Parcel # 032-2013-90-050 K; 715-3864680 Land Information &'T Dear Mr. Buster: Planning 715-3864674 This letter confirms zoning approval according to the plans you have submitted to the Planning and Zoning Department to add telecommunications equipment on an existing RealProperry wireless communication tower located on property listed above. Staff finds that the project 715-386-4677 meets the applicable tower provisions in Sections 17.82 17.85, 17.86, and 17.87, of the St. Rey ng Croix County Zoning Ordinance based on the following findings: 745-3864675 s 1. Approval of this land use permit includes the addition of telecommunications equipment on a 190 foot existing T-Mobile tower. The details of this antenna are on page C-3 of the project plans, dated October 9, 2012. 2. A structural analysis from Jaime Reyes, a Professional Engineer, shows that the tower will support the new installation. 3. A copy of the lease has been submitted. 4. A certificate of liability of insurance has been submitted. 5. This is the only tower located on this property. 6. The tower is screened by the natural vegetation. 7. Parking and access are adequate for this site. 8. Co locating on a tower is a permitted with a land use permit in the Agricultural Residential Zoning District. 9. A letter from Joshua Mathews, a Radio Frequency Engineer, has been submitted verifying this facility will not cause interference with public safety communication systems. This permit has been issued to T-Mobile with the following conditions; 1. The applicant shall contact the Planning and Zoning Department at least five days prior to commencing installation of the proposed antennae and equipment. 2. The applicant shall inform the Zoning Administrator when construction is complete and submit photos of the completed antennae and equipment. 3. The applicant shall have one (1) year from the issuance of this land use permit to complete construction. If the permit expires, the applicant will be required to secure a new land use permit. ST. The applicant shall notify the Zoning Administrator of any change in name or ownership of the approved facilities and provide new contact information for the purpose of keeping the file updated. 5. Any minor change (or addition) in the project shall require prior review and approval by the Zoning Administrator. Any major change and/or addition to the originally approved plans will require the submittal of a new land use permit application. 6. Within 12 months of cessation of operation or use of the wireless facilities, all unused antennas and equipment shall be removed at the applicant's expense. After the facilities are removed, the site shall be restored to its original condition or an improved condition. This approval does not allow for any structures (temporary or permanent), activities, or development beyond the limits of this request. Your information will remain on file in the St. Croix County Planning and Zoning Department. Please contact the Town of Somersrt to obtain the necessary building permits for the proposed construction.
