APPROVAL OF MINUTES
1. October 13, 2017 8:30 AM
Charir Ostness asked if there were any additions or corrections that needed to be made to the
minutes from October 13, 2017. Since there were none noted, Chair Ostness stated the minutes
will stand as written.

BUSINESS ITEMS
1. 85.21 Specialized Transportation Assistance Grant Application CY2018
Tracy Davis, Lead Community Services Coordinator, present a power presentation on the
85.21 Specialized Transportation Assistance Grant Application CY2018. She reviewed
the five projects being funded by the grant dollars and statistical data of each of the
programs plus how St. Croix County compares to surrounding counties. A motion to
accept the 85.21 Specialized Transportation Assistance Grant Application for CY2018
was made by Council member Zaske; seconded by Council member Brue; a voice vote
was taken, motion carried.

2. 2016-2018 Aging Unit Plan Amendments
Community Services Coordinator Davis presented the narrative portion of the Aging Unit
Plan Amendments. She reported there were no changes to the plan for the upcoming
year. Financial Manager Dr. Haug reviewed the 2018 budget that will be provided to the
state to accompany the Aging Unit Plan Amendments. A motion to approve the Aging
Unit Plan Amendments and 2018 Budget was made by Supervisor Anderson; seconded
by Council member Jonas; a voice vote was taken, motion carried.

3. 2016 St. Croix County Nutrition Programs Funding Analysis
ADRC Administrator Beckman presented the breakdown of the 2016 meal cost that was
reviewed during the October meeting. The charts presented broke down the funding
streams for the Nutrition Program as a whole - Congregate and Home-delivered meals as
well as breakdown for each of the respective programs. Council members reviewed and
discussed the charts provided.
RESULT: DISCUSSED
4. Financial Statements
Dr. Haug reviewed the financial reports up to September 2017. She stated there is
nothing out of line. She mentioned that receipt of payment for claiming is a bit behind
due to a change in reporting deadlines implemented by the state.

5. Information and Assistance Specialist/Options Counselor Update Sheet
Council members reviewed the October 2017 Information & Assistance Update Sheet. In
the month of October there were three enrollments into Inclusa and no disenrollments;
there were four enrollments into Care Wisconsin and no disenrollments; there were three
enrollments into IRIS and no disenrollments. The Council was informed that referrals
have increased and the Information and Assistance Specialist have been working more
with the Adult Protection Social Workers.

6. Out of County and Weekend Senior Center Transportation Requests
There were a couple of out-of-county or beyond hours of operation requests related to the
utilization of the ADRC/Aging fleet vehicles reviewed by Council.

Dec. 3 - Glenwood City to Menomonie
Dec. 16 - Glenwood City to Chippewa Falls
A motion to approve to transportation requests was made by Council member Zaske;
seconded by Council member Brue; a voice vote was taken, motion carried.

7. ADRC Secret Santa Program Serving Older Adults and People With Disabilities
Council members received a flyer explaining the ADRC Secret Santa Program. Council
members were informed to reach out to Tracy if they would like to make are referral or
assist in delivering. Beckman informed the Council the program is sustained from
generous donations from the Knights of Columbus, New Richmond and from fundraisers
held by the ADRC at the Services Center.

REQUEST FOR FUTURE AGENDA ITEMS
Chair Ostenss asked if there any topics Council members would like to discuss at future
meetings. Topics mentioned were Supportive Decision Making, an update on the Transit
Committee and provide Nutrition Advisory Council updates.
ANNOUNCEMENTS & CORRESPONDENCE
Council member Jonas announced the upcoming Ground Water Study meeting.
Beckman announced that a new part-time Disability Benefit Specialist has been hired and will
start on December 4. There is currently an opening for Nutrition Program Manager in Glenwood
City and interviews will be held next week.
The Virtual Dementia Tour was held in Hudson on Thursday, November 2. The event was very
well attended. WCCO was in attendance and there will be an upcoming episode
The Medicare 101 session were held on Wednesday, November 8 at the Hudson and Westfields
Hospitals. Both locations were well attended and will take place again in the spring.
The state will be administering a survey of the clients within the SAMS-IR data who work
closely with the Information & Assistance Specialist as well as the Dementia Care Specialists.
The DCS has applied for a couple of grants to assist in projects.
