Johnson ROLL CALL: Long, Gohr, Wick, Campbell, Johnson, Black, Crist, Wochomurka, Pizzi, Strenke, Doehrmann nd APPROVAL OF MINUTES Motion to approve by Strenke, 2 by Wochomurka, Minutes approved 1. 9/28/2018 meeting PUBLIC COMMENT none BUSINESS ITEMS 1. CCS Enrollment Data: Reported 101 current CCS recipients, 72 Youth, 29 Adults 2. Review of any new member referrals to the Coordination Committee: Wochomurka introduced Diane Crist as a new Community Member. 3. Goal and/or focus suggestions for Committee Member Recruitment: Wochomurka shared that we are looking for ideas to recruit members. Wick shared that he did a talk at WITC this month. Discussion to finalize paperwork/brochure to be able to use for recruitment. 4. Provider Networking: Campbell shared from the Provider Networking meeting came a st discussion of having another Provider Fair. The 1 Provider Fair was held in May 2018 and the target audience at that time was Service Facilitators, St Croix County Staff, and Providers. Would like to open the next Provider fair to a larger group. Looking to what that Target group would look like, what time of the day would work best, and where to hold the Provider Fair. WITC was suggested, Pizzi will look at dates available. Ready also mentioned. Reach out to Law enforcement, Priest, Hospitals, and other community providers discussed. Having information/handouts available at the Provider Fairwould also be helpful. (Providerinformation, recipient information, general CCS information). Looking at spring to hold the Provider Fair. It was suggested to do breakout sessions the day of the Provider Fair as well. A time frame of 4pm to 7pm suggested. Open to anyone who would like to help to organize and get the word out. Campbell also shared that at the Networking meeting it was suggested that a binder would be put together to include Provider information and available for reference. This would be helpful for St Croix County Staff as well as Recipients. Another suggestion from the meeting would be to create a video of Providers as a reference. 5. Suggestions as to how to define success of CCS: Discussion that St Croix County Administration team is looking for suggestions of how best to measure/define success of the CCS Program. Brought up that the CCS program is structured such that recipients define their own goals. Discussion on who this information would be shared with. County Board and State were a couple groups as to who would be looking for this information. Many ideas thrown out such as identifying key indicators verses benchmarks, having recipient driven measures of success, and measuring progress of goals. Discussed the qualitative aspect of the program. 6. Update regarding expansion of Shared Services Model: Shared that at the last Regional meeting Barron County still waiting to hear back from the state. 7. Updates from Work Groups Quality Improvement: Shared that that CCS surveys are continuing to go well. 8. Updates from CCS Fiscal Staff: Fiscal is working with Providers to establish 2019 CCS Service rates. The revised 2019 Rate Tool is being utilized. This Tool developed in collaboration with Barron, Eau Claire, and St Croix Count to hopefully help to simplify the Rate setting process. Fiscal and CCS Service Director continuing to work with Providers regarding progress note quality. Recommendations continuing to be communicated with Providers.
