AND NONPROFIT NURSING HOME AND /OR ASSISTED -LIVING BUSINESSES TO
SUBMIT PROPOSALS TO OPERATE A FACILITY AT THE COUNTY OWNED BUILDING

St. Croix County, Wisconsin
WHEREAS, that the County Health and Human Services Board recommends that a request for
proposal (RFP) be developed to solicit private and nonprofit nursing home and/or assisted - living
businesses to submit proposals to operate a facility at the County owned building; and
WHEREAS, the RFP shall be open to purchase or lease options and/or a public - private
partnership; respondents may propose a facility which accommodates multiple campus uses; and
WHEREAS, the Health and Human Services Board shall create a subcommittee consisting of
Chairman Wentz, Dr. Kretz, Gordon Awsumb and staff to draft the RFP with input from consultants and
any other party the subcommittee deems appropriate.
THEREFORE, be it resolved that the St. Croix County Board of Supervisors approves the RFP
process, the RFP to be issued by December 1, 2006 or as soon thereafter as practical and responses to be
received by February 15, 2007.
