RESOLUTION TO PARTICIPATE IN THE STATE OF WISCONSIN
DEPARTMENT OF EMPLOYEE TRUST FUNDS PUBLIC
EMPLOYERS' GROUP INCOME CONTINUATION INSURANCE
PROGRAM
1 WHEREAS, pursuant to Wis. Stat. § 40.61 and the rules of the Wisconsin Department of
2 Employee Trust Funds (ETF), the Income Continuation Insurance (ICI) Program offers income
3 protection for public employees during periods of disability; and

5 WHEREAS, St. Croix County is currently a participating employer under the Wisconsin
6 Retirement System (WRS), which is a requirement for eligibility to offer ICI coverage; and

8 WHEREAS, the St. Croix County Board of Supervisors recognizes the value of
9 disability income protection and the importance of enhancing employee benefits to recruit and
10 retain a qualified workforce; and
11
12 WHEREAS, participation in the ICI Program requires the offer of coverage to all WRS-
13 covered employees, and at least 65% of eligible employees must enroll during the initial
14 enrollment period for the resolution to remain valid; and
15
16 WHEREAS, the County Board has determined that St. Croix County shall contribute an
17 amount equal to the full premium for the 90-day elimination period in accordance with program
18 rules.
19 NOW, THEREFORE, BE IT RESOLVED that the St. Croix County Board of
20 Supervisors hereby elects to participate in the Wisconsin Public Employers' Group Income
21 Continuation Insurance Program administered by ETF, effective on the first of the month
22 following at least 90 days after ETF receives a certified copy of this resolution, or such later
23 effective date as specified.

25 BE IT FURTHER RESOLVED that the Human Resources Director is authorized to
26 submit the Resolution for Inclusion (Form ET- 1326) and any other required documentation to
27 ETF on behalf of St. Croix County.
