A. C. page 5 of 6 Owner's Responsibility: The component owner is responsible for the operation and maintenance of the component. The county, department or POWTS service contractor may make periodic inspections of the components, checking for surface discharge, treated effluent levels, etc. The owner or owner's agent is required to submit necessary maintenance reports to the appropriate jurisdiction and /or the department. Septic Tank: Septic tank(s) are to be inspected routinely and maintained by department approved individuals when necessary in accordance with their approvals. The use of chemical /biological "treatments" is not required or recommended. If such additives are used, make sure they are approved by Department of Commerce, Safety and Buildings Div.. Effluent filters are to be removed & cleaned as necessary, with provisions to keep solids from passing the septic during removal. No more than 1/3 of the usable tank volume may be occupied by sludge /scum. 3 year inspection: If tank has greater than 1/3 volume sludge, tank contents must be emptied and disposed of in accordance with NR 113 Wisconsin Administrative Code by an approved individual. If the inspector does not recommend pumping of the septic tank, then the owner must be notified of when pumping should be done as to not exceed 1/3 sludge volume. Septic tank should be routinely inspected to be watertight and of good repair. Pump /Dose Tank If an effluent filter has been installed in the pump /dose tank, it must be removed & cleaned as necessary, with provisions to keep solids from passing to the mound component during removal. The pump, float switches and alarms must be inspected at least every three years for proper operation. Pump /dose tank should be routinely inspected to be watertight and of good repair. Mound and Lateral System The mound system component must remain free of ponded surface water prior to pump operation. If 4 inches or more water level is detected in the observation pipes, the owner must be notified of possible problems /failure. The designed daily flow capabilities of the component should never be exceeded. Trees and any other deep rooted vegetation should never be planted, or allowed to grow anywhere on the component. Activities OTHER than mowing /maintenance (i.e. excessive walking, pets, vehicles, etc...) could compress the component and reduce it's absorbtion capabilities and /or possibly cause it to freeze in winter conditions. Lateral distribution pipes should be flushed out/tested every 18 months using the cleanout points at each end of the component to remove scum that may clog orifices. Performance Monitoring: Performance monitoring must be done at least once every three years following the installation or at the time of a problem, complaint, or failure. Contingency Plan: If the septic tank, pump tank or any of their components therein (including floats, alarms, pumps, etc) become defective, the defective tank or component must be replaced immediately to ensure that the system can operate as designed. If the mound component cannot accept wastewater or ponds wastewater to the surface, the component must be repaired or replaced in it's current location by either: extending basal toe to provide added absorbtion area; or by removing the clogged bacterial mat, aggregate cell, and distribution piping within the mound and replacing said components in order to return system to proper working order as req u i red. 8 7 6 5 4 3 2 1 Plot Map I P/L P/L I 131 B 2 131 Al 100' O' O ■ M3 M4 j P/L Proposed mound area I LEGEND O ■ = Soil Borings With Backhoe Slope 2.3% M9, BM1= ELEV.100.0 -top of Road @ center Line- also HRP B2 U ', I 9T V BM2= ELEV.100.0' - Ground level at P/L stake- also HRP M2 SCALE: 1" = 49 I I I y . FOR: Object Type: Soil Saturation Determination Regulated Object ID No.: 1136437 The submittal described above has been reviewed for conformance with applicable Wisconsin Administrative Codes and Wisconsin Statutes. The submittal has been CONDITIONALLY APPROVED. The owner, as defined in chapter 101.01(10), Wisconsin Statutes, is responsible for compliance with all code requirements. The following conditions shall be met during construction or installation and prior to occupancy or use: • Approval is hereby granted to use the groundwater levels observed during the study as the high groundwater level in lieu of the depth to redoximorphic features for designing soil absorption systems in accordance with chapter Comm 85, Wis. Admin. Code. The groundwater elevation monitoring data was submitted in accordance with section Comm 85.60(3), Wis. Admin. Code. The determined highest level of saturation for 7 consecutive days was 0.58 feet or 7 inches below existing grade. • A mound system may be designed for this site and is limited to the following additional conditions: • The entire mound must be installed above the 96.00 ft contour line. • The mound must be designed using a linear loading rate of < 4.5 gpd/ft and a basal soil application rate of < 0.20 gpd/ft • The approval shall remain valid unless the site is altered in such a way that the depth to groundwater would change or if groundwater is observed for seven consecutive days at depths less than three feet below the infiltrative surface. • This approval in no way relinquishes the uses of color patterns to estimate the depth to high groundwater on any other parcels or portions of parcels. • If the Soil Saturation Determination Report was accompanied with a plan and an expiration date is shown on this letter, the expiration date only applies to the plans for the design, not to the decision on the Soil Saturation Determination Report. • As of today's date, a refund in the amount listed in the fee portion of this letter has been initiated and forwarded to a manager for review. The refund will be made under separate cover. Please expect a 6 -8 week time period for fiscal processing. Refunds will be made to the payer. CLARENCE L GLOTFELTY Page 2 6/11/2007 A copy of the approved plans, specifications and this letter shall be on -site during construction and open to inspection by authorized representatives of the Department, which may include local inspectors. If plan index sheets were submitted in lieu of additional full plansets, a copy of this approval letter and index sheet shall be attached to plans that correspond with the copy on file with the Department. All permits required by the state or the local municipality shall be obtained prior to commencement of construction/installation /operation. If this construction project will disturb one or more acres of land, an Erosion Control Notice of Intent (NOI) shall be filed with the department 7 days prior to any earth disturbing activities. You will need to either file the NOI and an erosion control plan summary on -line at www.commerce.wi.gov /sb or submit a completed NOI form and either a plan summary or complete plan to us, with additional fees, or to the certified municipality. In granting this approval the Division of Safety & Buildings reserves the right to reglire changes or additions should conditions arise making them necessary for code compliance. As per state stats 101.12(2), nothing in this review shall relieve the designer of the responsibility for designing a safe building, structure, or component. There was no good evidence of soil borings being completed. However, the monitoring pipes appear located properly. I would recommend that additional soil be mounded up around each monitoring pipe. Based on the above readings, the free water level varies from 8.0 to 14.5 inches below grade. Two hand dug soil borings were constructed within the .area being evaluated. Considering the CSTs report having 2 -3 inches of subsoil without redox features, I felt it may be possible there was actually more redox free subsoil than reported. However, the two soil borings showed just the opposite condition. There are common, medium, faint high and low chroma redoximorphic features immediately below the A horizon. Estimated high groundwater is zero inches. It would be advised therefore to continue with the monitoring until June 1st in an attempt to show the site is suitable for a mound system. P -1 35.5" from top of pipe to water. P -2 31.5" from top of pipe to water. P -3 37.5" from top of pipe to water. P-4 33.5: from top of pipe to water. P -5 37.5" from top of pipe to water. Based on the above readings, the free water level varies from 8.0 to 14.5 inches below grade. Two hand dug soil borings were constructed within the area being evaluated. Considering the CST's report having 2 -3 inches of subsoil without redox features, I felt it may be possible there was actually more redox free subsoil than reported. Wine (715) 688 -9282 Meeting with county inspectors and the owner at the above referenced site currently being monitored to determine the highest level of seasonal soil saturation. There was no good evidence of soil borings being completed. However, the monitoring pipes appear located properly. I would recommend that additional soil be mounded up around each monitoring pipe. A 50/50 mixture of powered bentonite clay and soil should be adequate to seal the soil surface and keep surface water away from the pipe. The two down slope pipes had free water in them. The others were dry. P -1 43" — 21" pipe height above grade = 22" pipe depth below grade, pipe is dry. P -2 48.25" — 23.5" = 24.75' pipe depth below grade, free water at 42" or at 18.5" below grade. P -3 51.75" — 29" = 22.75" pipe depth below grade, free water at 45" or at 16" below grade. P-4 48.5' — 24" pipe height above grade = 24.5" pipe depth below grade, pipe is dry. P -5 48.25' - 23.75' pipe height above grade = 24.5' pipe depth below grade, pipe is dry. The soil is presently frozen. I will return to this site when the frost is out to conduct a soil verification. Two hand dug soil boring should be adequate.
