CROIX COUNTY HEALTH AND HUMAN SERVICES BOARD June 13, 2006 The St. Croix County Health and Human Services Board met on June 13, 2006 at the St. Croix County Health and Human Services /Health Center complex. MEMBERS PRESENT: Leon Berenschot Lois Burri Bill Cranmer Dr. Michael Kretz Linda Luckey Deb Rasmussen Ron Troyer Esther Wentz MEMBERS ABSENT: Tom Caflisch, excused STAFF PRESENT: Fred Johnson, Health and Human Services Director Jennifer Pempek, Accountant /Fiscal Manager Bill Van Offeren, Nursing Home Administrator Kathy Leisch, Secretary OTHERS PRESENT: Wentz called the meeting to order at 4:30 PM noting the meeting was properly and legally publicized. NEXT MEETING The next Health and Human Services Board meeting is scheduled for Monday, July 17, 2006 at 8:30 AM. Rasmussen will review July vouchers. AGENDA Motion A motion was introduced by Kretz, seconded by Berenschot, and unanimously adopted: That the agenda be approved as circulated. MINUTES Motion A motion was introduced by Kretz, seconded by Burri, and unanimously adopted: That the minutes of the 05/15/06 Health and Human Services Board meeting be approved with the following additional explanations: Luckey suggested that when a correction is made to the minutes there be a better explanation as to what page, paragraph and subject is being corrected. ELECTION OF HEALTH AND HUMAN SERVICES BOARD OFFICERS Wentz stated that in talking with Johnson about the election of officers in other committees there has not been a second to the motion to nominate someone and she asked if that was agreeable with the other board members. Board members agreed. Wentz announced the election of officers would take place. Wentz asked for nominations for the position of Chair. Luckey nominated Wentz for the Chair position. Kretz requested to close nominations for Chair. Wentz requested a vote and a unanimous vote was made for Wentz to be Chair. Wentz asked for nominations for the position of Vice Chair. Burri nominated Berenschot for the position of Vice Chair. Kretz requested to close nominations for Vice Chair. Wentz requested a unanimous vote to be cast for Berenschot to be Vice Chair. Wentz asked for nominations for the position of Secretary. Kretz nominated St. Croix County Health and Human Services Board Page 2 June 13, 2006 Luckey for the position of Secretary. Luckey declined nomination. Wentz nominated Rasmussen for the position of Secretary. Burri requested to close nominations for Secretary. Wentz requested a unanimous vote to be cast for Rasmussen to be Secretary. NURSING HOME REPORT Van Offeren and Pempek provided an overview of per patient day (PPD) scheduling and discussion of application to various departments. Van Offeren discussed the benefits of benchmarking. Van Offeren stated that we need to change the way we do business and enforcing benchmarking is the way to do it. Pempek stated Medicaid funding is based on PPD benchmarks and Medicaid does not reimburse for expenses that are over the benchmark. Since March 2006 Nursing has staffed using PPD benchmarks. If census increases you have flexibility to increase hours, if census decreases you reduce staff hours. The remaining departments started PPD staffing May 1, 2006. In essence, every single department is affected when the census increases or decreases. Van Offeren stated PPD was initiated on May 1, 2006. Pempek stated a total potential savings of 30.95 hours per day. Kretz asked how the staff was dealing with this and Van Offeren indicated some have been very upset. Pempek noted that most staff changes have already been made. Kretz asked why there had not been staff changes made to maintenance. Johnson stated he and Pempek met with Robole last week and will meet again next week to see what they have come up with for staff changes. Kretz stated he would like to see equity in how staff changes are made. He doesn't want the nurses to make changes without all the other departments doing the same. Cranmer asked what the conclusion to this would be. Kretz said that the conclusion for him was that we are moving in the right direction in a lot of different areas. Van Offeren stated this is a way to decrease the tax levy. Johnson stated he will keep this on the agenda. Johnson noted there is a copy of a WIPFLI Health Care Advisory and a page from the operational assessment (12/13/05) that may be of assistance in addressing some of the recent County goal setting work that needs to be addressed no later than September 2006. Johnson is interested in getting an informal group together, work on some things and then come back to the Board in July or August for direction. Johnson asked if anybody on the Board would like to be part of this group. Wentz, Rasmussen and Luckey said they would be part of the group. Johnson stated they would start in the next couple of weeks. Johnson stated he would get a preliminary meeting set that would hopefully work for everybody. Workshop vouchers 61 through 66, dated June 01, 2006 in the amount of $11,139.66. BARN /FARM LEASE Johnson stated the request for bids is complete. We have two bids we are working on. He said we are awaiting reference checks at this time. At this time, before the new barn lease is completed, we also need to work with the current tenant on items that have been totally replaced. The current tenant has stated that they have totally replaced some equipment so they could take those items with them. That is not necessarily our preference because then the barn is definitely more difficult to rent. Johnson stated he is requesting that the current tenant must provide some document of those purchases. Johnson stated the manure pit is full and we have to figure out a way to drain it and dispose of the manure to allow a new person to come in and occupy the facility. He doesn't think these issues will be resolved so that the barn can be rented out by July l't. St. Croix County Health and Human Services Board Page 6 June 13, 2006 FARM HOUSE LEASE Johnson stated the farm house lease was extended for one year. CESA /HEADSTART LEASE Johnson stated he is currently in the process of negotiating the lease for CESA /Headstart. He is negotiating a 3% increase for approximately $13,000.00 a year. LONG TERM SUPPORT -LONG TERM CARE REFORM Johnson provided copies of three letters. First is a letter dated 4/12/06 which represents concerns from our County. Johnson stated that while this letter is not from our consortium, it accurately describes several concerns from the County perspective. The other two letters dated 05/05/06 and 05/17/06 are both from Secretary Nelson. In part, the letters are a response to the County concerns and they do begin to identify what the State would like to see, particularly in the way of County contribution. Counties in our consortium had a meeting recently to discuss the Counties position and one of the issues was that the Counties needed to establish a better direct communication with the State on this issue. Johnson stated he would keep this on the agenda. CAPITAL OUTLAY Johnson stated that staff has worked to complete the capital outlay planning for the 2007 budget. Copies of the outlay plans for St. Croix Industries, Nursing Home and the rest of Health and Human Services were distributed in the Board packets. Johnson stated this would be on the agenda for July. PERSONNEL ISSUES Johnson stated Mike Griffith from Mental Health has submitted a letter of resignation. Wentz asked if there were any other issues to bring before the Board. CROIX COUNTY HEALTH AND HUMAN SERVICES BOARD PUBLIC HEARING June 13, 2006 The St. Croix County Health and Human Services Board conducted an "Open Hearing" on June 13, 2006, at the St. Croix County Health and Human Services /Health Center complex. MEMBERS PRESENT: Esther Wentz, Chair Leon Berenschot, Vice Chair Deb Rasmussen, Secretary Lois Burri Bill Cranmer Dr. Michael Kretz Linda Luckey Ron Troyer Fred Johnson called the Open Hearing to order at 6:00 PM noting that the meeting was properly and legally publicized. Johnson welcomed everyone to the hearing. Johnson stated he was pleased to see everyone in attendance tonight. Johnson stated it is important that the people who came to provide testimony or just to listen in are able to see how important the process is in preparing for the 2007 budget. Johnson asked Board members to introduce themselves and the district they represent. Johnson stated how important it is to recognize the staff because they are the ones who make the budget work. Johnson stated that it is important for everyone to hear the needs of the community and the open hearing is an excellent way for that to happen. Johnson stated there were people attending who would like to give testimony and invited all those who wished to speak to do so. Johnson stated an overview power point presentation that covers all the services that we provide was available. Johnson turned to the public to see whether they would like to view the power point presentation. No objections were made so the power point St. Croix County Health and Human Services Board Page 2 June 13, 2006 overview was shown. Testimony was given regarding concern for the community integration program and the long wait list for the program. Testimony was given from a taxpayer from Star Prairie stating they would like the activities in the nursing home to continue and no cuts be made to those programs. Johnson stated there were letters that would be read from those who could not attend or did not wish to speak. See attachments in minute book. Nelson read letters thanking Public Health, WIC and Reproductive Health for assistance received from those departments. Lockwood read a letter concerning the family support program and why you can get help one year and then you can't get it the next year. Lockwood stated there is controversy with the program because of the maximum of $3,000.00 per year per family. The program is available to families who have a child with severe disabilities living with them in the home. Lockwood stated there are limited funds available and many families on the wait list. Lockwood said the State has given the County more flexibility at the local level to determine whether they want to just provide the grant for a short period of time so that more people can be served. Lockwood stated the County has an advisory committee that works with the program to determine the needs of the program. Lockwood also read a letter concerning changes needed in finding day care, work and personal care /respite services. She feels a directory should be made available to parents who need help so they can find the services they are looking for. Prokash read a letter expressing a vote of confidence to the St. Croix Health Center and to say it is a valuable resource for St. Croix County and the community of New Richmond. The family is very thankful that the facility is here and that it will be a part of the area for a long time. Vrieze read a letter supporting the Early Intervention program, Family & Children Services and Mental Health departments. The Early Intervention program provides much needed counseling for their families. The CSP program is also a much needed program that provides vital services to the community. She hopes all of these services will continue to operate in St. Croix County. Eichstadt read letters supporting St. Croix Industries and Long Term Support programs. The resident of the community is able to have more money from working at St. Croix Industries and she also volunteers at St. Croix Nursing Home. Kiesler read a letter stating a major issue in St. Croix County is the need for outpatient and inpatient psychiatric care. Kiesler stated Dr. Colon is our working psychiatrist in St. Croix County and works 24 hours a week. Kiesler stated St. Croix County is looking to add services for another Doctor. Johnson wanted the individuals who gave testimony to know that the information they are presenting is received and addressed. Johnson addressed the concern for the wait list and stated that while we have not served everyone on the wait list, we have implemented partial slot funding and pursued Nursing Home relocation. He stated that Long Term Support is also working on Long Term Care Reform for adults. Kretz addressed the tobacco issue in St. Croix County. He gave a phone number, 1- 800 - QUITNOW for anybody who is interested in quitting smoking. Johnson stated that on behalf of the Board we appreciate everyone coming to the Open Hearing. Johnson stated that staff members and the full Board were represented tonight. Johnson stated the information given tonight will be reviewed and we will do the best we can to serve the needs of the community. Johnson thanked everyone for coming tonight.
