UNFINISHED BUSINESS:
WIS Tech proposal to provide low interest loans to persons with
disabilities to purchase new or used assistive technology devices. Not
enough information available and also, the time limit is over for letters of
support. We will keep an eye on this grant and look into it next year if they
need support again.
State Biennial Budget Ron handed out an analysis by DAWN of the
positions taken by the governor and other constituents with proposed
vetoes and approvals to the biennial budget.

Financial Report: Report of expenses for January through June 2001 was presented.
All program expenses are on target.
Variance & Purchase Requests: None
Nursing Home relocation requests: R.Lockwood requests endorsement from this
committee for relocation slots from the state. These are funds available now to support
moving persons from nursing homes to community placements. The funds will continue
in the future by being allocated as a permanent increase to our COPW budget. We
have one client approved for this and one in process at the present time. Motion to
endorse applications for nursing home relocation slots by Gregerson; second by Boche;
carried.
Waiting list policy change: After discussion, a motion to change wording in #4
regarding the rejection of services to the following: "Persons who rise to the top of the
waiting list and subsequently reject an offer of application for services, but who wish to
be considered for application at some future time, will be moved to the bottom of the
waiting lists ". Motion made by Viellieux; second by Wolske; carried.
Calendar Year 2002 budget: Ron Lockwood distributed update of the budget request
that will be presented to the HSB. Ron noted that there was an additional $100,000
from MAPC that was collected in year 2001 for services in 2000. Because of the type of
accounting system we have, these monies are reported in the year they are received,
therefore, the $100,000 in collections is considered current year revenue. This will
represent a decrease to the community aides money needed and increase the amount
for carryover in the general fund. Motion to approve submitting the HSB with this
change made by Wolske, second by Boche; carried. Please call Ron if there are any
questions on this.
A13#473- Studying the future of DD Centers. Lockwood encouraged members to
watch this bill and possibly consider communicating concerns regarding this bill to reps
K.Rhoades and M.Pettis. A recommendation by the committee was made to have R.
Lockwood communicate with the state reps and put any response on the next meeting.
Discussion & questions regarding Foster Care. Will contact Fred Johnson, Family
Children's Services to come to the next meeting and present information regarding
such.
Motion made to adjourn by Peterson, second by Palewicz. Motion carried.

DEFINITIONS
PERSONS WITH LONG TERM SUPPORT NEEDS
Developmental disability - Delay in development attributed to one or more of the
following conditions:
Mental Retardation Autism
Cerebral Palsy Brain Trauma
Epilepsy
-Or-
a condition that is "like mental retardation" because the condition results in similar
impairment
-and-
- is manifested before age twenty -two (not applicable to brain trauma)
- is likely to continue indefinitely
- results in substantial functional limitations in three or more of the following
areas of major life activity: (1) self -care, (2) receptive and expressive
language (3) learning, (4) mobility, (5) self direction, (6) capacity for
independent living and (7) economic self - sufficiency
-and-
- reflects a need for special, interdisciplinary treatment or other services which
are long term in nature.
Frail Elderly - persons 65 or older who meet functional and financial criteria.
Other Disabled - persons with long term (12 month or longer duration) or
irreversible mental or physical illness or disability.
CASE MANAGEMENT - Those services which assist individuals in gaining access to
and managing needed medical, social, educational, vocational, rehabilitation, financial
and other services:
1. Assessment
2. Developing Case Plan
3. Ongoing Monitoring - one or more of the following activities performed by a
single and identifiable person
Ascertain what services have been or are being offered and whether
they are adequate
* Supportive contact to client to ensure services are meeting need and to
Monitor clientlfamily satisfaction
' Revisions of case plan as needed
CRISIS SITUATIONS: the client is deemed to be in an emergency situation and there
is a lack of natural supports necessary to assure safety.

PROCEDURE FOR SERVICE WAITING LIST
St. Croix County will use the following procedures for placing new applicants on
waiting lists for Community Options (COP) or Medicaid Community Waiver (CIP IB /CIP
IUCOPW) funded services:
1. Make a preliminary determination of the applicant's eligibility by documenting
that on the date of placement on the waiting lists: .
a. There has been contact with the applicantlapplicant's guardian and
that the applicant has long term care needs; and
b. The applicant is eligible for funding as determined by the COP
Functional Screen; and
c. A preliminary determination has been made, based on the
applicant's unverified declaration of income and assets, that the
applicant is likely to be financially eligible for funding; and
d. The applicant's declaration of state residency meets the funding
requirements.
2. Place the name of the applicant on the waiting list for Community Options
and /or Medicaid Community Waiver funding.
3. Offer the applicant a choice of an immediate assessment to be completed
within 45 days (See COP guidelines 5.05 D), or delaying the assessment
until a time closer to the point that funding will be available.
4. Provide the applicant with:
a. The applicants status on the waiting list, and an estimate of when
funding may be available: and
b. A copy of the county waiting list policy and procedures; and,
c. A copy of the county appeal/grievance procedures; and,
d. Information about other long term support programs for which the
applicant may be eligible and how to apply for these programs; and,
e. Information about potential estate recovery of the cost of any
services provided.

St. Croix County will use the following procedures for serving persons from waiting
lists:
1. New or increased service needs of current program participants which the
care manager has identified in the participant's most recent care plan as
necessary to maintain the participant in the community, and to adequately
provide for the participant's health, safety and wellbeing, shall be given
priority over providing services to new participants.
2. Priority status will be given to:
a. persons from target groups needed to satisfy the significant
proportions requirements,
b. to persons who are terminally ill, and/or persons who have been
protectively placed under Chapter 55,
c. persons in IMD slots who become 65 years of age,
d. persons who meet "crisis situation" definition on page 1.
e. children in out of home placements supervised by Family & Children's
Services when Family & Children's Services is assuming
responsibility for local match requirement.
3. In the absence of persons with priority status, persons on the waiting list will
be served in the order of the date of placement on it.
4. Persons who rise to the top of the waiting list and subsequently reject an
offer of application for services, but who wish to be considered for
application at some future time, will be moved to the bottom of the waiting
lists.
