CROIX COUNTY HEALTH AND HUMAN SERVICES BOARD June 26, 2003 The St. Croix County Health and Human Services Board met on June 26, 2003 at the St. Croix County Health and Human Services/Health Center complex. NEXT MEETING: The next Health and Human Services Board meeting is scheduled for Friday, July 11, 2003 at 8:30 AM. Bethke will review vouchers at 8:00 AM. AGENDA: Motion: A motion was introduced by Berenschot, seconded by Bethke, and unanimously adopted: That the agenda be approved as circulated. MINUTES: Motion: A motion was introduced by Wentz, seconded by Bethke, and unanimously adopted: That the minutes of the 06/13/03 Health and Human Services Board meeting be approved with the following correction to page 1, last paragraph should read as follows: ...to attend the NALBOH (National Association of Local Boards of Health) July 16th through 19th, in Baltimore. ANNOUNCEMENTS: Borup stated the Open Hearing would be held after the Board meeting at 6:30 PM in the small production area at St. Croix Industries. Borup stated that depending on the turnout at the open hearing, he might want to look into the option of having a voluntary advisory group. Dr. Kretz announced that "Breathe Easy" in River Falls passed the Smoke-Free Restaurant Ordinance on Tuesday. APPROVAL OF MENTAL HEALTH/AODA RATES FOR 2003-2004: Borup stated it is time to review the Mental Health/AODA rates. Kiesler discussed the current and proposed rates to be effective 07/01/03. These rates reflect direct and indirect overhead costs. Kiesler said these rates tend to be based on a • St. Croix County Health and Human Services Board Page 2 June 26, 2003 fiscal year rather than on a calendar year. Rates are calculated by adding staff salary and fringes, program-related costs, space costs, Health and Human Services overhead and St. Croix County Government (indirect) overhead. The sum is then divided by the number of direct service hours provided by each discipline. Ruka stated that most fees have been very competitive with private clinics in this area. Motion: A motion was introduced by Wentz, seconded by Bethke, and adopted with Swenson voting no, That the Board approved the following rates effective July 1, 2003: M.D. _ $ 210 (individual), $70 (group) Ph.D. _ $ 155 (individual), $52 (group) Master's = $ 132 (individual), $44 (group) Bachelor's = $ 110 (individual), $37 (group) AODA Counselor = $ 110 (individual), $37 (group) Registered Nurse = $ 110 (individual), $37 (group) Mental Health Technician = $ 85 (individual) IDP Assessment = $ 300 IDP Re-Assessment Fee = $ 300 IDP Final Report = $ 110 IDP No Show Fee = $ 110 IDP Out-Of-State Referral = $ 30 AODA Court Ordered Evals = $ 200 MMPI Court Ordered Evals = $ 275 SAP Assessment = $ 280 ATLAS House sliding rate = $ 15 per day RESOLUTION TO SUPPORT SUPPLEMENTAL FUNDING FOR WISCONSIN WIC PROGRAM FOR 2004-2005 BIENNIUM: Borup asked for support and approval of the resolution supporting the continued funding of the WIC Program. Borup stated this is a cost-effective program. Nelson stated the program provides one-stop shopping because it provides the participants with nutrition education and services as well as other health care services. Motion: A motion was introduced by Dr. Kretz, seconded by Berenschot, and unanimously adopted: That the Board approves the resolution to Support Supplemental Funding for Wisconsin WIC Program for 2004-2005 Biennium. NON-UNION RECLASSIFICATION REQUESTS FOR 2004: Borup stated per County Board Policy (Hay Study) non-union position reclassification requests are due now. a. Accredited Medical Records Practitioner (RHIT) b. Accounting Supervisor C. Accounts Receivable Supervisor Borup stated the proposed job descriptions more accurately reflect the duties of these employees. Borup stated if the Board approves these requests, they will be forwarded to the Personnel Department and Hay Committee for review. Borup said that as Agency Director, he supports this action. Motion: A motion was introduced by Burri, seconded by Rasmussen, and unanimously adopted: That the Board approves the non-union position reclassification requests and approves submitting them to Personnel for formal review. • St. Croix County Health and Human Services Board Page 3 June 26, 2003 NURSING HOME ENGINEERING REPORT: Borup stated the Engineer's report has not yet been received. Borup stated the nursing home census is at 81. Krieger stated they have been starting to deny admissions because of the downsizing. ANNOUNCEMENTS: Borup stated the Economic Support Unit has had 10 applications since June 16, 2003 for people being evicted from their homes. In order to be eligible for Emergency Assistance there must be some crisis that occurred for them to not pay their rent. Only two of the applications have had a crisis. The rest are not eligible for any help. Borup stated they are being referred to the Salvation Army and West Cap. All of them have children. We will make a referral to Family & Children's and if necessary the children will be sheltered by Family & Children's. Family & Children's will not shelter the parents. Along with the evictions, there have been 29 electric shut-offs from June 16 through June 19. Xcel Energy will not restore the electricity unless the bill is paid in full. Most of the bills are in the $1000.00 range and higher. There is some crisis money left in Energy Assistance, however, the maximum amount paid per family is $250.00 for Crisis Assistance and Xcel Energy will not turn the lights back on without full payment. Borup stated this is a sign of the times and the economy. Borup stated the nursing home roof repairs are in progress. The company was able to start a week earlier than planned. Borup stated that Rod Paulson has finished the 2002 audit. Borup stated we are on schedule in terms of how we were budgeted. Borup distributed a summary of the fiscal report for review. Our "designated" fund is set at $292,933, compared to $503,766 at the end of 2001. $292,933 is arrived at after applying $575,000 in "carryover" funds from 2002 to 2003. Borup added that the 2002 budget was allocated $662,459 in carryover funds, of which $415,762 was spent (minus $45,000 farm transfer = $370,762 net expenditure exceeding revenues). Borup stated this is not a final report and the formal report may have some adjustments to it. Borup reminded the Board of the Open Hearing to be held at 6:30 PM at the Sheltered Workshop. Borup stated that we may want to look into having an Open Advisory Hearing that would meet three or four times a year on Health & Human Service issues. Then a spokesperson would give a written and verbal report to the Board in terms of recommendations for next year. Dorsey asked if there were any other issues to bring before the Board. CROIX COUNTY HEALTH AND HUMAN SERVICES BOARD PUBLIC HEARING June 26, 2003 The St. Croix County Health and Human Services Board conducted an "Open Hearing" on June 26, 2003, at St. Croix Industries in the small production area. MEMBERS PRESENT: Thomas Dorsey, Chair Leon Berenschot Lois Burri Dr. Michael Kretz Deb Rasmussen Chris Bethke Mary Louise Olson Esther Wentz Ralph Swenson John Borup called the Open Hearing to order at 6:30 PM noting that the meeting was properly and legally publicized. Borup welcomed everyone to the hearing. He invited everyone to speak and asked that they sign the sign-in sheet. Borup introduced the committee members present. Borup gave special thanks for everyone attending tonight. Borup stated we hold an Open Hearing annually and we listen to all the concerns, complaints and praise from all. Borup stated there is a difficult year ahead. The County Board will be giving us our direction and then we will do the budget and have in final form in mid-July to present to the Health & Human Services Board in August. It will then be forwarded to the Finance Committee in September and in late October it will be forwarded to the County Board. Borup stated that Economic Support cases continue to grow. Borup stated the Economic Support Unit has scheduled the elimination of the W-2 program as of mid-2003. Economic Support has also had an increase in utility shut-offs. Borup contributed this to the increase in the unemployment rate. Borup stated MHIAODA cases have doubled in the last year. There has been an increase in outpatient services and will be requesting an additional Mental Health therapist. Borup stated that Long Term Support cases continue to grow. The waiting lists continue to grow in that area. Borup stated the Nursing Home has downsized to 72 beds. Borup contributed the downsizing to a drop in census. Borup stated Public Health has taken on additional responsibilities with Emergency Preparedness. Public Health has also taken over agent status last year for restaurant inspections. Borup stated Family & Children's continues to grow. Out of home placement demands continue to grow. Borup stated that as we face the 2004 budget, state and federal demands continue with shared revenue cuts. 1 • a Borup introduced staff: Ron Lockwood, Long Term Support Supervisor Barbara Nelson, Public Health Supervisor-Health Officer Ron Eichstaedt, Director, St. Croix Industries Fred Johnson, Family & Children's Supervisor Mary Squyer, Economic Support Supervisor Norma Vrieze, Early Intervention Director Carolyn L. Krieger, Nursing Home Administrator Ron Kiesler MN/AODA Coordinator Steve Kirt, MH/AODA Clinical Services Supervisor Lisa Leahy, N.H.-Director of Social Services Cindy Prokash, Activity Therapy Director Kathy Leisch, Secretary Letters were read from those who could not attend or did not wish to speak. See attachments in minute book. Borup invited all those who wish to speak to do so. Shelly Weyers gave testimony of her appreciation for the time and effort given to them with her son Danny's disability. Shelly stated the Birth to Three program has given them much support and encouragement for their family. She gave thanks for help in the quick diagnosis with Danny's seizures. Gloria Linder thanked the Health Center for all it has done for her in the last year when she was in a serious accident in Seattle. She was brought home to the Health Center for eight weeks. She said everyone there is very gracious and cheerful and it helped in her recovery. She is concerned about the downsizing of the Nursing Home. Stanley Hanson and family gave testimony of their appreciation and thanks to everyone who helped his wife Carolyn in her therapy after her aneurysm. He said Carolyn is home now and doing fine and he is proud of the Health Center and the people working together to help people. Janet Knutson testified to the great care her mom and dad received while they were in the Nursing Home. She questioned why it was decided to downsize to 72 residents. Diane Krear testified that she has a 30-year old daughter who works at the workshop. She said that her daughter had a problem with the size of the bathroom at the sheltered workshop. She voiced her concerns and they solved the problem by making the bathroom bigger. She shared her concerns on the difficulty of finding a dentist that would take Medical Assistance. She stated she is grateful for the open hearings. Bob Boche testified he thinks the Board has their priorities wrong and asked what they are going to do with the people that need to go to a Nursing Home. He stated he knows people that have had to go to Polk County for therapy. Jeanine Staples testified that her mother-in-law was in the Nursing Home and she expressed to the staff what a wonderful place the nursing home is. Her mother-in-law had Alzheimer's and needed constant care. Borup thanked everyone for all the written and verbal testimonies and asked if there was anyone who would like to make a comment. Borup stated that on behalf of the Board we appreciate everyone for coming to the Open Hearing and that we are pleased to serve St. Croix County. Hearing adjourned at 7:35 PM.
