TREATMENT TECHNOLOGIES LISTED IN WISCONSIN ADMINISTRATIVE
CODE SECTION COMM 83.04 -2.

ORDINANCE TO AMEND ST. CROIX COUNTY
RECORDS RETENTION ORDINANCE

The St. Croix County Board of Supervisors does ordain as follows:
WHEREAS, Article IV, sec. 0, par. 4, of the St. Croix County Records Retention
Ordinance requires the St. Croix County Nursing Home to retain patient accident reports for 5
years; and
WHEREAS, the information contained in the patient accident reports is also contained in
the resident's (patient's) medical record, which the Nursing Home is required to keep for 5 years
after discharge or death (Article IV, sec. 0, par. 1); and
WHEREAS, it is not required that a separate patient accident report be maintained.
THEREFORE, the St. Croix County Records Retention Ordinance is amended as follows:
Article IV, sec. 0, par. 4 is repealed.

B. The County Planning, Zoning and Parks Committee shall not grant
preliminary or final approval to any major or minor subdivision of land that
proposes the use of any technology prohibited in section III, above, on
sites that have Tess than 24 inches of suitable soil depths or slopes greater
than 12% or more than two residential units on one system.
C. This ordinance does not prohibit the use of the technologies prohibited in
section III, above, as replacement systems.
D. "New development" means property without an existing habitable building.
VI. Penalties
Any person, corporation or other entity who violates the provisions of this
ordinance shall be subject to civil forfeitures and injunctions as provided by sec.
17.71, St. Croix County Zoning Ordinance.
VII. Severability.
If any portion of this ordinance is found invalid by a court, that finding shall not
affect the applicability of any remaining portions of the ordinance which can be
applied without reference to the invalid portion.
VIII. Effective Date.
This ordinance shall become effective upon its enactment by the St. Croix
County Board of Supervisors and publication.
