FOR HEALTH INSURANCE FOR NON - REPRESENTED EMPLOYEES

St. Croix County, Wisconsin
WHEREAS, the Personnel Committee recommends a single coverage category and a family
coverage category for health insurance for St. Croix County non - represented employees, with different
premiums for each category; and
WHEREAS, the Personnel Committee believes the two categories represents a more traditional
approach to health insurance coverage and assesses more cost to more numerous users of the plan; and
WHEREAS, this change takes effect with the December, 2004 payroll payment for coverage
effective January 1, 2005; and
WHEREAS, an employee who chooses coverage for only himself /herself will be in the single
coverage category and an employee who chooses coverage for himself/herself and one or more
dependents, as defined in the health insurance plan, will be in the family coverage category.
WHEREAS, the premium for each category will be based on calculations received from the
County's reinsurance provider and/or independent advisor.
THEREFORE, be it resolved by the St. Croix County Board of Supervisors that, for the reasons
stated above, it approves and establishes a single coverage category and a family coverage category for
health insurance for St. Croix County non- represented employees.
