APPROVAL OF MINUTES 1.Council on Aging andDisabilities Regular Meeting December 18, 20158:30 AM Council Vice Chair Schreiber asked the Council if there were any changes, corrections, or additions to the December 18, 2015 meeting minutes. With none noted, a motion was made to accept the minutes by Council member Alfie Schrank and seconded by Council member Laurie Pelnar; motion carried. RESULT:ACCEPTED PUBLIC COMMENT BUSINESS ITEMS 1.ContinuUs Presentation Holly Miller, ContinuUs, presented to the Council on current statistics on St. Croix County participants within ContinuUs. Works closely with their provider network by supporting them. It is unfortunate that there were some in-home supportive care facilities that have either closed or discontinued in-home care. The Family Care/IRIS 2.0 changes are being followed by the staff at ContinuUs and they are preparing. An announcement was made on Friday, January 8th that a contract was awarded to Care Wisconsin to provide services within St. Croix, Pierce, Dunn, Chippewa, and Eau Claire along with ContinuUs. The state had always intended to have more than one Managed Care Organization in the counties to allow consumers choice. The services offered through Care Wisconsin will not differ from ContinuUs, it will still follow the Family Care model set forth by the state. RESULT: DISCUSSED 2. Project Search Presentation Fabreann Buffington, Instructor Project Search, presented a video to the Council on Project Search. The program works with individuals who are 18 - 23, some are from the school system, and within the River Falls Hospital. The internship is a nine month commitment and there is an interview process to be part of the program. There are three rotations of areas within the hospital so they are able receive a variety of experiences. When they move to their last rotation, they are interviewed by the department and program coordinator. There is one hour a day classroom time and the job coaches check in periodically with each intern within the department they are working in. RESULT: DISCUSSED 3. Financial Reports Beata Haug, HHS Fiscal Manager, reviewed the Financial handout in the Council packet for the month of November. Both Aging and Aging & Disability Resource Center claiming is in line and on track with last year. When looking at expense the amounts for Aging, the amount is higher than 2014 due to the Pay for Performance evaluations in July. RESULT: DISCUSSED 4. New Richmond Senior Center Relocation Report Brad informed the Council regarding the New Richmond Senior Nutrition Site relocation. At the Senior Center Board meeting earlier in the week, they decided to tour the First Methodist Church and revisit the Solid Rock Church to see the new dishwasher they installed. The VFW is moving forward with constructing a new building and including the Senior Center. RESULT: DISCUSSED 5. Information & Assistance Update Sheet Council members were provided a handout regarding the activities in December. There were four enrollments into ContinuUs and two disenrollments; there were three enrollments into IRIS and one disenrollment. Since the holidays have passed, the number of referrals have increased which is a trend. RESULT: DISCUSSED 6. Transportation Requests (Out of County/Weekend) Out-of-county or beyond hours of operation requests related to the utilization of the ADRC/Aging fleet vehicles. River Falls Senior Center - use Hudson van - February 20 and March 19 to go to Menomonie Glenwood City to Maiden Rock on February 18 and Menomonie on March 5. A motion to accept the trips was made by Council member Schrank, seconded by Council member Zaske; motion carried. ADRC Consumer & Follow up Survey The Aiming for Excellence committee within the Aging and Disability Resource Center (ADRC) created a survey to be provided to consumers who have received assistance through the ADRC. The survey rolled out January 4 in the Welcome Packets and mailed to individuals currently enrolled in the Caregiver Program and the Volunteer Medical Transportation Program. Results will be shared on a quarterly basis. RESULT: DISCUSSED 8. 2016 St. Croix County Senior Center Card Tournament Council members were provided with the schedule for the upcoming 2016 Countywide Card Tournament. The Tournament will kick of on March 23 in Glenwood City and will continue into May traveling to all the Senior Centers except River Falls. Bridge, 500, and Cribbage are played and last year's winner was Amery Senior Center. If Council members are interested in playing, they need to call one of the Senior Centers to be part of their team. RESULT: DISCUSSED 9. AARP Tax Assistance for 2016 Council members received information on the 2016 AARP Tax Assistance schedule. Volunteers through AARP will again assist individuals with filing taxes who are residents of St. Croix County. The schedule will begin in February in a variety of areas on a first come, first serve. RESULT: DISCUSSED REQUEST FOR FUTURE AGENDA ITEMS Discussion on what is Family Care and the process; Preventative/Nutrition Program Coordinator Randi Hanson to provide an update on the Senior Nutrition Sites.; building evacuation plan and what is the emergency procedure. ANNOUNCEMENTS & CORRESPONDENCE Council member Zaske presented information on an upcoming voter training sponsored by The Bridge to be held at First Presbyterian Church in Hudson on January 26th from 6 p.m. - 9 p.m. Beckman read a thank you regarding an article that was written in the ADRC News regarding Lewy Body.
