CALL TO ORDER — The meeting was called to order at 2:30pm

1. Proiect timeline for planning activities in anticipation of October 2007 application
submission — Positive feedback was received regarding Janet Zander's presentation at the
September 26 ADRC forum. Forum surveys were reviewed. Discussion regarding proposed
organizational structure, including proposal to have ADRC separate from AgingAEIHS at this time,
recognizing ADRC development will be an ongoing process. Norma will be reviewing the
application, utilizing materials available on base -camp.
2. Possible Agenda Items for next meetings — Meeting dates were scheduled for the month of
October, including proposing to meet at Health & Human Services on October l l th .
ADJOURN — Motion to adjourn by Kjersti; Seconded by Peg. The meeting ended at 5:00pm

Members of the ADRC Ad Hoc Committee will be attending a presentation by Janet Zander,
Portage County Aging and Disability Resource Center Director. An invitation to this forum has
also been extended to members of the Council on Aging, Long -Term Support Board, and Health
and Human Services board. This is not a regular committee meeting. Committee members will
be in attendance to obtain information from potential ADRC consumers that may be used in
making future recommendations or decisions.
