revisions, additions, or deletions to these CADD files shall be
made at the full risk of that party making such revisions, additions
or deletions and that party shall hold harmless and indemnify the
Consultant from any & all responsibilities, claims, and liabilities.

The property depicted on this survey and the easements of record shown hereon are the same as the property and the
easements described in the Commitment for Title Insurance issued by Land Title, Inc., as agent for Old Republic National Title
Insurance Company, File No. 700369, effective date August 15, 2024. The numbers below correspond to those in the title
commitment.

14.State of Wisconsin Department of Transportation Division of Highways Finding, Determination and Declaration establishing
controlled-access highway dated October 13, 1977, filed October 21, 1977 as Document Number 344011 (Vol. 563, Page 5).
Controlled access is shown hereon per the description in said document and the right of way as shown on
Transportation Project No. 8070-00-21-4.02 dated February 10, 2006.

conducting our field work.
17.We are not aware of any proposed street right of way lines or evidence of recent street or sidewalk construction or repairs
observed in the process of conducting our field work.
18.We are aware of any plottable offsite (i.e., appurtenant) easements or servitudes for this site.
ALTA/NSPS OPTIONAL TABLE A NOTES
SURVEY REPORT
1.The Surveyor was not provided utility easement documents for the subject property except for those shown on the Survey.
2.The bearings for this survey are based on the WISCRS St. Croix County Coordinate System NAD 83 (1991 Adjust).
3.All section monuments were found in place. No ties were set or tie sheets were required to be completed per the St. Croix
Surveyor.
