The above submittal has been reviewed for conformance with Wis. Admin. Code (WAC) & Wis. Stat., and has been
CONDITIONALLY APPROVED. This private onsite wastewater treatment system (POWT31 shall be installed in
accordance with the enclosed approved plans and any above referenced component manual(s) and specifications.
The owner, defined in Wis. Stat. § 101.01(10), is responsible for compliance with all code requirements.
Prior to install, a somillm permit shall be obtained from the county where located and inspection shall be
arranged per Wis. Stat. § 145.19 and 145.20(2)(d). No person may engage in or work at plumbing unless licensed
to do so per Wis. Stat. § 145.06. A copy of the approved plans, specifications, and this letter shall be onsite during
install and open to inspection by authorized representatives of the Department. All permits required by the state,
county, and local municipality shall be obtained prior to commencement of any construction/install/operation.
This approval shall be subject to the following conditions:
size soil sample shall be kneaded & rolled into a ball If it can then be rolled into a wire down to 1/4-inch thick
the site is too wet to proceed: if it crumbles before reaching 1/4-inch thick the site is dry enough to proceed.
Prior to plowing POWTS dispersal component area, tall grasses and shrubs shall be cut short and this and all
loose organic material shall be removed from the area to prevent matting.
Areas occupied by rock fragments, tree roots, stumps and boulders reduce the amount of soil available for
proper treatment. If no other site is available, in the POWTS dispersal component area, trees shall be cut off
at around level and removed, along with boulders; stumps and roots shall remain in place. A larger
infiltrative area is necessary when any of the above conditions are encountered.

• POWTS component areas & horizontal setbacks shall be preserved prior to and during construction —
prevent disturbance, compaction, fill or use of areas.
• Horizontal setbacks of all POWTS components and lines, especially from buildings, OHWM of navigable
waters, and wells, shall be verified prior to install to conform to WAC § SPS Table 383.43-1.
• Wastewater from contractor equipment & tool use & cleaning and leftover products shall not be discharged
to POWTS but shall be otherwise properly disposed of on or off site.
• Tank install shall conform to all manufacturer's requirements.
• All piping shall conform to WAC § SPS Tables 384.30-3 and 384.30-5.
• Insulation of building sewer length beyond 30 feet shall conform to WAC § SPS 382.30 (11Xc).
• Existing, unused POWTS components shall be abandoned per WAC § SPS 383.33.
• Surface water diversion/drainage away from all POWTS component areas shall be provided.
• Changes impacting dosing require state verification of TDH & GPM specifications.
• For new construction, it is recommended to not activate dose pump(s) until all tanks are pumped by a DNR
certified septage servicing operator after conclusion of all significant construction and interior finishing.
• Submitter shall provide owner a copy of this letter, the owner's manual & management plan, and all
instructions for proper operation & maintenance of this POWTS, for which the owner shall be responsible, per
WAC § SPS 383.54(1). Owner shall provide this information to any subsequent owners.
• Owner shall report each inspection, evaluation, maintenance, or servicing to county per WAC § SPS 383.55.
• If any POWTS component malfunctions so as to create a health hazard, the owner must follow the
contingency plan described in this approval.
In granting this approval the Division of Industry Services reserves the right to require changes or additions should
conditions arise making them necessary for code compliance. Nothing in this approval shall relieve the designer of
the responsibility for designing a safe building, structure, or component, per Wis. Stat. § 101.12(2).

START UP AND OPERATION: For new construction, prior to using the POWTS check treatment tank(s) for the presence of painting
products or other chemicals that may impede the treatment proem and/or damage the dispersal celks). If high concentrations ate detected have

The property owner is teVonable for the operation and maintenance of the POWTS and submission of required reports. The quantity and
quality of the wastewater stream will affect the performance and longevity of your POWTS. The installation of water -saving appliances and
futtmes along with Prompt repair of leads reduces the wastewater volume. Also the brine or waste from water softeners. iron removal units,
other clear wow treatment devices and foundation drains should be discharged to the ground surface whenever possible. Note: this does not
include laundry waste, showers, dishwater, etc.
This system is designed to handle domestic strength wastewater, however, the disposal of food based greases, oils, vegetable/fruit peels,
seeds, bones, and food solids, such as those produced by a garbage disposal should be minimized. Toilet tissue is the only paper that should
be discharged into the system. Other non -biodegradable items, such as baby wipes, tampons, sanitary napkins condoms, cigarette butts,
dental floss, and cotton swabs. should Dot enter the system. Chemicals, such as petroleum products, paint, disinfectants, pesticides,
antibiotics, solvents, etc., should not be flushed into the system becis they an seriously damage yourPOW'IS and contaminate your
drinking water supply. Maintain a regular steady flow by spreading laundry washing throughout the week. Avoid vehicle traffic over
all system components. Compaction of snow over the dispersal unit may cause it to freeze up.

INSPECTIONS & MAINTENANCE: Inspection shall be made by an individual carrying one of the following licenses or
certifications: Master Plumber, Master Plumber Restricted Sewer, POWTS Maintainer, or Septage Servicing Operator (per the attached
Maintenance Schedule). Tank inspections must include a visual inspection of the tank to identify any missing or broken hardware,
identify any cracks or leaks, measure the volume of combined sludge and scum and check for any backup or ponding of effluent to the
ground surface and test all electrical equipment such as pumps and alarms. Any defects shall be promptly corrected. Exposed openings
greater than 8 inches in diameter shall be secured with effective locking devices to prevent accidental or unauthorized entry the tanks.
When the combination of sludge and scum in any tank exceeds one-third (Ir) or more of the tank volume, the entire contents of the
tank shall be removed by a Septage Servicing Operator and disposed of in accordance with Ch. NR 113. Wisconsin Admin. Code.
Specific wm icing mechanics must be provided if vertical is > 15 feet or if horizontal is > 150 feet and instructions to be provided belotr.
The outlet filter(s) shall be inspected and cleaned to remove any accumulated solids accord ingto manufacturers specifications. Solids
washed from the filter shall be retained in the tank. Filter cleaning may be necessary at more frequent intervals than stated in the
maintenance schedule to kecpthe system operating.
Alarms should be tested on a regular basis by the home owner. If an alarm sounds, contact an individual licensed to service POWTS.
There is normally a I day reserve under regular operating conditions, however water should be conserved until any problems with the
system are corrected to prevent back-up of sewage into the dwelling or surfacing.
ABANDONMENT: When the POWTS fails and/or is pemmnendy taken out ofservicc the following steps shall he taken to ensure that
the system is properly and safely abandoned in compliance with Ch. SPS 383.33, Wisconsin Admin. Code:
• All piping to tanks and pits shall be disconnected and the abandoned pipe openings sealed.
The contents of all tanks and pits shall be removed and properly disposed of by a Septage Servicing Operator.
After pumping, all tanks and pits shall be excavated and removed or their covers removed and the void space filled with soil,
gravel, or other inert solid material.
CONTINGENCY PLAN: If the POWTS fails and cannot be rcpaircd the following measures have been, or must be taken, to provide a
code compliant replacement system:
O A suitable replacement area has been evaluated and may be utilized for the location of a replacement soil absorpt ion system. The
replacement area should be protected from disturbance and compaction and should not be infringed upon by required setbacks
from existing and proposed structure. lot lines and wells. Failure to protect the replacement area renders it unusable_ Replacement
systems must comply with the rules in clfect at the time of replacement.
O A suitable replacement area is not available due to setback and/or soil limitations. Barring advances in POWTS technology u
holding tank may be installed us a last resort to replace the failed POWTS.
® The site has not been evaluated to identify a suitable replacement area. Upon failure of the POWTS a soil and site evaluation must
be performed to locate a suitable replacement area. If no replacement area is available a holding tank may be installed as a last
reson to replace the failed POWTS.

The filter screen is a type 316L stainless steel
with .062" (1/16") diameter holes. Optional socks
are available for finer filtration. The screen is 3
inches in diameter and 18 inches long, with 41%
open area. This large open area (69.52 square inch-
es) allows the filter to operate at up to 83.8 gallons
per minute at 1 psi. With features like these even a
95% plugged screen will keep your pressurized sys-
tem well protected and working properly.

INSTALLATION: INSTALLATION & SERVICE INSTRUCTIONS
When installing an STF-100, screw filter into discharge port of any pump that has a 2" National Pipe Thread.
Pumps with a smaller discharge port may be adapted to fit. When installing an STF-100A2 a tailpiece and male
adapter will need to be added to the inlet end of the filter to the desired height and a 2" union will need to be
added to the outlet end of the filter. Always install the filters in a position where they can be easily serviced.
"Always use caution when starting threads to avoid cross threading". Plumb force main into the 2" sch 80
PVC union. "We recommend that the union remain together during gluing to insure that glue or cleaner does
not ruin O-ring or sealing surface".
DO NOT REMOVE FILTER IF WATER LEVEL IS ABOVE FILTER CANISTER
Service of filter screen is dependent on usage as every system is unique. For most residential systems we
recommend inspecting the filter within the0oOp to determine the necessary service intervals for the filter. In
high volume systems we recommend inspection within the first 6 months to determine necessary service
intervals for the filter. Once the service interval is determined it should be consistent unless something changes
in the system. Always inspect the filter screen for any j9fj and replace if necessary. If our
STF-101 service alarm switch has been installed and adjusted property it will alarm when the filter requires
service. It should be serviced$91W.. ' ,�f the septic tank and pump chamber is
performed. Servicing will be more frequent if using any one of our optional filter socks (600 micron, 150-190
micron, and 100 micro systemrvicing recommendations.

If the screen becomes clogged a ore the periodic pumping requirements, a nigh level alarm or light will
indicate the need for service. If system is equipped with a "pump on light" that stays on longer than normal, this
also may indicate a need to servire filter

appropriate location with proper protection. In some cases an additional filter screen allows quicker service
allowing the dirty filter to be washed later at the shop. Note that in cold conditfons the firltercap maybe difficult
to remove. Keep the fitter in a warm area or pour warm water over the cap before removing. Once the filler is
installed in the tank it maintains a stable temperature and removing the cap will not be a problem.
If the system is equipped with our Service Alarm Switch, the filter screen does not need service until the
Service Alarm Switch activates a light or audio alann. We still recommend that the filter be inspected once a
year for damage or corrosion.
NOTE: The total dynamic head loss of the system must be increased by 0.5 feet of head to overcome friction
loss through the filter.
SERVICE ALARM SWITCH
The alarm switch is available in three pressure ranges, low head, medium head, and high head. Installation
is simple, on SIMlTECH FILTER systems, remove'/4" plug from base of filter chamber and connect tube fitting.
Next, run the tube up into the tank riser and connect to service alarm switch. The alarm switch is fastened to
the side of the riser via the nylon strap provided. Run alarm wire to alarm box. The service alarm switch can be
wired with its own alarm or with the high water alarm.
Pressure adjustment is made by removing the end plug, and inserting the 7/32 alien. Clockwise increases
pressure. One turn equals approximately 3 PSI. The low head alarm switch comes factory preset at 8 PSI and
is completely field adjustable within it's range (3 to 24 PSI). We recommend the use of a ball valve when using
an alarm switch. Once you have installed the filter and alarm switch, the ball valve can be closed off to simulate
a plugged filter so that you can make sure the alarm switch is working correctly.

(a) The purpose of the sanitary permit Is to allow installation
of the private sewage system described in the permit.
(b) The approval of the sanitary permit Is based on
regulations in force on the date of approval.
(c) The sanitary permit is valid and may be renewed for a
specified period.
(d) Changed regulations will not impair the validity of a
sanitary permit
(e) Renewal of the sanitary permit will be based on
regulations in force at the time renewal is sought, and that
changed regulations may impede renewal.
(f) The sanitary permit Is transferable.
