INSTRUCTIONS {
1. •A sanitary permit invalid for two (2) years. ,
2. Your sanitary permit may be renewed before the expiration date, and at the time of renewal any new
criteria in the Wisconsin Administrative Code will be applicable.
3. All revisions to this permit must be approved by the permit issuing authority.
4. Changes in ownership or plumber requires a Sanitary Permit Transfer/Renewal Form (SBD 6399) to be
submitted to the county prior to installation.
5. Onsife sewage systems must be properly maintaihed. The septic tanks must be pumped by" a licensed
pumper whenever necessary, usually every 2 to 3 years.
6. If you have questions concerning your,onsite sewage system, contact your local code administrator!ror-the
State of Wisconsin, Safety & Buildings Division, 608-266-3815.
To be complete and accurate this sanitary permit application must include:
1. Property owner's name and mailing address. Provide the legal description and parcel tax number(s) of
where the system is to be installed.
II. Type of building being served. Check only one and complete # of bedrooms if 1 or 2 Family Dwelling.
III. Building use. If building type is Public, check all appropriate boxes that apply.
IV. Type of permit. Check only one in line A. Complete line B if permit is for tank replacement, reconnection, or
repair.
V. Type of system. Check appropriate box depending on system type.
VI. Absorption system information. Provide all information requested in ##1-7.
VII. Tank information. Fill in the capacity of every new and/or existing tank, list the total gallons, number of
tanks and manufacturer's name. Indicate prefab or site constructed and tank material. Mmplete for all
septic, pump/siphon and holding tanks for this system. Check experimental approval only if tanks received
experimental product approval from DILHR.
VIII. Responsibility statement. Installing plumber is to fill in name, license number with appropriate prefix (e.g.
MP, etc.), address and phone number. Plumber must sign application form.

Complete plans and specifications not smaller than 8'f x 11 inches must be submitted to the county. The
plans must include the following: A) plot plan, drawn to scale or with complete dimensions, location of
holding tank(s), septic tank(s) or other treatment tanks; building sewers; wells; water mains/water service;
streams and lakes; pump or siphon tanks; distribution boxes; soil absorption systems; replacement system
areas; and the location of the building served; B) horizontal and vertical elevation reference points;
C) complete specifications for pumps and controls; dose volume; elevation differences; friction loss; pump
performance curve; pump model and pump manufacturer; D) cross section of the soil absorption system if
required by the county; E) soil test data on a 115 form; and F) all sizing information.

1983 Wisconsin Act 410 included the creation of surcharges (fees) for a number of
regulated practices which can effect groundwater.
The monies collected through these surcharges are user for mortttoring groundwater, ground-
water contamination investigations and establishment of standards

The plumbing plans and specifications for this project have been reviewed for
compliance with applicable code requirements. This approval is based on Chapter
145, Wisconsin Statutes and the Wisconsin Administrative Code. The plans are
stamped 'conditionally approved'. This approval is contingent upon compliance with
any stipulations shown on the plans. All items that are noted must be corrected.
All permits required by the city, village, township or county shall be obtained
prior to construction. The licensed plumber responsible for this installation
shall keep one set of plans with the department's approval stamp at the
construction site. The installer shall notify the appropriate inspector"when
inspections can be made.
This approval will expire two years from the date approved or if a sanitary
permit is obtained, it will expire the day the initial sanitary permit expires.
The Section of Private Sewage has reviewed these plans for private sewage system code
requirements only. These plans have not been reviewed for the code requirements
set forth in Section ILHR 82 for general plumbing or in Chapters 50-64 of the
Wisconsin Administrative code.

We acknowledge the installation of (a) holding tank(s) on the following property: (Provide legal description:)
Part of the NW-4 of the NW-4 of Section 17,T29N, R19W, Town of Hudson,
St.Croix County, Wisconsin as recorded in Volume 443, pages 523-525,
document no. 292851.
( See attached description and land use agreement.)
1. The owner agrees to file a copy of this contract with the local governmental unit hereinafter called the "municipality", which has
signed the pumping agreement required in Ch. ILHR 83.18 (4) (b), Wis. Adm. Code and
with the County of S t. C r o i x
2. The owner agrees to have the holding tank(s) serviced by the pumper and guarantees to permit the pumper to have access and to
enter upon the property for the purpose of servicing the holding tank(s). The owner agrees to maintain the all-weather access
road or drive so that the pumper can service the holding tank(s) with the pumping equipment. The owner further agrees to pay
the pumper for all charges incurred in servicing the holding tank(s) as mutually agreed upon by the owner and pumper.
3. The pumper agrees to submit to the municipality which has signed the pumping agreement required by s. ILHR 83.18 (4) (b), Wis.
Adm. Code, and to the county, a report for the servicing of the holding tank(s) on a semiannual basis. The pumper further agrees
to include the following in the semiannual report:
a. The name and address of the person responsible for servicing the holding tank;
b. The name of the owner of the holding tank;
c. The location of the property on which the holding tank is installed;
d. The sanitary permit number issued for the holding tank;
e. The dates on which the holding tank was serviced;
f. The volumes in gallons of the contents pumped from the holding tank for each servicing;
g. The disposa! sites to which the contents from the holding tank were delivered.
4. This agreement will remain in effect until the owner or pumper terminates this contract. In the event of a change in this contract.
the owner agrees to file a copy of any changes to this service contract or a copy of a new service contract with the municipality
and the County named above within ten (10) business days from the date of change to this service contract.

9. The Permittee shall not permit grazing on the subject property.
10. This agreement does not give the Permittee, its members or agents, any rights pertaining to hunting,
fishing, or trapping; and rights remaining under the control of the State of Wisconsin.
11. The Permittee shall not unnecessarily disturb or molest wildlife or wildlife habitat.
12. The area covered by this agreement Is open for use to all members of the general public without regard
to race, creed, marital status, color, sex, national origin, age, handicap, ancestry, sexual
orientation, arrest record or conviction record.
13. In connection with the performance of work under this agreement, the Permittee agrees not to
discriminate against any employe or applicant for employment because of age, race, religion, color,
handicap, sex, physical condition, developmental disability as defined in s. 51.01(5)0 Stats., sexual
orientation or national origin. This provision shall include, but not be limited to, the following:
employment, upgrading, demotion or transfer; recruitment or recrutiment advertising; layoff or
termination; rates of pay or other forms of compensation; and selection for training, including
apprenticeship. Except with respect to sexual orlentation,.the Permittee further agree to take
affirmative action to ensure equal employment opportunities. The Permittee agrees to post in a
conspicuous places available for employes and applicants for employment, notices to be provided,by the

Agreements estimated to be ten thousand dollars (510,000) or more require the submission of a written
affirmative action plan. Permittees with an annual work force of less than ten employees are exempted
from-this requirement.
11. This agreement, together with the specifications In the bid request and referenced parts and
attachments, shall constitute the entire agreement and previous communications or agreements pertaining
to this agreement are hereby superseded. Any contractual revisions including cost adjustments and time
extensions must be made by an amendment to this agreement or other written documentation, signed by both
parties at least 30 days prior to the ending date of this agreement.
15. Additional conditions specifically pertinent to this agreed.land use will be valid If enumerated In the
space provided between this condition and closing signature. Every such addition will be Initialled by
the Permittee and the Owner.
16-The Permittee wil submit a written plan to the Park Manager prior to the
placement of the holding tank. The plan will address proper abandonment
of the existing septic system. The Permittee agrees to be responsible
for upkeep, pumping costs and any other maintenance related to the holding
.tank.
17.This Agreement may be renewed upon expiration with approval by both
parties.
18. Upon.expiration of this Agreement, the Permittee, heirs or assigns
shall have 120 days to remove the dwel1ina and ahandon the site. na,--n_.
donment will include removal or proper filling of holding tank. If
the dwelling is not removed in the specified period it will become
the property of the Department.
