Located in the SE'/4 of the NW '/4 of section 33, T28N, R19W, Town of Troy Code Adminisuauo Parcel # 040-1124-90-000 715-386-4680 Dear Mr. Cernohaus: Land Information a Planning This letter confirms zoning approval according to the plans you have submitted to the 715-386-4674 Planning and Zoning Department to conduct a nonmetallic mining borrow site at the property listed above based on the following findings: Real Prompty 715-3$,04677 1. This site will be permitted as temporary borrow site since it's in connection with a Re#cang municipal road reconstruction project. 5-386-4675 2. This site will be reclaimed within 30 days of the completion of the project. 3. The project meets the applicable provisions in Sections 14.2 A. 1 through 9, B. 1 through 4 and C.1. of the St. Croix County Non-Metallic Mining Ordinance. 4. Nonmetallic mining is permitted in the Agricultural district since it is for governmental purposes. 5. A financial assurance is not required for this operation. This permit has been issued to David Cemohaus with the following conditions; 1. The applicant shall have one (1) year from the issuance of this land use permit to complete construction. If the permit expires, the applicant will be required to secure a new land use permit. 2. The applicant shall notify the Zoning Administrator of any change in name or ownership of the approved facilities and provide new contact information for the purpose of keeping the file updated. 3. Any minor change (or addition) in the project shall require prior review and approval by the Zoning Administrator. Any major change and/or addition to the originally approved plans will require the submittal of a new land use permit or special exception permit application. 4. This site shall be reclaimed within 30 days of the completion of the Cascade Avenue project, when complete the applicant shall file a notice of completion with the zoning office. This approval does not allow for any structures (temporary or permanent), activities, or development beyond the limits of this request. Your information will remain on file in the St. Croix County Planning and Zoning Department. It is your responsibility to ensure compliance with any other local, state, or federal rules or regulations. ST.
