Brad Beckman, Aging and Disability Resource Center Administrator; Tracy Davis,
Aging and Disability Resource Center Community Services Coordinator; and Ed Boeve,

APPROVAL OF MINUTES
1.Council on Aging and Disabilities Regular Meeting October 20, 20168:30 AM
Chair Ostness asked the Council if there were any corrections to the minutes from the
October 20, 2016. Chair Ostness approved the minutes.

BUSINESS ITEMS
1.85.21 Specialized Transportation Assistance Grant Public Hearing
The Public Hearing was called to order at 8:34 a.m. by Chair Ostness. Tracy Davis,
Community Services Coordinator, provided a summary of the Specialized Transportation
Assistance Grant Public Application CY2017. Council members received details on the
specific projects and the budgets for each. The Public Hearing was closed at 9 a.m.
RESULT: DISCUSSED
2. 85.21 Specialized Transportation Assistance Grant Application CY2017
Chair Ostness asked for a motion to approve the 85.21 Specialized Transportation
Assistance Grant Application. Supervisor Anderson made a motion to approve the grant
application; motion seconded by Council Member Pelnar; motion passed.

3. ESR Works Presentation
Ed, ESR, began providing services in 2014 upon the closure of St. Croix Industries. Ed
provided a handout to the Council with information regarding construction of the new
building and changes under Department of Labor regarding employment.
RESULT: DISCUSSED
4. 2016-2018 Aging Unit Plan Amendments
Council members reviewed the changes that may have needed to be done and the 2017
budget. A motion to approve the Plan Amendments was made by Council member
Pelnar and seconded by Council member Drath; motion carried.

5. Roberts Nutrition Program Presentation
Nutrition Program Manager Chris Stork was not able to make the meeting this month, so
she provided her report to Administrator Beckman to present. The report included details
on monthly activities, previous fundraising events and upcoming programming.
RESULT: DISCUSSED
6. Financial Reports
HHS Director Johnson reviewed the Financial and Trust reports included in the Council
packet. The Trust account for Nutrition will be spent by the end of 2016. In 2017, those
expenses will be on the operating expense side. Revenue and expenses for both ADRC
and Aging are in line.
RESULT: DISCUSSED
7. Aging and Disability Resource Center At-a-Glance Report
Council members reviewed the ADRC At-a-Glance report comparing January - June
2015 to January - June 2016.
RESULT: DISCUSSED
8. Aging and Disability Resource Center Statewide Customer Survey in 2015
Council members reviewed the survey results from the Dr. Flowers survey of consumers
from the SAMS-IR database where clients receiving assistance through the ADRC are
entered. The overall results of the survey was favorable.
RESULT: DISCUSSED
9. Dementia Care Specialist Grant Funding update for 2017
The ADRC was notified that the Dementia Care Specialist will be fully funded by the
state in 2017.
RESULT: DISCUSSED
10. State Request for Proposal to Operate the Family Care Program Update
ContinuUs, Community Care Connections of Wisconsin, and Western Wisconsin Cares
will be merging in 2017 to create Community Links, Inc. They have responded to the
request for proposal by the state, but have yet to hear from the state that it has been
approved. Council members were provided with the notification that was sent to the
state.
RESULT: DISCUSSED
11. Information and Assistance Specialist/Options Counselor Update Sheet
Council members reviewed the October 2016 Information & Assistance Update Sheet.
There were a total of nine enrollments into ContinuUs and two disenrollments; there were
three enrollments into Care Wisconsin and no disenrollments; there were four
enrollments into IRIS and no disenrollments.
RESULT: DISCUSSED
12. Out of County and Weekend Senior Center Transportation Requests
There were a few out-of-county or beyond hours of operation requests related to the
utilization of the ADRC/Aging fleet vehicles.
December 3: Woodville to Menomonie
December 11: Glenwood City to Chippewa Falls
December 31: Glenwood City to Stillwater
A motion was made by Council member Zaske and seconded by Council member Brue;
motion carried.

REQUEST FOR FUTURE AGENDA ITEMS
ANNOUNCEMENTS & CORRESPONDENCE
Beckman read two Thank Yous for Elder Benefit Specialist and Disability Benefit Specialist
programs.
St. Croix County United Way Employee Campaign for 2016 are close to the fundraiser goal.
The Health Center Open House was on Tuesday, November 15. There was a nice turn out for
the event.
Beckman announced that the ADRC Secret Santa program was reinstated for this year.
