The owner, as defined in chapter 101.01(10), Wisconsin Statutes, is responsible for compliance with ail code requirements. No person may engage in or work at plumbing in the state unless licensed to do so by the Department per s.145.06, stats. The following conditions shall be met during construction or installation and prior to occupancy or use: Key Item(s) • In the event this soil absorption system or any of its component parts malfunctions sous to create a health hazard, the property owner must follow the contingency plan as described in the approved plans. In addition, the owner must insure that the operation, maintenance and monitoring duties as described in section VIII of the mound component manual are complied with. A copy of this information must be given to the owner upon completion of the project. • All conditions of the approval of the Interpretive Determination, transaction number 1569703, shall be adhered to. • Deep and thorough chisel plowing to the depth of at least 12" shall be done to the absorption area after the placement of 4" of the mound sand to help break up the platy soil structure that was reported at the site and incorporate the sand with the in situ soils. The county inspector shall be present on the site when this is done. • Surface water drainage shall be diverted away from the system area per Mound Component Manual. • The bottom of the distribution cell shall be level per the Mound Component Manual. The "D" dimension shall be a minimum of 30". The maximum finished slope of the mound surface shall be equal to or less than 3:1 per the Mound Component Manual. T ~ -- BENNIE W HELGESON Page 2 8/5/2008 • The designer proposes to install a state approved effluent filter to achieve the requirement of wastewater particle size. Pursuant to outlet filter product approval stipulations, maintenance information must be given to the owner of the POWTS explaining that periodic cleaning of the effluent filter is required. The access opening used to service the filter shall terminate at or above finished grade with a watertight cover. Reminder • The orientation of the mound system must be such that the longest dimension is oriented along the surface contour per COMM 83.44(6)(a)2. • Limit activities in the area 15' beyond the down slope edge of the mound per Mound Component Manual. • Materials shall conform to the requirements of COMM 84. • Maintain well and waterline set backs per COMM 83.43(8)(1). Consult the Department of Natural Resources for wel( setbacks and other regulations and exceptions. A copy of the approved plans, specifications and this letter shall be on-site during construction and open to inspection by authorized representatives of the Department, which may include local inspectors. All permits required by the state or the ioea! municipality shall be obtained prior to conunencement of construction/installation/operation. In granting this approval the Division of Safety & Buildings reserves the right to require changes or additions should conditions arise making them necessary for code compliance. As per state stats 101.12(2), nothing in this review shall relieve the designer of the responsibility for designing a safe building, structure, or component. Tank inspections must include a visual inspection of the tank(s) to identify any missing or broken hardware, identify any cracks or leaks, measure the volume of combined sludge and scum and to check for any back up or ponding of effluent on the ground surface. The dispersal cell(s) shall be visually inspected to check the effluent levels in the observation pipes and to check for any ponding of effluent on the ground surface. The ponding of effluent on the ground surface may indicate a failing condition and requires the immediate notification of the local regulatory authority. When the combined accumulation of sludge and scum in any tank equals one-third (Y31 or more of the tank volume, the entire contents of the tank shall be removed by a Septage Servicing Operator and disposed of in accordance with chapter NR 113, Wisconsin Administrative Code. All other services, irroluding but not limited to the servicing of effluent filters, mechanical or pressurized components, pretreatment units, and any servicing at intervals of <_12 months, shall be performed by a certified POWTS Maintainer. A service report shall be provided to the local regulatory authority within 10 days of completion of any service event. OWNER`: BOB OLSON Page 7of 8 START OP AND OPERATION For new construction, prior to use of the POWTS check treatment tankls) for the presence of painting products or other chemicals • that'may impede the treatment process and/or damage the dispersal cell(s), if high concentrations are detected have the contents of the tankls) removed by a Septage servicing operator prior to use. System start up shall not occur when soil conditions are frozen at the infiltrative surface. During power outages pump tanks may fill above normal highwater levels. When power is restored the excess wastewater will be discharged to the dispersal cell(s) in one large dose, overloading the ceII1s) and may result in the backup or surface discharge of effluent. To avoid this situation have the contents of the pump tank removed by a Septage Servicing Operator prior to restoring power to the effluent pump or contact a Plumber or POWTS Maintainer to assist in manually operating the pump controls to restore normal levels within the pump tank. Do not drive or park vehicles over tanks and dispersal cells. Do not drive or park over, or otherwise disturb or compact, the area within 15 feet down scope of any mound or at-grade soil absorption area. Reduction or elimination of the following from the wastewater stream may improve the performance and prolong the life of the POWTS: antibiotics; baby wipes; cigarette butts; condoms; cotton swabs; degreasers; dental floss; diapers; disinfectants; fat; foundation drain (sump pump) water; fruit and vegetable peelings; gasoline; grease; herbicides; meat scraps; medications; oil; painting products; pesticides; sanitary napkins; tampons; and water softener brine. ABANDONMENT When the POWTS fails and/or is permanently taken out of service the following steps shall be taken to insure that the system is properly and safely abandoned in compliance with chapter Comm 83.33, Wisconsin Administrative Code: • All piping to tanks and pits shall be disconnected and the abandoned pipe openings sealed. • The contents of alt tanks and pits shall be removed and properly disposed of by a Septage Servicing Operator. • After pumping, all tanks and pits shall be excavated and removed or their covers removed and the void space filled with soil, gravel or another inert solid material. CONTINGENCY PLAN If the POWTS fails and cannot be repaired the following measures have been, or must be taken, to provide a code compliant replacement system: ^ A suitable replacement area has been evaluated and may be utilized for the location of a replacement soil absorption system. The replacement area should be protected from disturbance and compaction and should not be infringed upon by required setbacks from existing and proposed structure, lot lines and wells. Failure to protect the replacement area will result in the need for a new sail and site evaluation to establish a suitable replacement area. Replacement systems must comply with the rules in effect at that time. ^ A suitable replacement area is not available due to setback and/or soil limitations. Barring advances in POWTS technology a holding tank may be installed as a last resort to replace the failed POWTS. ^ The site has not been evaluated to identify a suitable replacement area. Upon failure of the POWTS a soil and site evaluation must be performed to locate a suitable replacement area. If no replacement area is available a holding tank may be installed as a last resort to replace the failed POWTS. ($1 Mound and at-grade soil absorption systems may be reconstructed in place following removal of the biomat at the infiltrative surface. INTRODUCTION On May 14, 2008 Mark Iverson, Professional Soil Scientist #197 and Certified Soil Tester #46672, completed an interpretive onsite soils evaluation. An interpretive evaluation is required because the site does not meet the requirements to install a new private on-site waste treatment system (POWYS) through completion of a conventional onsite soils evaluation (<A+4 inches of acceptable soil must be present). The onsite evaluation was completed through the observation of the soil profiles at more than nine test pit locations. This report has been prepazed to present the results of an interpretive onsite soils evaluation. The evaluation was completed at the request of the client, Robert Olson. The property is located'/Z mile east of 250' Street in the Township of Baldwin, St. Croix County, Wisconsin. II. PROJECT INFORMATION The following is site-specific background information: Property Owner: Robert Olson lOS3 250' Street Woodville, WI 54028 Site Location: SW'/4 of the NE 1/4, Sec. 11, Toum 29N, Range 16W Parcel ID # 002-1022-30-000 Town of Baldwin St. Croix County Historic Land Use: Agricultural Crop Production /Construction Site Current Land Use: Fallow Vegetation: The vegetation within the tested area consists of miscellaneous grasses. There are no signs of stress within the tested area due to excess or insufficient moisture. Hydric vegetation was not observed within the project area. III. METHOD This interpretive evaluation and report was completed following the guidelines set forth in Comm 85.60(2) of the Wisconsin Administrative Code and in the summary provided by Leroy Jansky titled, "Preparing the Interpretive Determination Report" (March 13, 2002). IV. DISCUSSION OF RESULTS A. Local Hydrology 1. Surface Water Drainage Patterns -Surface water drainage is relatively uniform over the proposed system area. Less surface water flow occurs near B-3, B-4, and B-7 because they are located on the nose of the slope. More surface water drainage is anticipated from B-1 south as surface water from behind the mound on the adjacent property drains in this vicinity. The proposed system location is on the back slope. The slope within the proposed system area is approximately 2 to 3%. Evidence of concentrated surface water flow was not observed within the proposed system area. 2. Subsurface Water Drainage -Subsurface water drainage is limited by the moderate compaction of the surface horizon caused during construction of the house and by the dense glacial subsoil. Subsurface drainage is most limited in two highly compacted areas identified on the attached site map. The compacted old entrance and the compacted ATV trail have both been severely compacted and have very limited subsurface water drainage potential. Platy structure was observed in most other borings beginning at 2 to 4 inches below surface. The platy structure will limit the infiltration of surface water within the profile. The platy structure can be remediated by chisel plowing, vegetative growth, freezing and thawing and incorporation of sand during chisel plowing. During and immediately following frozen conditions and following heavy precipitation events, runoff will be much greater. 3. Artificial Drainage Devices - No artificial drainage devices were observed during the completion of the evaluation. 4. Contributing Drainage Area -The proposed system area is located on the backslope of the hill slope profile. The summit of the profile is located 100 to 200 feet west of the proposed installation contour. There is contributing drainage area up slope of the tested area. The drainage area is primarily mowed lawn however the mound on the adjacent site will contribute some surface water run off due to the location and slope of the mound. A surface water diversion ditch will need to be constructed to limit the effects of potential surface water received from upslope. The proposed location of the surface water diversion ditch has been identified on the attached site map. 5. Soil Saturation -Saturated soil was not observed in any of the test pits at the time of the assessment. 6. Surface Water Elevations -The nearest surface water is an intermittent stream located 500 to 1000 feet north of the proposed system. Surface water within the immediate vicinity of the site is shown on the topographic map included as Figure 1. B. Geomorphology 1. Parent Material -The parent material is loess over glacial outwash/till. 2. Landform Type -Ground moraines. 2 C. D. Soil Disturbance and Hydraulic Modifications The proposed system location is in a fallow field. No hydraulic modifications, such as drain the or ditches, were observed at the time of the assessment. No additional surface water is received from impervious surfaces. Landscape Position and Local Topography 'The regional topography is shown on figure 1. The ground surface elevation is approximately 1230 feet mean sea level (msl). The topography within the proposed system area is shown on the site map included in Appendix A. The proposed system elevation is 99.0 feet (site specific elevation). The shape of the slope within the proposed system area is generally linear (perpendicular to the contour) and slightly convex (parallel to the contour). Field Observations The soil present within the proposed system area is very similar in texture, color, and redox features to the description of the mapped soil unit Freeon identified on the Natural Resources Conservation Service Web Soil Survey. The primary difference between the mapped soil description and the actual conditions is the platy structure observed in the A horizon. The soil profile for each of the borings located near the proposed system has been summarized in the chart below. A soil evaluation was completed by James Thompson in July 1999 (Appendix B). Two borings (#2 & #3) were completed by Mr. Thompson near the current proposed system area. The surface horizon was approximately 10 to 11 inches thick and the structure was described asmoderate-fine-cr (likely meant granular). When the soil was reassessed in the summer of 2008 granular and blocky structure was only observed in the upper two to four inches. Highly compacted areas including the old entrance and the ATV trail still had moderate to strong platy or massive structure at the surface. The granular structure observed in the upper two to four inches in 2008 reflects the extent to which the site has self remediated the compaction since 2005. To determine what the original extent of compaction may have been, soil beneath construction debris was observed at three locations between borings 3, 4, 5, 7, & 8. Soil was observed at locations where the debris was larger than 2 feet by 2 feet and therefore has been subjected to freezing and thawing for the last three winters however it has not been remediated by root growth. The soil beneath the construction debris still had weak to strong, fine to medium, platy structure at all three locations observed (Boring - Misc. Soil observed in the borings that have been impacted by root growth had blocky and granular structure in the upper 2 to 4 inches. Redoximorphic features where encountered at 7 to 14 inches below surface within the proposed system area (borings 2, 3, 4, 7, & 8). Redoximorphic features were observed from the surface to 3 inches below surface in B-1 however were not present throughout the entire thickness of the A horizon. Boring 1 is located 7 feet south of the proposed distribution cell. The distribution cell was moved as far north as was possible to avoid the soil at B-1. The redox at B-1 was most likely caused by compaction and a higher volume of surface water runoff coming from behind the mound located on the adjacent property. The ability of the soil near B-1 to infiltrate and treat wastewater will be significantly improved by remediating the platy structure through tillage and diverting the surface water around the edge of the mound. Manganese accumulation was observed on the ped surface in boring 2 from two to ten inches below surface. Redox features were not observed on the interior of the peds. The structure consisted of strong fine to medium platy soil. Reorientation of the soil during tillage will greatly improve the infiltration capabilities of the soil. The upper two to four inches of soil within the majority of the proposed system area has been remediated by freezing and thawing of the soil and root growth of the naturally occurring vegetation, primarily grasses. The topsoil greater than two to four inches below surface was still compacted and exhibited platy structure prior to remediation. In order to speed up the remediation process the site was chisel plowed to approximately 8 to 10 inches below surface. The soil structure following chisel plowing still had large clods of platy structured soil. In order to break the clods up further a tiller mounted on a rubber tracked skid steer was used to till the upper six to eight inches of soil at a very slow speed (less than one 6 revolution per second). The tillage of the soil resulted in a moderate, fine, granular structure within the upper 6 to 8 inches and a mix of the granular and platy structured soil from eight to ten inches. The soil moisture condition at the time of tillage was moderate therefore eliminating the chances to smear the soil or turn it to powder during the tillage process. To encourage lateral movement of water below the proposed system the soil was tilled approximately 15 feet beyond the proposed toe of the mound. Following tillage the entire area was seeded with oats and the native grasses were allowed to reestablish. Sprinklers were set up to water daily and promote the establishment of the vegetation. The soil on site does not meet the hydric soil criteria. The lack of hydric soils indicates that the soil is likely not saturated for greater than 5% of the growing season (approximately 7 consecutive days). A dominance of hydrophytic vegetation was not observed within the proposed system area. The lack of these features does not preclude the possibility that the soil is saturated for a prolonged period of time prior to or on occasion during the growing season. G. Comparative Analysis Soil borings (B-2, B-3, B-4, and B-8) were completed within the proposed system area. These borings represent the soil conditions at 90% of the proposed system area. To assess the compaction with the ATV trail, boring B-6 was completed in the trail and boring B-8 was completed just off of the trail. The comparison indentified a significant amount of compaction at B-6 and restrictions to water infiltration as indicted by the mottling observed from the surface through the bottom of the test pit at 21 inches below surface. To assess the hydrology and soil conditions up slope boring B-7 was completed. The conditions at B-7 were very similar to those observed at B-3 and B-4 within the proposed system area. To assess the soil conditions down slope, B-9 was completed approximately 60 feet down slope from the proposed system. The soil conditions at B-9 indicated that this location is saturated for a significantly greater duration than any of the other boring locations observed. Low chroma colors were the dominant soil color observed directly beneath the A horizon. In addition, many of the redox contrast colors were prominent at B-9, while the contrast colors at the other soil boring locations were no more than distinct. H. Reports and Comments The original soil test completed by James Thompson is included in Appendix B. V. Conclusions and Recommendations Seasonal saturation appears to be limited to greater than 8 inches below surface within the proposed system area, the majority of the time. The lack of sail dominated by low chroma colors within the proposed system area indicates that the redoximorphic features observed are not due to a high water table or perching conditions. The cause of the redox features observed in the test pits is most likely attributed to short-term seasonal saturation due to slow permeability of the dense glacial subsoil and compaction caused during the construction process. Reorientation of the platy structure was completed by chisel plowing and tilling the soil on July 3 2008. This changed the platy structure observed in the topsoil to a fine granular structure. Reorientation of the soil structure combined with deep chisel plowing (plow to >12 inches below the original ground surface) after placing 4 to 6 inches of ASTM-33 sand on the system area will aid the dispersion of effluent through the subsoil. The site is suitable for installation of a POWTS (mound system) for the following reasons: 1.) Redoximorphic features were not observed in the upper 7 inches of soil within the proposed system area. (Except for the azea of the ATV trail near B-6.) 2.) Neither hydric soils nor a dominance of hydrophytic vegetation are present within the proposed system area. 3.) Surface water flow is slightly concentrated at the south end of the proposed system near B-1. Surface water diversion will be completed to redirect surface water away from this location. 4.) The slope at the proposed system location is 2 to 3%. The slope is uniform with no dips or valleys at the proposed system location. 5.) The lack of soil dominated by low chroma colors indicates that the redoximorphic features observed at depth are due to temporary seasanal saturation and not from a permanently high water table or perching condition. The following recommendations should be considered during design and installation of the proposed system: 1.) An individual site design will need to be completed for the system due to installation of the system over the compacted ATV trail shown on the site features map (Appendix A). 2.) The system should be installed on the 99.0' contour and stay within the tested area. 3.) Sand fill shall consist of 2.5' to 2.75' of material meeting ASTM C-33 specifications. 4.) To break up the platy structure tillage must be completed with a chisel plow to a depth of greater than 12 inches below native ground surface during moderate soil moisture conditions after placing 4 to 6 inches ASTM C-33 soil over the infiltration surface. 5.) The basal loading rate shall not exceed 0.2 GPD/ft2. 6.) The system shall be designed to have a lineaz loading rate of 4.0 GPD/ft or less. 7.) The system will follow the contour (99.0') and be neazly linear. There is approximately 6 feet of deflection at the maximum point. 8.) The area upslope of the mound near B-5 must be altered to limit the influence of surface water received from upslope. 9.) The installer must either confirm that the well identified on the site map meets the casing requirements of Wisconsin Administrative Code NR812.08 or redirect any potential .surface water flow generated from the soil absorption area around the well. The final approval of this site must be made by the Wisconsin Department of Commerce and the St. Croix County Zoning Department. Arrangements for inspection shall be made with the designated county official in accordance with the provisions of Sec. 145.20(2)(d), Wis. Stats. • The area within 15 feet horizontally below the system shall remain undisturbed. Vehicular traffic or soil compaction in this area is prohibited. • A state approved effluent filter is required. Maintenance information must be given to the owner of the tank explaining that periodic cleaning of the filter is required. Owners shall receive a copy of the appropriate operation and maintenance manual and/or owner's manual for the POWTS described in this approval. • Comm 83.52(1)(a) -The owner of a POWTS shall be responsible for ensuring that the operation and maintenance of the POWTS occurs in accordance with this chapter and the approved management plan under s. Comm 83.54(1). • Comm 83.52(2) - A POWTS that is not maintained in accordance with the approved management plan or as required under s. Comm 83.54(4) shall be considered a human health hazard. In the event this soil absorption system or any of its component parts malfunctions so as to create a health hazard, the property owner must follow the contingency plan as described in the approved plans. The owner is responsible for submitting a maintenance verification report acceptable to the county for maintenance tracking purposes. Reports shall be submitted at intervals appropriate for the component(s) utilized in the POWTS. In granting this approval the Division of Safety & Buildings reserves the right to require changes or additions should conditions arise making them necessary for code compliance. As per state stats 101.12(2), nothing in this review shall relieve the designer of the responsibility for designing a safe building, structure, or component. Stats. • Inspection of the private sewage system installation is required. Arrangements for inspection shall be made with the designated county official in accordance with the provisions of Sec. 145.20(2)(d), Wis. Stats. • The area within 15 feet horizontally below the system shall remain undisturbed. Vehicular traffic or soil compaction in this area is prohibited. • A state approved effluent filter is required. Maintenance information must be given to the owner of the tank explaining that periodic cleaning of the filter is required. Owners shall receive a copy of the appropriate operation and maintenance manual and/or owner's manual for the POWTS described in this approval. • Comm 83.52(1)(a) -The owner of a POWTS shall be responsible for ensuring that the operation and maintenance of the POWTS occurs in accordance with this chapter and the approved management plan under s. Comm 83.54(1). • Comm 83.52(2) - A POWTS that is not maintained in accordance with the approved management plan or as required under s. Comm 83.54(4) shall be considered a human health hazard. In the event this soil absorption system or any of its component parts malfunctions so as to create a health hazard, the property owner must follow the contingency plan as described in the approved plans. • The owner is responsible for submitting a maintenance verification report acceptable to the county for maintenance tracking purposes. Mound System Management Plan pursuant to comm 83.54 W. A. C. page 5 of 6 Owner's Responsibility: The component owner is responsible for the operation and maintenance of the component. The county, department or POWTS service contractor may make periodic inspections of the components, checking for surface discharge, treated effluent levels, etc. The owner or owner's agent is required to submit necessary maintenance reports to the appropriate jurisdiction and/or the department. Septic Tank; Septic tank(s) are to be inspected routinely and maintained by department approved individuals when necessary in accordance with their approvals. The use of chemical/biological "treatments" is not required or recommended. If such additives are used, make sure they are approved by Department of Commerce, Safety and Buildings Div.. Effluent filters are to be removed & cleaned as necessary, with provisions to keep solids from passing the septic during removal. No more than 1!3 of the usable tank volume may be occupied by sludge/scum. 3 year inspection: If tank has greater than 1/3 volume sludge, tank contents must be emptied and disposed of in accordance with NR 113 Wisconsin Administrative Code by an approved individual. If the inspector does not recommend pumping of the septic tank, then the owner must be notified of when pumping should be done as to not exceed 113 sludge volume. Septic tank should be routinely inspected to be watertight and of good repair. Pump/Dose Tank If an effluent filter has been installed in the pump/dose tank, it must be removed & cleaned as necessary, with provisions to keep solids from passing to the mound component during removal. The pump, float switches and alarms must be inspected at least every three years for proper operation. Pump/dose tank should be routinely inspected to be watertight and of good repair. Mound and lateral System The mound system component must remain free of ponded surface water prior to pump operation. If 4 inches or more water level is detected in the observation pipes, the owner must be notified of possible problemslfailure. The designed daily flow capabilities of the component should never be exceeded. Trees and any other deep rooted vegetation should never be planted, or allowed to grow anywhere on the component. Activities OTHER than mowing/maintenance (i.e. excessive walking, pets, vehicles, etc...) could compress the component and reduce it's absorbtion capabilities and/or possibly cause it to freeze in winter conditions. Lateral distribution pipes should be flushed out/tested every 18 months using the Geanout points at each end of the component to remove scum that may clog orifices. Performance Monitoring: Performance monitoring must be done at least once every three years following the installation or at the time of a problem, complaint, or failure. Contingency Plan: If the septic tank, pump tank or any of their components therein (inGuding floats, alarms, pumps, etc) become defective, the defective tank or component must be replaced immediately to ensure that the system can operate as designed. If the mound component cannot accept wastewater or ponds wastewater to the surface, the component must be repaired or replaced in it's current location by either: extending basal toe to provide added absorbtion area; or by removing the clogged bacterial mat,aggregate cell, and distribution piping within the mound and replacing said components in order to return system to proper working order as required. According to the diagrams in both the soil report and the plot plan submitted for the approved sanitary permit application, the mound should be located beginning 40 feet east of the corner stake for adjacent lots land 3. This mound site has now been damaged by vehicle traffic during construction of the house and cannot be used for installation of a mound in its current condition. Leroy Jansky, Dept. of Commerce Regional Wastewater Specialist, accompanied me during the June 2"d inspection to consider the potential for remediation of the soil compaction and loss of structure that has occurred. Jim Thompson, CST on the original 1999 soil report, accompanied me on the May 16`" inspection and dug several hand borings to check his reported profiles against current conditions. We found that the less disturbed areas to the south of the house construction have mottling, which indicates the seasonal 'nigh water table, just below the topsoil horizon. Better conditions exist closer to the property line shared with lot 3, however, there has been surface disturbance, rock stockpiled, and tire ruts were observed in that area. At this time, you will need to contract with a Certified Soil Tester to have the original borings relocated, re-evaluated and flagged, with additional borings made to document soil conditions and depth to seasonal high water table surrounding the original mound site. The soil tester may check for a different mound location, but it appears that soils south and east of your house site only get worse with regard to high water table limitations. Mr. Jansky recommends that, after the original site has been re-evaluated, the soil be chisel-plowed to >12" deep, then seeded to get vegetation established during the 2005 growing season. In the fall, Mr. Jansky can then schedule another on-site to determine whether the area must remain undisturbed until the spring of 2006. This in no way implies a guarantee that the site will be approved for mound installation based on the original state-approved design. Revision of the design may be necessary to add a pretreatment component or other modifications to improve the effectiveness of wastewater treatment to compensate for soil and high water table limitations. According to Comm 83.22(6)(b) the county may revoke any plan approval for "misrepresentation of facts", which in this case is the lack of a site on which to install the mound system. Until the mound site remediation issue has been resolved, no part of the original permitted POWT5 may be installed, according to Comm 83.22(6)(d).
