Planning This letter confirms zoning approval according to the plans you have submitted to the
715-386-4674 Planning and Zoning Department to replace and add telecommunications equipment on an
existing wireless communication tower located on property listed above. Staff finds that the
I., I RealPropey project meets the applicable tower provisions in Sections 17.82 17.85, 17.86, and 17.87, of the
715-38644677 St. Croix County Zoning Ordinance based on the following findings:

5-386-4675 1. Approval of this land use permit includes the replacement and addition of
telecommunications equipment on a 128 foot existing T-Mobile tower. A list of these
replacements and additions is on the front page of the construction plans, sheet number
C-1, dated June 12, 2012.
f 2. The existing tower has the capacity to structurally support the proposed new
equipment according to the structural analysis that was prepared by Matthew T.
Mokanyk, a professional engineer.
3. A certificate of liability of insurance has been submitted.
4.- This is the only tower located on this property.
E 5. Co locating on a tower is a permitted with a land use permit in the Commercial Zoning
r; District.
6. The tower site is screened with a building.
7. Security lighting and fencing appeared to be in good condition on my November 2009
site visit.
8. The colors of the components are gray and will match the existing tower.
9. A letter from Alan Roberts a Radio Frequency Engineer has been submitted verifying
this facility will not cause interference with public safety communication systems.

10. A site lease agreement between T-Mobile and Monea Johnson has been submitted.
<uy 11. Parking and access are adequate for this site.

c This permit has been issued to American Tower with the following conditions;
1. The applicant shall contact the Planning and Zoning Department at least five days prior to
commencing installation of the proposed antennae and equipment.
2. The applicant shall inform the Zoning Administrator when construction is complete and

submit photos of the completed antennae and equipment.
3. The applicant shall have one (1) year from the issuance of this land use permit to complete

4. The applicant shall notify the Zoning Administrator of any change in name or ownership of the
approved facilities and provide new contact information for the purpose of keeping the file
updated.
5. Any minor change (or addition) in the project shall require prior review and approval by the
Zoning Administrator. Any major change and/or addition to the originally approved plans
will require the submittal of a new land use permit application.
6. Within 12 months of cessation of operation or use of the wireless facilities, all unused
antennas and equipment shall be removed at the applicant's expense. After the facilities are
removed, the site shall be restored to its original condition or an improved condition.
This approval does not allow for any structures (temporary or permanent), activities, or
development beyond the limits of this request. Your information will remain on file in the St.
Croix County Planning and Zoning Department. Please contact the Town of Somerset to obtain the
necessary building permits for the proposed construction. It is your responsibility to ensure
compliance with any other local, state, or federal rules or regulations.
