too wet to prepare. If it crumbles, site preparation can proceed. If the site is too wet to prepare, do not
proceed until it dries.
• It is recommended to cuter short M possible and remove ground cover prior to tilling the ground surface for
the dispersal area.
• Electrical connections shall comply with SPS 316.300 and NEC 300
• All piping shall conform to SPS Table 384.30-3 and SPS Table 384.30-5
• Insulate building sewers beyond 30 feet per SPS 382.30 (I1)(c)
• Well shall be located 50 feet from drainfield and 25 feet from treatment or pump tanks.
• Verify property line location.
• The homeowner must maintain a Servicing Contract with a POWTS Maintainer for the life of the private
onsite wastewater treatment system using pretreatment technology.
• The inspection, maintenance and servicing reports shall be submitted to the governmental unit within 30
calendar days from the date of inspection, maintenance, and servicing.
• The system was designed to meet the influent quality discharged into a POWTS dispersal component
consisting in part of in situ soil to be equal to or less than all of the following:
1. A monthly average of 30 mg/L BOD5.
2. A monthly average of 30 mg/L TSS.
A copy of the approved plans, specifications and this letter shall be on -site during construction and open to
inspection by authorized representatives of the Department, which may include local inspectors. All permits
required by the state, or the local municipality shall be obtained prior to commencement of
construction/installation/operation.
In granting this approval the Division of Industry Services reserves the right to require changes or additions should
conditions arise making them necessary for code compliance. As per state stats I01.I2(2), nothing in this review
shall relieve the designer of the responsibility for designing a safe building, structure, or component.
Inquiries concerning this correspondence may be made to me at the telephone number listed below, or at the address
on this letterhead.
The above left addressee shall provide a copy of this letter and the POWTS management plan to the owner and any
others who are responsible for the installation, operation, or maintenance of the POWTS.
Sincerely,
Travis Wagner

Plan Narrative
l"he following shared sanitary" system desig discussion foe scs on the proposed 12 Bedroom!
1800 gallon per day sanitary system to scree ots 33 th f the Rolling I tills Farm
Subdivision. This plan complies with the St. Croix County" L nd Use and Development
Ordinances. A "Conservation Development Rolling Hills Farm is designed for 77 lots serviced
by 22 dispersal components located on common ground. Appropriate casements and
maintenance language were required prior to final approval.
The St, Croix County subdivision ordinance requires that each buildable lot has a soil and site
evaluation completed. For these 3 lots the mound dispersal area will be located within Sanitar
Easement Area 10 and the tested areas for lot 61 within the common area known as Outlot 2.
A four bedroom maximum design flow has been established for each lot. The county will require
a sanitary permit for each lot prior to construction for setting the septic/treatment components
verifying the design flow for each lot. These lots will required to set an aerobic treatment tank
with built in trash capacity that will be connected to a 4" conveyance pipe that will gravity flow
to a common duplex dose tank located north of Lot 3Ss north lot line, that will alternately
distribute the highly treated effluent to a common mound dispersal system consisting of two
6.0 ft. x 100 ft. mound dispersal cells.
The soil tests completed by David Steel are provided. The Bl+l for the soil test for Lot 61 equals
100.00' or—1072.00' using NAVD 88 Datum. A surveyor was hired to locate the existing irons
and staked the septic easement area as the agricultural fields within the subdivision had
continued to be cropped and some of the irons and 'I -posts marking the lot comers had been
disturbed as it has been 15 years since the original soil tests had been completed.
Ae use a wooden hub set on the northeast comer of Septic Easement 410 with an elevation of
1072.52as our benchmark for the sanitar systems constructiorimeter of the
lease Went "as staked with shots taken on the existing easement iron. n soil verification pits
were du_* to verifi, the soil conditions and tied into the benchmark along with contours within the
system area

Single phase, duplex alternating pump control with override. Page 7a + b of 9
The Model 122 control penal is designed to aftematey control two 120, 208, or 240 VAC single phase pumps in water and sewage
installations. The alternating action equalizes pump wear. In addition to the altertrolleg pump control, this system provides override
control should either pump fail. If an alarm condition occurs, an alarm switch activates the audiovisual alarm system. Common
applications include pump chambers, sump pump basins, lrigabon systems and lift stations.
1. Enclosure measures 10 X 10 X 6 inches (25.4 X 25 4 X 15.24 cm).
Choice of NEMA 1 (steel for indoor use), or NEMA 4X (ultraviolet stabilized
thermoplastic with removable mounting feet for outdoor or indoor use).
Note: Options selected may increase enclosure size and change component
layout.
2. Magnetic Motor Contactors control pumps by switching elecincal tines.
3. MOA Switches for manual pump control (mounted on circuit board).
