Existing tower on Arthur Jensen property at.SP Cty Rd. BB

Dear Mr. Ries:
This letter confirms zoning approval according to the plans you have submitted to the Planning and
Zoning Department to co-locate Crown Castle Int'1 Corp. and AT&T Mobility equipment at an
existing wireless communication tower located on property listed above, owned by Arthur D. Jensen
Trust in the Town of Eau Galle. Staff finds that the project meets the tower provisions in Sections
17.85(2) and 17.87(2) of the St. Croix County Zoning Ordinance based on the following findings:
1. Approval of this land use permit includes the addition of new Crown Castle/AT&T facilities to
an existing 250' tower on property leased to STC Five LLC c/o Crown Castle Int'1 Corp.,
including placement of panel antennas at a centerline height of 220 feet and a new equipment
shelter;
2. All proposed Crown Castle/AT&T antennae and equipment meets the required setbacks and
height requirements in the Ag-Residential District;
3. The existing tower has the capacity to structurally support the proposed Crown
Castle/AT&T facilities as certified by a registered professional engineer for AT&T January
8, 2009;
4. The proposed Crown Castle/AT&T facilities meet mechanical and electrical standards as
indicated in Sheets S-01 through S-03, and E-01 through E04 of the specification sheets
dated January 08, 2009 and submitted as part of this permit application;
5. The proposed Crown Castle/AT&T facilities will provide cellular and Personal
Communication Services (PCS) that will not interfere with other existing facilities or pose a
threat to public health and safety as certified by, Raghavendra Parigi, Sr. Radio Frequency
Engineer for AT&T Mobility Wireless, in a letter dated January 29, 2009; and
6. Co-location is an accepted use, was a condition of the original Special Exception permit
issued in 1999 for a 250' telecommunications tower in the Ag Residential District, and is
encouraged by the Ordinance.
This permit has been issued to the applicant, Crown Castle International Corp. and AT&T Mobility,
with the following conditions, which are the responsibility of Crown Castle/AT&T Mobility or
agents acting on its behalf:
1. The applicant shall contact the Planning and Zoning Department at least five days prior to
commencing installation of the proposed antennae and facilities.
2. The applicant shall inform the Zoning Administrator when construction is complete and
submit photos of the completed antennae and facilities.
3. The applicant shall have one (1) year from the issuance of this land use permit to commence
construction. If the permit expires, the applicant will be required to secure a new land use
permit.
4. The applicant shall notify the Zoning Administrator of any change in name or ownership of the
approved facilities and provide new contact information for the purpose of keeping the file
updated.
5. Any minor change (or addition) in the project shall require prior review and approval by the
Zoning Administrator. Any major change and/or addition to the originally approved plans
will require the submittal of a new land use permit application.
6. Within 12 months of cessation of operation or use of the wireless facilities, all unused
antennas and equipment shall be removed at the applicant's expense. After the facilities are
removed, the site shall be restored to its original condition or an improved condition.
This approval does not allow for any structures (temporary or permanent), activities, or
development beyond the limits of this request. Your information will remain on file in the St.
Croix County Planning and Zoning Department. Please contact the Town of Eau Galle to obtain the
necessary building permits for the proposed construction. It is your responsibility to ensure
compliance with any other local, state, or federal rules or regulations.
