Motion to accept previous meeting minutes by Leonard, second by Jacobs, motion carried. * * Special Meeting Date: Wednesday, December 13, 2006 at 1:00 pm in the Community Room, bottom level of the Government Center, Hudson Next Regular Meeting Date: Thursday, February 15, 2007 at 1:30 pm in Room 1281 at the Government Center, Hudson UNFINISHED BUSINESS: 1. The role of the Aging and Disability Resource Center in Long Term Care Reform: Power Point presentation from the State. Handout by Lockwood of Current and Future Funding of the Reformed LTC system. Handout of Medicaid Benefits comparison. Discussion. NEW BUSINESS: 1. Financial Report: Handout through September 2006 by Lockwood. Discussion. 2. Variance & Purchase Requests and COP high -cost requests: None at this time. 1 3. Nursing Home relocation requests: 3 requests for relocations currently. Motion to approve 3 Nursing Home Relocation Requests for persons to move to the community by Mortensen, second by Cranmer. Motion carried. Lockwood explained that there isn't any county money involved in these relocations. 4. Consider utilizing an integrated long term care management model in St. Croix County through Community Health Partnership, Inc.: This model integrates long term care services with acute and primary health care for persons with long term care needs. The Partnership model is currently operating in Eau Claire, Chippewa, and Dunn counties. Discussion. 5. Kjersti Duncan, Director, Department on Aging — Program Report: Duncan reported November is National Caregiver Month. Nancy Abrahamson is the Caregiver Support Coordinator. Department on Aging is collecting Secret Santa donations through 12/8/06. Items can be dropped off at the Department on Aging. There is art work done by seniors on display at the Department on Aging.
