TO: BOARD OF COMMISSIONERS OF PUBLIC LANDS 127 West Washington P.O. Box 7857 Madison, Wisconsin, 53707 SIRS: Pursuant to law and the authority granted by the annexed resolution, we, the undersigned, Norman E. Anderson Chairman, and Jill A. Berke , Clerk, of the County of St. Croix , Wisconsin, hereby make application for a loan of one hundred seventy five. thousand and no/100 Dollars ($ 175,000,00 ), from the Trust Funds of the State of Wisconsin, which loan is to be solely applied or paid out for the following purposes, viz: Purchase of data grocessii� P=r1nPn+ - and for no other purpose. Said loan to be continued for the term of ten years from the 15th day of March next preceding the date the loan -is made. Said loan is to be payable in annual installments with interest thereon at the rate of 74 percent per annum from the date of making the loan to the 15th day of March next and thereafter annually as provided by law. CROIX , state of Wisconsin, this 9th day of August , 19 83 Chairman, a Clerk FORM OF RECORD The following presents and resolutions were presented by Supervisor and were read to the meeting. WHEREAS, By the provisions of Chapter 25 and Section 67.12(12) of the Wisconsin Statutes and acts amendatory thereto, the Board of Commissioners of Public Lands of Wisconsin are authorized to invest the Trust Funds of this state in loans to towns, villages, cities and counties in this state, as in said chapter and acts provided, and, WHEREAS, Every town, village, city and county in this state is empowered to borrow of said Commissioners from said Trust Funds, such sum or sums of money, for such time, and upon such conditions and terms as may be agreed upon by and between the said Commissioners and the town, village, city and county applying for a loan, subject to the limitations, restrictions and conditions in said -chapter and acts set forth: N014 THEREFORE, RESOLVED, That we the Supervisors of the County of St. Croix duly assembled in meeting according to law, do hereby approve and authorize application to be made to said Commissioners by the Chairman and Clerk of the county of St. Croy for a loan of one hundred seventy five thousand and no,100 Dollars ($ 175,000.00 ) to this county for the purpose of purchasing data processinar equipment Said loan to be payable within 10 years from the 15th day of March next preceding the date the loan is made. Said loan is to be payable in annual installments with interest thereon at the rate of 7u percent per annum from the date of making the loan to the 15th day of March next and thereafter annually as provided by law. RESOLVED, Further, That no money obtained by said county of St. Croix by such loan from the state, be solely applied or paid out for any purpose except the purchase of data processing equipmelhit,, without the consent of the Board of Commissioners of Public Lands, hereinafter obtained; RESOLVED FURTHER, That in case the aforesaid application for a loan from the Trust Funds shall be approved by the said Board of Commissioners of Public Lands, that the chairman and clerk of the county of 'St. Croix be, and they are hereby fully authorized and empowered, in the name of the said county of St. Croix , to make, execute and deliver to said Commissioners, certificates of indebtedness, in such forms as required by said Commissioners, for any and all sums of money that may be loaned to said county of St. Croix pursuant to said application, hereby empowering the said chairman and clerk of said county to do and perform all necessary things to be done by the county of Ste Croix to fully carry out the provisions of said Chapter 25, and acts amendatory thereof, and these resolutions: RESOLVED FURTHER, That this preamble and these resolutions and the aye and no vote of which the same were adopted, be duly recorded and that the clerk of this couinty forward a certified copy of said record with the application of said In.in to the Board of Commissioners of Public Lands of Wisconsin; 0 Supervisor Thomas O'Keefe moved the adoption of the resolutiona. The following preamble and resolution were offered by Supervisor WHEREAS, The supervisors of the county of St. Croix have this day authorized and directed the chairman and clerk of said county to make application for and borrow from the Trust Funds of the state of Wisconsin, the sum of one hundred seventy five thdusand and no/100 Dollars ($ 175,000,00 ) for the purpose of purchasing data processina equi=en+. , therefore, be it 3. 4 RESOLVED, That there shall be raised and there is hereby levied upon all taxable property, real and personal, within the county of St. Croix a direct annual tax for the purpose of paying and sufficient to pay interest on such debt as it falls due, and also to pay and discharge the principal thereof, as such principal falls due, within fen years from the time of contracting said debt, in accordance with the amendment of Article XI, Section 3 of the constitution of the state of Wisconsin. STATE OF WISCONSIN COUNTY OF St. Croix I, Jill A. Berke , Clerk of the County of St. Croix do hereby certify that the foregoing is a true copy of the record of the proceedings of the Board of Supervisors of the County of St. Croix , held on the th day of August , 193, relating to a loan from the State Trust Funds; that I have compared the same with the original record thereof in my custody as such clerk, and that the same is a true copy thereof, and is the whole of such original record. I further certify that the whole number of members constituting by law the -Board of Supervisors of the county of St. Croix , is 31 , and that the original of said preamble and resolutions was adopted at a regular meeting of said Board of Supervisors by a vote of 26 ayes to 0 noes, and that the said vote was had and taken in the manner provided by law, and the rules of said Board of Supervisors and said proceedings are duly recorded in Book of said county in Volume on pages (to be filed with 1983 Resolutions) IN TESTIMONY WHEREOF, I have hereunto set my hand and affixed the seal of the said county of St. Croix in the state of Wisconsin, this 9th day of August 19 83 CTdrk STATE OF WISCONSIN COUNTY OF St. Croix I Jill A.
