CALL TO ORDER — The meeting was called to order at 2:36pm

1. Introductions — The following appointments were made by the ad hoc committee: Kjersti L.O. Duncan, Chair;
Norma Vrieze, Vice - Chair; Peg Gagnon, Secretary
2. Discuss purpose of ADRC
The following was reviewed:
- ADRC application instructions
- April 17, 2007 and August 21, 2007 County Board Long -Tenn Care Reform/ADRC Resolutions
- The following websites:

Kjersti will follow -up by contacting Laurel Kieffer (Kieffer Consulting & Facilitation -LLC) to grant Peg access
to the West Central Wisconsin — Care Management Collaborative (WCW -CMC) Base Camp for further
information regarding ADRC and Long -Term Care Reform initiatives in Western Wisconsin.
3. Discuss potential stakeholders — County Staff = Department on Aging Director; Long Tenn Support
Supervisor — Birth -3; Health & Human Services Director; Long Tenn Support Supervisor; Long -Term Support
Social Workers (Elderly and PD/DD); Elderly Benefit Specialist; Department on Aging Community Services
