Several excavations into the sand filter surface indicate that the surface is not biologically clogged and should not pose a restriction to wastewater infiltration through the filter's surface. To put this system back into operation the following should be completed: 1. The manhole riser rings on both the septic and dose tanks need to be disassembled and sealed watertight as they are reassembled. The septic tank manhole riser needs to be protected from physical damage by vehicles. 2. A water meter with an external reader needs to be installed in the building to monitor water usage. A substitute for the water meter is also acceptable as long as readings are accurate and data is accessible. 3. The gravel and sand cover which caps the ISF will be removed and replaced. 4. Observation pipes on the mcund wi!I be reelaced/repaired as necessary and extended to an elevation above grade such persons mowing grass can avoid them. 5. Pumps will be tested and replaced as necessary in .accordance with the approved plans. 6. Mound and ISF distribution laterals will be cleaned out. 7. Surtace grading up slope of the mound and ISF will be completed so that surface water is diverted around the system and will not pond in up slope areas. 8. The shower and laundry facilities shall be closed to help ensure that the system operates within its design flow. 9. It would be advisable to pump out the contents of the lSF and monitor the water levels within the filter for several days to determine if the liner is watertight. 10. A document will be developed for the owner describing proper maintenance and operation of the system. My last meeting with the managers was on October 17, 2001. The high water alarms for both the dose tank and sand filter were activated. The water level in the dose tank is nearly to the top of the manhole riser and sewage effluent is leaking out of the uppermost manhole riser joint and onto the ground surface. Technically, this system. is failing by surface discharge of sewage effluent. Such discharges are prohibited by Comm 83.32(1)(f), Wis. Adm. Code and are considered a human health hazard as defined ins 254.01(2) Wis. Statutes. The owners need to take the following actions to correct the system failure and abate a human health hazard. 1. Begin servicing the septic tank at regular intervals. Septic tank capacity is 1000 gals. The best way to begin servicing is to have the septic tank pumped out weekly and to note if levels were high enough to begin filling the dose tank that can also be pumped out as needed. This will establish the service frequency and possibly help establish the approximate load on the system. Pumping and hauling needs to begin immediately and continue until the system malfunction is corrected. 2. At the time the septic and dose tanks are initially pumped, the pumper should also pump out the sand filter pump basin. The capacity of the filter is approximately 5,000 gallons. 3. Engage a licensed plumber to repair failed mechanical components, inspect all portions of the system, and repair/renovate/replace the sand filter to bring it back online as a pretreatment component. 4. Obtain all permits and approvals for the repair of the sand filter component. 5. Sand filter systems require regular maintenance which this unit has never received. The owner will need to enter into a service agreement with a certified person or company to provide operational oversight and system maintenance for the life of the system.. The system must be maintained and operated with the manufacturer's and designer's specifications. This will generally result in one to two inspections per year and septic/dose tank servicing once every three years. The septic tank outlet filter also needs regular maintenance and that interval can be established by routine inspection. However, I do not feel that the septic tank was serviced properly since the manhole cover did not appear to have been excavated and opened. This tank should contain an outlet filter that needs periodic cleaning. Neither the filter nor tank cleaning can be properly completed without opening the manhole cover. At my last visit in August I questioned the manager about water usage within the building since hydraulic overloading was a possibility. The original system was designed for 12 employees and was limited by the capacity of the modified mound to a flow of 240 gpd. However, the sand filter is capable of treating 450 gpd. Current problems appear to be associated with the sand filter and not the modified mound portion of the system. The sand filter is currently ponded as was previously observed. Several items listed in my inspection report of April 27, 1999, in my opinion, have not been completed as listed below and are the responsibility of the installer to complete: 1. No documentation has been supplied that the septic and dose tanks have been tested and shown to be watertight (including manhole risers). 2. Final switch settings and dose volumes in the dose tank have not been verified to be as per the approved plans. 3. I stilt feet that the area up slope and along the entire length of the modified mound is not draining freely to the east. 4. The locking cover on the dose tank needs adjustment to be secured properly. 5. The septic tank manhole must terminate above grade with a locking manhole cover as per the stipulations for installation of the septic outlet filter approval. In addition, other actions. must be taken that include: 1. Properly servicing the septic, dose tank, and sand filter pump basin. This needs to be done under county and/or state supervision. 2. Renovation of the sand filter by pumping, aeration, or sand/gravel media removal and replacement. Approval and permits are likely required for media removal and replacement. 3. Distribution laterals must be cleaned out with a brush or pressure sprayer to ensure orifices are clear. MTS of Wisconsin October 17, 2001 Page 2 of 2 4. Cleaning of the aggregate over the sand filter of all weeds and grass. 5. Repair of at least one broken observation pipe in the modified mound. 6. Water meter installatson may still be helpful to monitor inflow to the system in consideration that additional employees and water using devices are installed in the building. 7. A maintenance contract with a licensed POWTS maintainer is needed to ensure proper maintenance in the future. I contacted Troy Thomas, Vice President of Operations at MTS. yesterday to arrange a meeting with him to discuss the repair of the system. It looks like we may be able to meet out at the site next Wednesday or Thursday. You are welcome to meet with us at that time, and I'll let you know when. I'm not sure exactly what went wrong with this installation, but as you pointed out, there is some scum floating around in the dose tank and I noted this same condition in the pump vault in the sand filter too. When I stopped by there for a few minutes on August 2, the high water alarm went off 3 times in less than 5 minutes. I suspect what is occurring is that the sand filter is ponded and when wastewater is discharged from the dose tank to the filter most of it returns to the tank and sets the alarm off and cycles the pump on again to relieve the high water alarm condition. You stated that you were out to the site on Sunday, July 15, 2001, and felt that the water level in the dose tank was normal and that you did not observe water flowing back into the tank. However, depending on the number of employees present on a Sunday wastewater flows may not have been representative of normal conditions during the workweek. I'm glad to heir #hat you completed many of the items mentioned in my April 27, 1999 inspection report. However, it would have been nice to be contacted for an inspection when the work was being done so we could verify the test results. If you have any notes relating to the dates tests were completed and the results of the tests, please send them to me for review as soon as possible. I still feel that the final grading around the at-grade structure is inadequate. I have several times had to walk through 4-6 inches of water ponded up slope of the at-grade and feel that drainage could still be made much better. In retrospect, the issue of the water meter installation may bean important one in that we would have a better idea of water usage in the building. The plan approval letter for this project states that the water meter installation was requested by the university for monitoring purposes, and that the owner was suppose to get the meter from the university and report back to them at intervals as requested. The owner may not have been notified of this requirement. In addition, I do not believe that the university included this project in their final research so they made no requests for meter readings and thus no meter was installed. At the time of this approval and even currently, there is no absolute requirement to install a water meter on sand filter installations. One thing we could do is calculate flows to the filter based on the number of dose cycles and pump run times. I have a number of readings recorded for August 14, 2001 Proud Excavating Letter Page2of2 1999 and 2000 when the system appeared to be in working order. One thing these readings won't tell us is if there is a leaky tank since we cannot compare water usage in the building to the wastewater discharged to the system. This is where your tank water tightness test results would come in handy. The bottom line here is that the system is not working well at his time and is considered failing from a treatment standpoint. At that time we found the following: 1. Lock was not locked on lift tank. 2. Avery large amount of sludge in left tank (Would like to see records of septic tank pumping. Also, did the interior plumber even install a water meter so this system can be monitored for usage? If not, why have they not been ordered to? It was in the plan approval. 3. While we were there water in tank was at proper level and within one hours time not water was noticed running in from the septic tank or sand filter. 4. We have no problem trying to correct anything that might be wrong do to our workmanship. However, before I would spend any of my time and MTS's money, I think we should be monitoring the amount of water going into the system, then adjust accordingly. Regarding Inspection Report of 4-27-99: Items 1-10 were done in 1999 other than electrical work on control panel. Item 11: Was not done per Harold of MTS, they were going to pour concrete in order to have access that way. Item 12: Done. Item 13: I can't remember if this was done or not. Remove excess vegetation by mowing. Place 4-6" of sand, then chisel plow it so the sand falls into the openings. .After plow- ing, place some sand in the area where the aggregate is placed and level it before placing the aggregate. When chisel plowing, fiRY not to pull the subsoil up to the top. When drilling 1/8" holes use a sharp bit and make sure all the burrs and shavings are removed. Don't force the drill bit as it will burn the hole instead of cut the hole in. The owner is responsible for all costs associated with the unit, including any repair costs. The owner is to install a water meter and purchase the meter from the University of Wisconsin-Madison, College of Agricultural and Life Sciences. The owner reads the water meter monthly by recording it on a card that the University sends them monthly. The water meter should be installed to record the water used within the facility and not what is used outside the unit through faucet bibs. For single pass sand filters the distribution system must be flushed at least once a year to minimized hole plugging and the efflu- ent monitored at least annually. This maintenance should be done by a professional but can be done by a trained homeowner. THE CONTRACTOR MUST MAKE SURE THAT THE HOMEONWER UNDERSTANDS THAT THE UNIT NEEDS MAINTENANCE ON A REGULAR BASIS AND THAT THERE WILL BE A COST ASSOCIATED WITH IT. In time, some of the orifices become plugged, the pressure in the pipes generated by a high-head turbine pump increases sharply until it's high enough to scour the orifices. For example, a typical sand filter design with 5 ft. of residual pressure and sixty 1/8-inch orifices, 44 ft. of static lift, a on a inch hose and valve -assembly and 300 ft. of 1-1/4" dia. (SEVERAL OF YOU MAY NOT HAVE RECEIVED THE PREVIOUS ONE) My records indicate that you will be installing a single pass sand filter for the m7S - ~^/~-~-- site. There aze currently over 50 sand filters installed around the state. We are monitoring most of them and my graduate student is inspecting many of them. Based on very limited information so far, I have several points that I would like to share with you about the sand filters so you can incorporate these points when constructing the sand filter. As indicated, you received a letter dated April 1 ~ or 30 from me requesting some changes be made during the installation of the sand filter. Those changes aze still appropriate except for one which needs some modification. SEVERAL MORE HAVE BEEN ADDED. To reduce and save confusion, I am repeating what I sent out previously and also providing an update of other things that have come to light since my last correspondence. Please toss the previous Letter. It is important that all these items be incorporated during the construction of the sand filter. 1. Depth of installation. After final grading, there should be less than 6" of soil cover over the unit. The final grade should be higher than the surrounding soil so surface waters are directed away from the unit. If necessary, add extra No.l stone to the top of the sand filter so there is less than 6" of soil cover. Be aware of the elevation difference between where the pipe exits the internal pump chamber and the soil absorption unit distribution laterals. It is essential that the force main drains either to the pump chamber or the soil dispersal unit after each dose. 2. Soil cover. The soil texture must be sandy loam or lighter. Silt loam and clay loam soils are too heavy and will restrict the movement of air into the unit. If the homeowner is willing, you might place decorative rock on the surface of the unit without any soil. This will allow for maximum oxygen transfer. If decorative rock is used, consider placing a layer of fabric about 1" above the laterals, placing two inches of sand and then placing the decorative stone on top of the sand. This will minimize any potential odors that may occur during dosing. (If interested, I can provide a drawing). The unit will not freeze. Make sure that surface water is diverted away from the sand filter. This must be done regardless if soil or rock cover is used. 3. Observation tubes. Place two observation tubes with the bottoms at the aggregate/sand (where the effluent enters the sand near the laterals) interface. Put slots in the sides and secure them using toilet flanges or suitable means. Do it the same way you do for mounds and in-ground units. Bring them to the surface with caps. If slip caps are used, put several saw marks about 2" deep into the top of the pipe so the caps slip on easier. 4. Floats in the internal pump cham'oer. Make sure the on-float (middle float) is set so the pump comes on when the liquid level reaches just the top of the underdrain. THE LIQUID LEVEL IS NOT TO GO PAST THE TOP OF THE PIPE as it may interfere with oxygen transfer in the system. The floats on the float trees are preset at the factory, but sometimes the trees may be too short. This can be easily checked as the screw holding the middle float must be at the same elevation as the top of the underdrain pipe. With float tree in place, measure from the top of the vault to the top of the tree. Pull the float tree out and measure from the top of the tree to the screw holding the middle float. These two distances combined should equal the distance from the top of the pump chamber vault to the top of the under- drain. If they do, then place the float tree in position. If not, then adjust the on (middle) float accordingly. If it is adjusted, also adjust the others the same. The distance between the screws for the on (middle) and off (bottom) is two inches. The alarm (top) float should be 1.5" above the on (middle) float. IF THERE IS A QUESTION, PLEASE CONTACT ME AT 608-262-1106. If you are using a single tank with a septic vault, make sure that it is easy to remove the pump and screen (filter) for cleaning. That means the electrical box is next to the wall and not protruding out excessively from the wall. Also make it so that the pump can be removed easily. It is my understanding that the splice boxes can be placed inside the pump chambers and septic tanks if it is the Orenco splice box. Local codes may prohibit this. 6. Underdrain orientation. Place the underdrain pipe (4" slotted pipe) in the sand filter with the slots vertical instead of horizontally on the top. This will allow more flexibility in changing float setting if needed for dosing the soil dispersal unit. r 7. Circuits. Connect either a 30 amp circuit or two 20 amp circuits from the house electrical box to the control panel. Wire size will be dependent upon distance. Smaller circuits may cause overload when both pumps come on at the same time disconnecting the circuit breaker and disenabling the alarm. Connect the alarm to the sand filter pump circuit as that is the least likely one to blow. 8. Septic tankiPump chamber risers. It is extremely important that the risers do not leak at the joints and at the septic tank connection. Take extra care to make sure leakage is avoided. 9. Timer adjustments. You will need to return to make adjustments and fine tune the system to fit the water use habits of the family. Timer adjustments and float level adjustments (in septic tank pump chamber) may be required. To estimate water usage, use 50 gpd/per person living in the home. It is critical that adequate surge storage capacity be provided. The net dose volume is approximately 15 gallons/dose. The instructions with the sand filter should tell you how to set the dose volume. There is also a procedure outlined in the Sand Filter publication described below. A publication entitled "Single Pass Sand Filter for On-site Treatment of Domestic Waste" is available through our publication office. The publication has a construction section showing astep-by-step procedure for installing single pass sand filters. It may be very helpful, especially if you have not yet installed a sand filter. You can order it by calling our SSWMP office at 608-265-6595. It is number 16.7. There is a $2.50 charge. There are currently over 50 sand filters installed around the state. We are monitoring most of them and my graduate student is inspecting many of them. Based on very limited information so far I have several points that I would like to share with you about the sand filters so you can incorporate these points when constructing the sand filter. L Depth of installation. After final grading there should be less than 6" of soil cover over the unit. The final grade should be higher than the surrounding soil so surface waters are directed away from the unit. If necessary, add extra no. 1 stone to the top of the sand filter so there. is less than 6" of soil cover. Be aware of the elevation difference between where the pipe exits the internal pump chamber and the soil absorption unit distribution laterals. It is essential that the force main drains after each dose. 2. Soil Cover. The soil texture must be sandy loam or lighter. Silt loam and clay loam soils are too heavy and will restrict the movement of air into the unit. If the homeowner is willing, you might place decorative rock on the surface of the unit without any soil. This will allow for maximum oxygen transfer. If decorative rock is used, consider placing a layer of fabric about 1" above the laterals, placing two inches of sand and then placing the decorative stone on top of the sand. This will minimize any potential odors that may occur during dosing. (If interested, I can provide a drawing). The unit will not freeze. Make sure that surface water is diverted away from the sand filter. This must be done regardless if soil or rock cover is used. 3. Observation Tubes. Place two observation tubes with the bottoms at the aggregate/sand (where the effluent enters the sand near the laterals) interface. Put slots in the sides and secure them. Do it the same way you do for mounds and in-ground units. Bring them to the surface with caps. University of Wisconsin-Madison provides equal opportunities for admission and employment. 4. Floats in the internal pump chamber. Set the "on" float in the internal pump chamber so the pump starts to discharge before the high liquid level reaches the mid point of the collection pipes sticking into the pump chamber. This will allow for some air circulation in the pea gravel below the sand media. 5. Septic tank vault. If you aze using a single tank with a septic vault, make sure that it is easy to remove the pump and screen (filter) for cleaning. That means the electrical box is next to the wall and not protruding out excessively from the wall. Also make it so that the pump can be removed easily. 6. Septic tank/Pump chamber risers. It is extremely important that the risers do not leak at the joints and at the septic tank connection. Take extra care to make sure leakage is avoided. A publication entitled "Single Pass Sand Filter for On-site Treatment of Domestic Waste" is available through our publication office. The publication has a construction section showing a step-by-step procedure for installing single pass sand filters. It may be very helpful, especially if you have not yet installed a sand filter. You can order it by calling our SSWMP office at 608- 265-6595. It is number 16.7. There is a $2.50 charge. Nov. 20, 1997 Audrey Thomas faxed the Zoning Department a letter from Jim Converse, P. E. Professor with the University of Wisconsin. Jim Converse is also responsible for the Small Scale Waste Management Project. The letter stated that the site would be accepted into his highly pretreated effluent study. Audrey thought this approval would allow her to obtain a sanitary permit so she could ebtain a building permit to start construction. I called Audrey back to advise heron how to proceed. First, I informed Audrey that sanitary permits are only issued to licensed plumbers and I could. not issue a sanitary permit based on this letter she faxed. Then I told her that this system still needed approval from the Department of Commerce. It was at this point Audrey became upset and said that the county didn't need to be involved if the state was going to approved the site. Then she complained about the government. Dec. 22, 1997 The St. Croix County Zoning Department received a copy of the approval letter from the Department of Commerce for an experimental onsite sewage system for the MTS of Wisconsin, Ltd. This approval letter was carbon copied to the installing plumber (Proud Excavating). This system was granted approval by the Department of Commerce subject to conditions. Condition 2 reads: This approval is based on Wisconsin Division of Safety and Buildings requirements. It will be necessary to obtain a sanitary permit. Failure to obtain installation approval from local authorities will automatically void this acceptance. Jan. 12, 1998 Town of Cady notified the Zoning Department that MTS of Wisconsin, Ltd. started construction without a sanitary permit. )im called Audrey Thomas and informed her that a sanitary permit is required prior to the start of construction. Audrey became very upset after hearing that a sanitary permit was needed from the county. Jim told Audrey that fines would be assessed immediately. I sent the notice of violation on January 12, 1997 to Audrey Thomas. Jan. 13, 1998 Jim took pictures of the site. Jan. 15, 1998 The sanitary permit for MTS of Wisconsin, Ltd. was issued to Proud Excavating. Jan. 16, 1998 After discussing the matter with Greg, he recommended that we should not seek a forfeiture through the court system for the violation, but instead issue a citation for every day the violation existed. A second notice of violation was sent to Audrey clarifying that in lieu of seeking a forfeiture through the court system for the violation, a citation was going to be issued for each day of the violation. I issued three citations to MTS of Wisconsin, Ltd. MTS of Wisconsin, Ltd. was given notice in the letter dated January 16, 1998, to appear on February 4, 1998 at 10:00 a.m. or make a cash deposit with the County Clerk of Court by the same date. Feb. 4, 1998 Initial court appearance for MTS of Wisconsin, Ltd. at 10:00 am. No one from MTS was represented at this hearing. The sanitary system received state approval from the'Department of Commerce on December 17, 1997. The approval letter arrived in the Zoning Office on December 22, 1997 with conditions. Code 15.04 (3)(b) St. Croix County Zoning Ordinance Correspondences: November 20, 1997, I had a phone conversation with Ms. Thomas regarding the need for the sanitary permit before the start of construction. Ms. Thomas faxed a letter to the Zoning Department regarding the sanitary system design and stated no permit was necessary from the county. On January 12, 1998, the township contacted Jim Thompson and informed him that the building was under construction. A building permit was never issued by the township for that construction. January 12, 1998, was the date that the Zoning Department became aware of the violation. (Sand filter with a modified mound) Town of Cady, St. Croix County, Wisconsin Dear Bill: I'm writing to inform you that the sanitary permit for MTS of Wisconsin, Inc is on hold until the benchmark has been identified by the original soil tester, Lynn Misfeldt. The benchmark was not located when the septic and dose chamber were installed on July 17, 1998, and is believed to have been destroyed during the construction of the parking lot. If the site has been destroyed then a new state plan will be. required. The building will not be allowed to be occupied until these issues get resolved. I also want to remind you that apre-construction meeting with the installing plumber, the county inspector, and the district wastewater specialist is required prior to the commencement of the system installation. Once again, the MTS of Wisconsin, Inc. (permit # 307618) is on hold until the benchmark has been identified and until the pre-construction meeting has been held.
