BB, 715-386-4680 Baldwin WI 54002 Parcel # 008-1008-50-000 Land Information Planning Dear Ms. Fulton: 715-386-4674 This letter confirms zoning approval according to the plans you have submitted to the 4677 Planning and Zoning Department to replace and add telecommunications equipment on an 715Real-33868G--4677 existing wireless communication tower located on property listed above. Staff finds that the Recycling project meets the applicable tower provisions in Sections 17.82 17.85, 17.86, and 17.87, of the 7J5-386-4675 findings: St. Croix County Zoning Ordinance based on the following 1. Approval of this land use permit includes the replacement and addition of ti telecommunications equipment on a 250 foot existing tower. A list of these replacements and additions is on pages 3 and 4 of the structural analysis report, dated June 28, 2012. 2. The existing tower has the capacity to structurally support the proposed new equipment according to the structural analysis that was prepared by Peter G. Jernigan, Jr., professional engineer. 3. A certificate of liability of insurance has been submitted. 4. Co locating . on a tower is a permitted with a land use permit in the Ag. Residential Zoning District. 5. The tower site is screened with trees. 6. Fencing appeared to be in good condition on my site visit in 2011. 7. The colors of the components are gray and will match the existing tower. 8. A letter from Waterford Consultants has been submitted verifying this facility will not cause interference with public safety communication systems. 9. A site lease agreement between Crown Castle and Arthur Jensen has been submitted. 10. Parking and access are adequate for this site. This permit has been issued to Global Signal Acquisitions with the following conditions; 1. The applicant shall contact the Planning and Zoning Department at least five days prior to commencing installation of the proposed antennae and equipment. 2. The applicant shall inform the Zoning Administrator when construction is complete and submit photos of the completed antennae and equipment. 3. The applicant shall notify the Zoning Administrator of any change in name or ownership of the approved facilities and provide new contact information for the purpose of keeping the file updated. 5. Any minor change (or addition) in the project shall require prior review and approval by the Zoning Administrator. Any major change and/or addition to the originally approved plans will require the submittal of a new land use permit application. 6. Within 12 months of cessation of operation or use of the wireless facilities, all unused antennas and equipment shall be removed at the applicant's expense. After the facilities are removed, the site shall be restored to its original condition or an improved condition. This approval does not allow for any structures (temporary or permanent), activities, or development beyond the limits of this request. Your information will remain on file in the St. Croix County Planning and Zoning Department. Please contact the Town of Eau Galle to obtain the necessary building permits for the proposed construction.
