The submittal has been CONDITIONALLY APPROVED. The owner, as defined in chapter 101.01(10), Wisconsin Statutes, is responsible for compliance with all code requirements. No person may engage in or work at plumbing in the state unless Licensed to do so by the Department per s.145.06, stats. Soil compaction, excavation, vehicular traffic and other similar activities that impact the treatment and dispersal are prohibited. DALE E HUDSON Page 2 1/19/2005 • The well must be a minimum of 25 feet from any POWTS tank, and a minimum of 50 feet from the absorption area. chs. NR 811 & 812c • A Sanitary Permit must be obtained from the county where this project is located in accordance with the requirements of Sec. 145.135 and 145.19, Wis. Stats. • Inspection of the private sewage system installation is required. Arrangements for inspection shall be made with the designated county official in accordance with the provisions of Sec. 145.20(2)(d), Wis. Stat • Comm 83.22(7) A copy of the approved plans, specifications and this letter shall be on-site during construction and open to inspection by authorized representatives of the Department, which may include local inspectors. Owner Responsibilities: • Comm 83.52 Responsibilities. The owner of a POWTS shall be responsible for ensuring that the operation and maintenance of the POWTS occurs in accordance with this chapter and the approved management plan under s. Comm 83.54(1). • Comm 83.52(2) A POWTS that is not maintained in accordance with the approved management plan or as required under s. Comm 83.54(4) shall be considered a human health hazard. • Comm 83.55 The owner is responsible for submitting a maintenance verification report acceptable to the county for maintenance tracking purposes. Reports shall be submitted at intervals appropriate for the component(s) utilized in the POWTS. All permits required by the state or the local municipality shall be obtained prior to commencement of construction/installation/operation. In granting this approval the Division. of Safety & Buildings reserves the right to require changes or additions should conditions arise making them necessary for code compliance. As per state stats 101.12(2), nothing in this review shall relieve the designer of the responsibility for designing a safe building, structure, or component. The above left addressee shall provide a copy of this letter to the owner and any others who are responsible for the installation, operation or maintenance of the POWTS. Adm. Code en I This system shall be operated in accordance with Comm 82-84 Wis. Adm. Code, and shall maintained in accordance with its' component manuals [SBD-10691-P (N.01 /01) and SSWMP Publication 9.6 (01 /81 )) and focal or state rules pertaining to system maintenance and maintenance reporting. No one should ever enter a septic or pump tank since dangerous gases may be presets that could cause death. Septic and pump tank abandonment shall be in accordance wtth Comm 83.33, Wis. Adm. Code when the tanks are no longer used as POWTS components. Septic or pump tank manhole risers, access risers and covers should be inspected for water tightness and soundness. Access openings used for service and assessment shall be sealed watertight upon the completion of service. Any opening deemed unsound, defective, or subject to failure must be replaced. Exposed access openings greater than 8-inches in diameter shall be secured by an effective locking device to prevent acadeMal or unauthorized entry into a tank or component. Septic Tanll The septic tank shall be maintained by an individual certified to service septic tanks under s. 281.48, Stats. The contents of the septic tank shall be disposed of in accordance with NR 113, Wis. Adm. Code. The operating condition of the septic tank and outlet filter shall be assessed at I~st once every 3 years by inspection. The outlet fitter shah tie cleaned as necessary to ensure proper operation. The filter cartridge should not be removed unless provisions are made to retain solids in the tank that may slough off the fitter when removed from its enclosure. If the fitter is equipped with an alaml, the f+iter shall be serviced if the alarm is activated continuously. Intermittent filter alarms may indicate surge flows or an impending continuous alarm. The septic tank shall have its contents removed when the volume of sludge and scum in the tank excee<Js 1/3 the liquid volume of the tank. If the contents of the tank are not removed at the time of a triennial assessment, maintenance personnel shall advise the owner of when the next service needs to be pertonned to maintain less than maximum scum and sludge accumulation in the tank. The addition of biological or chemical additives to enhance septic tank pertormance is generalty not required. However, if such products are used they shall be approved for septic tank use by the Department of Commerce. pro Tank The pump (dosing) tank shall be inspected at least once every 3 years. All switches, alarms, and pumps shat! be tested to verify proper operation. If an effluent fitter is installed within the tank it shall be inspected and serviced as necessary. Mound and Pressure Distribution Svstem No trees or shrubs should be planted on the mound. Plantings may be made around the mound's perimeter, and the mound shall be seeded and muk;hed as necessary to prevent erosion and to provide some protection from frost penetration. Traffic (other than for vegetative maintenance) on the mound is not recommended since soil compaction may hinder aeration of the infiltrative surface within the mound and snow compaction in the winter will promote frost penetration. Cold weather installations (October-February) dictate that the mound be heavily mulched as protection from freezing. Influent quality into the mound system may not exceed 220 mg/L BOD5,150 mg/L TSS, and 30 mg/L FOG for septic tank effluent or 30 mg/L BODS, 30 mg/L TSS, 10 mgJl FOG, and 104 cfu/100 mL for highly treated effluent. Influent fk-w may not exceed maximum design flow specified in the permit for this installation. The pressure distribution system is provided with a flushing point at the end of each lateral, and lt is recommended that each lateral be flushed of accumulated solids at least once every 18 months. When a pressure test is pertormed tt should be compared to the initial test when the system was installed to determine if orifice clogging has occurred and if orifice cleaning is required to maintain equal distribution within the dispersal cell. Observation pipes within the dispersal cell shall be checked for effluent ponding. Ponding levels shall be reported to the owner, and any levels above 8 inches considered as an impending hydraulic failure requiring addfional, more frequent mon'doring. Contingency Plan If the septic tank or any of its components become defective the tank or component shall be repaired or replaced to keep the system in proper operating condition. If the dosing tank, pump, pump controls, alarm or related wiring becomes defective the defective components} shall be immediately repaired or replaced with a component of the same or equal pertormance. If the mound component fails to accept wastewater or begins to discharge wastewater to the ground surface, tt will be repaired or replaced in its` present location by increasing basal area if toe leakage occurs or by removing biologically clogged absorption and dispersal media, and related piping, and replacing said components as deemed necessary to bring the system into proper operating condition. See Page 6 of this plan for the name and telephone number of your local POWTS regulator and service provider. G)=OLOGICAL t115TORY: As we know, this area in Wisconsin is an area of the Glaciated (~cgion which occurred 35,000 to 5o,o0oyears ago. This caused erosion, deposition, stream erosion and other processes. Wisconsin is divided into five natural regions which are related to land by the use of plants, animals and 6y man. Also the boundaries arc determined by soil texture and structure in the underlying rocks. St. Croix County is located in the Western (,IPland which is underlain by Ordovician dolomites. (Martin, L. (1965). The Physical Geology of Wisconsin, (,Iniversity of Wisconsin Press, Madison, Wisconsin). Page 2 - ULDRYCII This Part of St Croix County was formed 6y. ground moraine deposits from the Wisconsin stage of glaciation. The parent material at this site is loess overglacial till. LOCAL HYDROLOGY: This land area is located within the Lower Chippewa River ~asin which includes theE,au Gallo River. (St Croix County j~evelopment Management Plan; Physical Features Analysis Pg. 2-t 1.) The ~au Gallo River is located two and one-half milts due west and flows at a southerly direction. Much of the groundwater is recharged from rainfall. Rainfall amounts for this area are based on readings from ~aldwin Waste Water Treatment Plant located 2 2/3 miles northwest from this site. The attached topographic map show the land sloping to the west and the moraine west is sloping to the cast. The low lying area inbetween the moraines is an area that tames and holds water. This area is 438 feet westerly from the tested area. This area is indicated on the copy of the cjoil Survey map as a `jlcybcr g series which is poorly draincd soils on foot slopes and in drainage ways ofground moraines. The tested area is mapped as Santiago silt loans that have a depth to seasonal high water table greater than five feet. SOIL MAPPING: As indicated in the `-foil Survey of St Croix County, Wisconsin Published by the jvational Cooperative Soil Survey the area tested is mapped as a Santiago silt loam with 2 to 6 Percent slopes that arc wel) drained soils on ridges and foot slopes of ground moraines or on Pittcd glacial drift and thcsc soils formcd in silty and loamy scdimcnt and in the underlying sandy loam till. The Qrdcr is an Alfisols and is classified as Typic Glossoboralfs. These soils arc formed mostly under broadleaf deciduous forest and where drainage is not restricted from the water table. SOiL CONDITIONS AND SURFACI=. WAT)=.R FLOW: Page 3 - ULDRYCII The `foil evaluation Report submitted by this certified soil tester encountered some gravel and a sandy clay loam texture at 18 inches. Rcdoximorphic features are observercd at 9 inches. There were no dense conditions observed. After a rainfall amount recorded at the ~aldwin Waste WatcrTreatment Plant of 1.5 inches within a seven day Period, three hand borings were observed on 24 May 2004. No standing water was observed in any boring. A hand dug pit to the depth of 24 inches was also observed with no free flowing water. Qn the same day, a site located 3 miles south which had a soil saturation determination on Simi{ar soil conditions was observed. Two observation pipes at that location had 1 O inches of standing water. The depth of the two holes were twenty-four inches. This site was approved in Qctober 2003 to a depth of t O inches for a mound system. ,also observed are many newly constructed sites within a milt south on lower lying land with mound systems. These areas are mapped as the same soil types. Man has influenced the topography of the area with the construction of `jtate Trunk Highway 94. The (,~Idrych property borders the south side of the freeway. The same moraine had been cut with an area of approximately t oo feet wide and an a{ovation of 20 feet. This would also influence the groundwater that is tamed to the site. The soil profiles show a 1 OYR3/2 to a depth of 9 to 1 1 inches with no redoximorphic features. Perching is not taking place in this horizon for any considerable period of time. PROPOSAL CORRECtION NEEDED 1.) The depth of the approved sand be at least 2.25 feet. CROIX COUNTY GOVERNMENT CENTER 1101 Carmichael Road Hudson, WI 54016-7710 Phone: (715)386-4680 Fax (715)386-4686 Date: 4/22/04 Re: Soil Report verification on-sites (4/15/04 and 4121/04) On April 15, 2004 I performed on-site inspections for proposed CSM sites to verify soil reports submitted for system installations in soils documented as meeting the A+4" restriction for mounds. On Apri121, 2004 Leroy Jansky, Regional Wastewater Specialist and myself visited three of the four sites to follow up on my inspections. The following is a brief summary of our findings: Site #1 Jamie Langer Lot 1 CSM# 637772 V. 15/P. 4028 - SE %4 NE 1/a Section 32 T28N R16W (Town of Eau Galle) -The soil tests submitted by Adam Schumaker (9/24/00) are not suitable for A+4" conditions. Due to the delay in using the reports, the site has been hayed and benchmarks/soil test pit locations have been obliterated. The general location for hand borings were documented in my notes and I used the dimensions shown on Adam's plot plan to find approximate tested area. The site contours were not properly represented on the plot plan and the slope is more concave than expected. Three hand borings were dug, all with prominent mottling found @a 10" in massive sandy loam below the A-horizon. Soil profile descriptions are attached. An interpretative review or groundwater monitoring would be necessary for this tested area, but there may be other locations on the property that can be investigated. The owner will have to contact a soil tester that can do A+0" determinations before a mound design can be reviewed. Site #2 Steve Lynum/Jeremy Wildner - NW'/4 NW Y4 Section 26 T28N R16W (Town of Eau Galle) -The soil report prepared by Lewis Bjork 2116/02) did correctly identify mottling at <A+4", but this condition was overlooked during the mound plan design and state plan review process. It was discovered during county sanitary permit application review. Lewis excavated another test pit next to his original B1 for my inspection and I found that distinct mottling was evident @ 11 ". Lewis concurred with this finding, as did Leroy Jansky. However, Mr. Jansky took time to examine 2 borings farther south and uphill from the original tested area, finding that the soils would just meet the A+4" requirement for a standard mound using component manual version 2.0. Steve Lynum and Jeremy Wildner were present and were advised to contact the soil tester and have a revised plan developed to send to the state for review and approval to move the proposed mound septic system to the acceptable area. Site #3 Jay UldrXch proposed CSM - NE '/a SW '/a Section 4 T28N R16W (Town of Eau Galle) -Mary Jo Hollister had completed a soil report in May 2003 that separated the Ap horizon into 2 parts, but that doesn't count toward the A+4" minimum requirement. I dug three hand borins to verify soils near the test pits indicated on the soil report plot plan. I found prominent mottling @ 9" (just below the A horizon) near B1, @ 12" near B2, and faint mottles between 11- 18" near B3. An Interpretive determination will be required to support a design for a mound with <A+4" and additional area needs to be tested to provide room for a larger mound using a lower linear loading rate. Site #4 Roger/Tom Christianson proposed CSM - SE '/4 NE '/4 Section 34, T28N, R18W (Town of Kinnickinnic) -Carl Heise prepared three reports for this CSM (3/5/04) and I checked soil conditions on Lots 2 & 3 using the existing test pits; I removed backfill and exposed fresh soil along the excavated walls. I found his descriptions to be accurate regarding depth to mottling, although B 1 on Lot 2 appeared to have some redoximorphic iron deposits below the silt loam A horizon at approximately 10". This may have been tension-related due to the loamy sand layer observed at 8" -17". His site plans were completed properly so I could identify the borings reported, but there were some excavated pits not reported and the installer will have to make sure that the mound is placed over acceptable borings at the contour indicated on the soil report. Even if the soil tester "doesn't like" the pit, it should be described and/or noted on the site plan to avoid confusion or worse, installation of a system in the wrong soils. During the 4/21/04 site visits, Leroy Jansky concurred with my findings that the Uldrych and Langer parcels will need further soil investigation, saturation determinations, and designs developed for <A+4" mound systems. Proper maintenance consists of pumping out the septic tank every three years or sooner, if needed, by a licensed pumper. What you put into the system can affect the function of the septic tank as a treatment stage in the waste disposal system.
