CROIX COUNTY HEALTH AND HUMAN SERVICES BOARD March 27, 2003 The St. Croix County Health and Human Services Board met on March 27, 2003 at the St. Croix County Health and Human Services/Health Center complex. Krieger, Nursing Home Administrator Judy Ziegler, Accounting Supervisor Ron Lockwood, Long Term Support Coordinator Barbara Nelson, Public Health Supervisor-Health Officer Kristie Tellijohn, Director of Nursing Kathy Leisch, Secretary OTHERS PRESENT: Clarence Malick, County Board Chair David Lund, Chief of Nursing Home Services-Health Care Benefits (DHFS) Chair Dorsey called the meeting to order at 8:30 AM noting the meeting was properly and legally publicized. NEXT MEETING: The next Health and Human Services Board meeting is scheduled for Friday, April 11th at 8:30 AM. AGENDA: Motion: A motion was introduced by Wentz, seconded by Burri, and unanimously adopted: That the agenda be approved as circulated. MINUTES: Motion: A motion was introduced by Berenschot, seconded by Wentz, and unanimously adopted: That the minutes of the 03/14/03 Health and Human Services Board meeting be approved as distributed. RESOLUTION TO DOWNSIZE NURSING HOME TO A LICENSED CAPACITY OF 76 BEDS: David Lund, Chief of Nursing Home Services under Health Care Benefits for the DHFS (Madison) discussed the pros and cons of downsizing and loss of IGT dollars. This process would need to be completed by June 30th to avoid the licensed bed tax and capture incentives for downsizing. The Board discussed the best number to downsize to. Dr. Craig expressed concern that 76 is too low. Krieger stated we could downsize again next year, but can't increase the number of licensed beds. Administration will work on downsizing and revenue issues for a 76-84 bed capacity "range" for the board meeting. St. Croix County Health and Human Services Board Page 2 March 27, 2003 Motion: A motion was introduced by Wentz, seconded by Berenschot, and unanimously adopted: That the Board table the resolution until the next meeting on Friday, April 11tH The Board recessed from 10:00 to 10:10. PUBLIC HEALTH: Nelson distributed a "Public Health In America" handout. Nelson stated in State Statute 251 the local health department/board is mandated to assess public health needs, to develop policy and assure measures be taken so that people live in a healthy environment. Public Health is a population-based service. Nelson stated Public Health receives no dollars from the State of Wisconsin for public health services. The dollars that fund public health are local county dollars, grants and fees for services. Nelson gave an update on Communicable Disease. There are currently 39 suspected cases of SARS, Severe Acute Respiratory Syndrome, in 18 states. None of these cases were in Wisconsin. The leading hypothesis on what is causing SARS is the coronavirus, a novel virus that hasn't been seen before. Nelson stated they were alerted in February about two cases of Avian Influenza in Hong Kong, which resulted in one death. The significance of this is that historically new human influenza strains resulting in influenza pandemic have emerged from swine and poultry. Nelson stated there have been sporadic cases of whooping cough. Nelson stated there have been alerts of Ecoli related to alfalfa sprouts and beef trimmings. There has been one positive rubella cases in a three year old unvaccinated child in Madison. Nelson stated there would be increases in vaccine costs for 2003-04. Nelson updated the board on the Smallpox Pre-Event Vaccination. The initial Phase 1 vaccination clinics are occurring throughout the State. DPH staff will operate approximately 25 clinics in March and April to vaccinate volunteer preparedness teams from hospitals and local health departments. These workers would provide care and treatment for people suspected of having smallpox, as well as investigates potential cases of smallpox disease. DPH staff with local health departments is conducting weekly teleconferences. There have been approximately 460 people vaccinated in Wisconsin. Approximately one-third of these was local health department staff. Nelson stated the Western Region Partnership for Public Health Preparedness (bioterrorism consortium) meets on a monthly basis and efforts to meet the outcome-based objectives continues. The four consortium positions have been filled. These are Program Assistant, Program Coordinator (Readiness and Assessment Coordinator), Surveillance and Epidemiologist Specialist, and Education/Training Coordinator. These staff all began employment through Polk County Health Department on March 3rd. Nelson stated the pandemic preparedness tabletop exercise on February 27th at Baldwin Area Medical Center, which included community organizations, was rated as successful. We are waiting for River Falls and New Richmond hospitals to take the lead on setting a date for an exercise involving their communities. Hudson is planning one in September after the hospital move has been completed. Nelson stated that Public Health would be partnering with Land and water Conservation. Public Health was awarded funds through an environmental health mini grant dealing with ground water protection, abandoned wells and manure storage facilities. St. Croix County Health and Human Services Board Page 3 March 27, 2003 Nelson stated that Public Health week is April 7-13. Public Health will be holding a potluck for the staff on April 11th and she invited Board members to attend. Nelson also noted the next legislative event would be held on April 28th. The annual Wisconsin Public Health Association Conference will be held on May 20th & 21st. The (WALHDAB) Wisconsin Association of Local Health Departments and Boards joint meeting will held in Eau Claire on May 28th. LONG TERM SUPPORT: Lockwood discussed the proposed downsizing of Northern Center. Lockwood distributed cost and revenue numbers for the seven residents of St. Croix County. (Fictional names used.) Lockwood did a preliminary estimate of what the cost would be for these seven residents. Based on current information, it is projected that there would be no County cost for five years. Lockwood discussed the Community Links Project. Lockwood stated he would likely be making an application to the State to purchase nursing services to assist our care managers. The State is moving towards a system where eventually one-third of care managers will be nurses and two-third will be social workers. The State is proposing a team of two nurses and 1 social worker to provide the best care for persons with severe and complex medical needs. Lockwood said the social workers agree with this proposal to work more closely with a nursing staff in regard to planning for the care of an individual. Lockwood stated he has one nurse and is proposing to buy nursing services from our public health department rather than hiring nurses. Lockwood discussed the children's waiver program. He stated there is a new waiver being developed but it has to be approved as part of the biennial budget. The advantage of this waiver is that it opens the door to serve children that are severely emotionally disturbed. Lockwood discussed the IGT funding as applied to community services. These funds are pending because of the conflict in the legislature over the use of these funds for this purpose. NURSING HOME REPORT: Krieger stated the census today is 82 with one leave of absence and three in the hospital as of 3/20/03. Borup stated the Nursing Home State survey was just completed on 3/29/03. Borup stated it went very well! Two "lesser" cites noted. There are no "patient care cites." Borup said the survey team was very complimentary about patient care, staff attitude, the condition of the facility, etc. The State engineer is expected to arrive today. Every year the engineer cites the facility for locked doors in which we are granted a waiver. PERSONNEL ISSUES: Borup stated we are pursuing potential staffing options for Mental Health services. Long term we plan to pursue a new position for 2004. Borup stated he would like to take steps at the next meeting to secure temporary services through a contracted consultant for the remainder of 2003. FINANCIAL REPORT: Borup stated he did not have any substantial information on the budget proposal. Borup said Rod Paulson would be performing the Nursing Home Audit on April 7th. Borup anticipates that this will be a positive process. • St. Croix County Health and Human Services Board Page 4 March 27, 2003 REQUEST TO ISSUE BIDS FOR 12-PASSENGER VAN FOR ST. CROIX INDUSTRIES: Borup requested to issue bids for 12-passenger van for St. Croix Industries. The current van has 145,000 miles on it. Borup stated this is a budget item and is part of the capital outlay plan. Motion: A motion was introduced by Berenschot, seconded by Wentz, and unanimously adopted: That the Board approves the request to issue bids for 12-passenger van for St. Croix Industries. Dorsey asked if there were any other issues to bring before the Board. Motion: A motion was introduced by Burri, seconded by Wentz, and unanimously adopted: that the meeting be adjourned.
