St. Croix County, Wisconsin
WHEREAS, on October 5, 2010, the St. Croix County Board of Supervisors adopted a resolution
requiring any position in which a vacancy occurs to be reviewed by the parent committee, the
Administration Committee and the County Board prior to refilling; and
WHEREAS, the Health and Human Services Board is the parent committee for the position of Social
Worker in the Health and Human Services Department; and
WHEREAS, a vacancy has occurred in this position; and
WHEREAS, the Health and Human Services Board has reviewed the vacant.position and recommends
not filling the position as is but rather restructuring the position to a Clinical Social Worker and then fill;
and
WHEREAS, the Administration Committee has reviewed the position and agrees with the conclusions of
the Health and Human Services Board and recommends the positions be restructured as recommended
and then filled; and
Va-IEREAS, the County Board has reviewed the position and agrees with the conclusions of the Health
and Human Services Board and the Administration Committee and concludes that the position should be
restructured and then filled.
THEREFORE, be it resolved by the St. Croix County Board of Supervisors that the position of Social
Worker in the Health and Human Services Department be restructured to a Clinical Social Worker and
filled.

Clinical Social Worker
Adult Community Support Services
General Definition of Work
Performs intermediate clinical Social Work services admitting, assessing, diagnosing, counseling
and assisting clients with their needs, dete available programs and services, preparing
and maintaining files and records, preparing reports and related work as apparent or assigned.
Work is performed under the general direction of the Adult Community Support Services
(ACSS) Supervisor.
Qualification Requirements
To perform this job successfully, an individual must be able to perform each essential function
satisfactorily. The requirements listed below are representative of the knowledge, skill and/or

Essential Functions
(50% or more of this position's work is performed in clients homes or in the community)
• Provides psychotherapy and direct service to clients in an individual, family, or group setting.
• Performs admission histories and clinical assessments, diagnoses and evaluates clients'
strengths and needs as required for Psychiatric Intakes and admission to the Community
Support Program.
Provides clinical supervision to ACSS staff.
® Serves as a backup to the ACSS Supervisor.
Provides outreach, crisis assessment, coordination and support for clients accessing the
Emergency Services system.
Develops treatment and /or recovery plans with clients and their natural and formal support
systems.
® Assists clients in gaining employment through job development and referral if appropriate.
® Collaborates with other service providers, both internal and external, as required.
® Attends weekly CSP and multi- disciplinary team meetings to review and coordinate services.
Accurately documents in writing or electronically, all client related information utilizing an
S.O.A.P. or D.A.P format, within professional standards.
Document and submit client related billing information as necessary.
® Establishes and/or maintains necessary linkages through advocacy with Federal, State and
local public and private agencies /services.
Performs other duties as assigned.
Knowledge, Skills and Abilities
Knowledge of social, economic and health issues; knowledge of social agencies and the methods
of the helping process; knowledge of individual and group behavior; ability to identify social
issues and needs to assess the ability of individuals and families to use services in problem
solving; ability to plan and organize work and to understand and interpret laws, policies and
regulations; ability to prepare reports and maintain records; ability to communicate effectively
both orally and in writing; ability to establish and maintain effective working relationships with
clients, associates, professional and technical staff, social agencies and the general public.
Ability to utilize modern office equipment, including computers to automate workloads and
interface with client information systems.
Clinical Social Worker –Adult Community Support Services
IHIS – Behavioral Health
Education and Experience
Masters Degree in Social Work or other behavioral science field with a inhn—imurnn of 3,000 hours
of supervised post-degree experience with persons with chronic mental illness or a Masters
degree with 1,500 hours or supervised clinical experience in a CSP.
Physical Requirements
This work requires the regular exertion of up to 10 pound of force, frequent exertion of up to 25
pounds of force and occasional exertion of up to 50 pounds of force; work regularly requires
sitting, frequent speaking or hearing and using hands to finger, handle or feel, frequently requires
walking and repetitive motions and occasionally requires standing, reaching with hands and arms
and pushing or pulling; work requires close vision, distance vision, ability to adjust focus, depth
perception, night vision and peripheral vision; vocal communication is required for expressing or
exchanging ideas by means of the spoken word and conveying detailed or important instructions
to others accurately, loudly or quickly; hearing is required to perceive information at normal
spoken work levels and to receive detailed information through oral communications and/or to
make fine distinctions in sound; work requires preparing and analyzing written or computer data,
operating motor vehicles or equipment and observing general surroundings and activities; work
regularly requires exposure to multiple recognized health hazards (weapons, second-hand smoke,
dangerous/psychotic/mentally unstable clientele, mold, blood borne pathogens, bedbugs,
uncontrolled pets, driving motor vehicles) and potential for lost-time accidents; work is generally
performed in a moderately noisy location (e.g. business office, light traffic).
Special Requirements
Wisconsin Licensure as Licensed Clinical Social Worker, Licensed Marriage and Family
Therapist or Licensed Professional Counselor, or Certification as an Advanced Practice
Social Worker.
Ability to maintain required Liconsure or Certification through continuing education.
Valid driver's license.
