Council on Aging and Disabilities - Regular Meeting - Nov 20, 2014 8:30 AM RESULT: ACCEPTED PUBLIC COMMENT* BUSINESS ITEMS 1. ContinuUs presentation Holly Miller, ContinuUs, presented information regarding ContinuUs and the process between the ADRC and ContinuUs. Once a member is enrolled into ContinuUs, a member receives a care manager and nurse to review needs. A ten day assessment is done to determine immediate health and safety needs as well as an initial service agreement is signed. Within 30 days a comprehensive evaluation of the person is done to further assess their needs. After 60 days of start date they complete the Member Centered Plan where the services necessary for the client are listed and implemented. If, when reviewing member needs, the wants or desires of the client do not match up with the member needs, members are able to make an appeal. RESULT: DISCUSSED 2. 85.21 Specialized Transportation Assistance Grant Application Community Services Coordinator Tracy Davis presented the 2015 Grant Application to the Council members. The power point presentation reviewed what the 85.21 is and the projects administered with grant dollars. There was discussion on the priority areas, budget and the purchase of a new vehicle. A motion was made to approve the grant application by Council member Pelnar; seconded by Council member Schreiber; motion carried. Financial Reports Council reviewed the October Financial reports. Their was discussion on whether the reports should be reviewed monthly quarterly. Council decided to review reports on a quarterly basis. RESULT: DISCUSSED 4. Temporary Relocation of Somerset Senior Nutrition Site The Somerset Senior Center will be relocating January 5 to the First Lutheran Church for approximately 4 weeks while the kitchen in the current location is remodeled. RESULT: DISCUSSED 5. Information & Assistance Update Sheet Council members received a handout with the numbers for November 2014. In November, there were nine enrollments into ContinuUs; three disenrollments from ContinuUs; five IRIS enrollments; two disenrollments from IRIS. RESULT: DISCUSSED 6. 2015 Council meeting dates and location Council members discussed the possibilities of moving the Council meetings to New Richmond. Discussed meetings being held 50/50 between New Richmond and Hudson. It was decided the meetings will be held in New Richmond with the meeting dates to be the third Friday of the month effective March 2015. The January meeting will be held in the Government Center in Hudson, with no meeting in February, and starting in March the meetings will be held in room 171 in the St. Croix County Services Center. RESULT: DISCUSSED 7. Santa to a Senior Campaign - update For the 2nd year in a row, the ADRC teamed up with HomeInstead Senior Care to bring a little holiday cheer to elderly and disabled folks of St. Croix County. There were over 60 folks who will receive gifts before December 23rd through the Be a Santa to a Senior program. The gifts were wrapped at the Hudson Senior Center and will be delivered by ADRC staff. RESULT: DISCUSSED 8. 2014 Walk to End Alzheimer's - wrap up Beckman informed the council a wrap up was held recently and the funds raised for the 2014 Walk to End Alzheimer's topped $48,000 for Pierce and St. Croix Counties. RESULT: DISCUSSED ANNOUNCEMENTS & CORRESPONDENCE Chair Ostness mentioned there has been a change in Fund Balance remainder. The State will begin to take back the remaining dollars accrued through the GPR. If there are any additional funds, the monies will be used to cover indirect costs and other allowable expenses so there is no money returned to the state. Merlin Blaisdell, Veteran's Services Director, will be retiring and there will be two retirement parties held in the upcoming months. Pam VanKampen, GWAAR, meet with Brad Beckman, Randi Hanson, and Brandy Zeuli and reviewed the nutrition program. Once the report has been received, it will be shared with the Council. The newest member of the Council on Aging and Disabilities will hopefully be Mary Ellen Brue, Baldwin. She has accepted the position and needs to be appointed at the next County Board meeting. Her first meeting will be in January. Chair Ostness mentioned a Hunger and Homeless summit that was held in December. Council member Anderson attended the Summit and was able to tour the new Grace Place location in New Richmond. Grace Place is currently only taking in families with children. There were presentations on the homeless in the county and the food programs offered in the county as well. ADJOURN DATE OF NEXT MEETING
