HomeMy WebLinkAboutResolution 2004 (44) RESOLUTION NO. 4LI Ca Mk) Authorizing Application For Resource Center Grant Whereas, The Wisconsin Department of Health and Family Services is soliciting proposals to develop and implement Aging and Disability Resource Centers (ADRC), and Whereas, the ADRC concept is intended to offer the general public a single entry point for information and assistance on issues affecting older people, people with disabilities and their families; and as a clearinghouse of Tong -term care information available to physicians, hospital discharge planners, or other professionals working with older persons or people with disabilities, and Whereas the ADRC would function as a catalyst for community prevention strategies to prevent or delay the use of publicly funded long -term care, and Whereas, the ADRC would provide seamless access to public benefits, as well as counseling and advocacy to overcome barriers to using benefits, and Whereas, federal grant funds would at minimum support a project planner /manager for one year and state funds for the development of a Disability Benefit Specialist for adults under age 60, and Whereas the local match requirement can be in -kind and met by the current and ongoing county programs and services which are closely related to the goals and purposes of an ADRC, and Whereas, St Croix County is committed to providing quality long -term care services to older persons and people with disabilities in our county, Now, therefore be it resolved that The St Croix County Board of Supervisors authorizes the St Croix County Health and Human Services Board to apply for the above referenced ADRC development and ongoing operating grant in collaboration with the Department on Aging, and Be it further resolved that if the grant is awarded the plan for full implementation to serve all target groups (developmental disabilities, physical disabilities, mental illness and substance abuse, elders with disabilities, healthy elders, young adults with disabilities and family caregivers) will be completed within the three year span allotted, and Be it further resolved that the program sunsets in the absence of continued federal and state financial support.
