The following conditions shall be met during construction or installation and prior to occupancy or use: Key Item(s) • In the event this soil absorption system or any of its component parts malfunctions so as to create a health hazard, the property owner must follow the contingency plan as described in the approved plans. In addition, the owner must insure that the operation, maintenance and monitoring duties as described in section VIII of the mound component manual are complied with. A copy of this information must be given to the owner upon completion of the project. • The designer proposes to install a state approved effluent filter to achieve the requirement of wastewater particle size. Pursuant to outlet filter product approval stipulations, maintenance information must be given to the owner of the POWTS explaining that periodic cleaning of the effluent filter is required. The access opening used to service the filter shall terminate at or above finished grade with a watertight cover. Reminder • This mound is designed to be installed on a level site. • Limit activities in the area 15' beyond the edges of the mound per Mound Component Manual. • Surface water drainage shall be diverted away from the system area per Mound Component Manual. • Materials shall conform to the requirements of COMM 84. • Maintain well and waterline set backs per COMM 83.43(8)(1). Consult the Department of Natural Resources for well setbacks and other regulations and exceptions. . . No one should ever enter a septic or pump tank since dangerous gases may be present that could cause death. Septic and pump tank abandonment shall be in accordance with Comm 83.33, Wis. Adm. Code when the tanks are no longer used as POWTS components. Septic or pump tank manhde risers, access risers and covers should be inspected for water tightness and soundness. Access openings used for service and assessment shall be sealed watertight upon the canpletion of service. Any opening deemed unsound, defective, or subject to failure must be replaced. F~osed access openings greater than 8-inches in diameter shall be secured by an effective locking device to prevent accidental or unauthorized entry into a tank or component. Septic Tank The septic tank shall be maintained by an individual certified to service septic tanks under s. 281.48, Stats. The contents of the septic tank shall be disposed of in accordance with NR 113, Wis. Adm. Code. The operating condition of the septic tank and outlet filter shall be assessed at least once every 3 years by inspection. The outlet fitter shall be cleaned as necessary to ensure proper operation. The fitter cartridge should not be removed unless provisions are made to retain solids in the tank that may slough off the filter when removed from its enclosure. If the filter is equipped with an alarm, the fitter shall be serviced ff the alarm is activ~ed continuously. Intermittent filter alarms may indicate surge flows or an impending continuous alarm. The septic tank shall have its contents removed when the vdume of sludge and scum in the tank exceeds 1/3 the liquid volume of the tank. If the contents of the tank an: not removed at the time of a triennial assessment, maintenance personnel shall advise the owner of when the next service needs to be perfomred to maintain less than ma>amum scum and sludge accumulation in the tank. The addition of bidogical or chemical additives to enhance septic tank performance is generally not required. However, 'rf such products are used they shall be approved for septic tank use by the Department of Commerce. Pump Tank The pump (dosing) tank shall be inspected at least once every 3 years. All switches, alarms, and pumps shall be tested to verify proper operation. If an effluent filter is installed within the tank tt shall be inspected and serviced as necessary. Mound and Pressure Distribution Svstem No trees or shrubs should be planted on the mound. Plantings may be made around the mound's perimeter, and the mound shall be seeded and mulched as necessary to prevent erosion and to provide some protection from frost penetration. Traffic (other than for vegetative maintenance) on the mound is not recommended since soil compaction may hinder aeration of the infiltrative surface within the mound and snow compaction in the winter wilt promote frost penetration. Cold weather installations (October-February) dictate that the mound be heavily mulched as protection from freezing. Influent quality into the mound system may not exceed 220 mg/L BODS, 150 mg/L TSS, and 30 mg/L FOG for septic tank effluent or 30 my/l. GODS, 30 mg/L TSS, 10 rrg/L FOG, and 10" cfuh 00 mL for highly treated effluent. Influent flow may not exceed maramum design flaw specified in the permtt for this installation. The pressure distribution system is provided with a flushing point at the end of each lateral, and tt is recommended that each lateral be flushed of accumulated solids at least once every 18 months. When a pressure test is perfomred it should be compared to the initial test when the system was installed to determine if orifice clogging has occurred and ff orifice cleaning is required to maintain equal distribution within the dispersal cell. Observation pipes within the dispersal cell shall be checked for effluent ponding. Ponding levels shall be reported to the owner, and any levels above 8 inches considered as an impending hydraulic failure requiring additional, more frequent monitoring. A copy of this letter including instructions and information relating to proper use and maintenance of the system must be given to the owner and each subsequent owner upon completion of the project. • The well must be a minimum of 25 feet from any POWTS tank, and a minimum of 50 feet from the absorption area. • Limited activities are allowed in the area l0 feet around the parameter of the component area. Soil compaction, excavation, vehicular traffic and other similar activities that impact the treatment and dispersal are prohibited. • Comm 83.52 Responsibilities. The owner of a POWTS shall be responsible for ensuring that the operation and maintenance of the POWTS occurs in accordance with this chapter and the approved management plan under s. Comm 83.54(1). [n addition, the owner is responsible for submitting a maintenance verification report acceptable to the county for maintenance tracking purposes. Reports shall be submitted at intervals appropriate for the component(s) utilized in the POWTS. • Comm 83.52(2) A POWTS that is not maintained in accordance with the approved management plan or as required under s. Stats. • Inspection of the private sewage system installation is required. Adm. Code General This system shall be operated in accordance with Cantu 82-84 Wis. Adm. Cade, and shall maintained in accordance with its' component manuals [SBD-10691-P (N.01/01) and SSWMP Publication 9.6 (01/81)] and local or state rules pertaining to system maintenance and maintenance reporting. No one should ever enter a septic or pump tank since dangerous gases may be present that could case death. Septic and pump tank abandonment shall be in accordance with Comm 83.33, Wis. Adm. Code when the forks are no longer used as POWTS components. Septic or pump tank manhde risers, access risers and covers should be inspected far water tightness and soundness. Access openings used far service and assessment shall txx sealed watertight upon the completion of service. Any opening deemed unsound, defective, or subject to failure must be replaced. F~osed access openings greater than 8-inches in diameter shall be secured by an effective loddng device to prevent accidental or unauthorized entry into a tank or component. Seotio Tank The septic tank shall be maintained by an individual certified to service septic tanks under s. 281.48, Stets. The contents of the septic tank shall be disposed of in accordance with NR 113, Wis. Adm. Code. The operating condition of the septic tank and outlet fitter shall be assessed at least once 3 tom. - The outlet filter shall be leaned ~ rx~say to ensure proper operation. The finer cartridge should nct be remxyued unless provisions are o n sdids in the tank that may sioug i from its enclosure. If the filter is equipped with at alarm, the filter shall be serviced if the alarm is activated continuously. Intermittent filter alarms may indicate surge flaws or an impefxling continuous alarm. The septic tank shall have its contents remioved when the vdume of sludge and scum in the tank exceeds 1/3 the liquid vdume of the tank. ff the contents of the tank are not remxr~ned at the time of a triennial assessment, maintenance personnel shall advise the owner of when the next service needs to be pertomred to maintain less than ma~amum scum and sludge accumulation in the tank. The addition of bidogical err chemical additives to enhance septic tank perfomnance is ger~ly nct required. However, if such products are used they shall be appraaed for septic tank use by the Departrrrent of Cornrrrerce. Pumo Tank The pump (dosing) tank shall be inspected at least once every 3 years. All switches, alarms, and pumps shall be tested to operation. If an effluent filter is installed within the tank ft shall be inspected and serviced as necessary. ~y Proper Mound and Pressure Distribution Svstem No trees or shrubs should be planted on the mound. Plantings may be made around the mound's perimeter, and the mound shall be seeded and mulched as necessary to prevent erasion and to provide same protection from frost penetration. Traffic (other than for vegetative maintenance) on the mound is mat recarrrrrended since sdl compaction may hinder aeration of the infiltrative surtace within the mound and snow carripactiai in the winter will promote frost penetration. Cdd weather installations (October-February) dictate that the mound be heavily mulched ~ protection from freezing. Influent quality into the mound system may not exceed 220 mg/L GODS, 150 mglL TSS, and 30 mag/L FOG far septic tank effluent err 30 mg/L BODS, 30 mg/L TSS, 10 mg/L FOG, and 104 cfu/100 mL for highly trued effluent. Influenrt flanr may not exceed ma~dmum design flaw specified in the permit for this installation. The pressure distribution system is provided with a flushing print at the end of each lateral, and ft is rewmrrrended that each lateral be flushed of accumulated sdids at least once every 18 months. When a pressure test is pertomied it should be compared to the initial test when the system was installed to determine if once clogging has occurred and 'rf once cleaning is required to maintain equal distribution within the dispersal cell. Observation pipes within the dispersal cell shall be ct~eciced for effluent ponding. Parading levels shall be reported to the awrxe•, and any levels above 6 inches considered as an impending hydraulic failure requiring additional, more frequent monitoring. If the c tank or an of its Contingenov Plan ~ y components become defective the tank or compa'tent shall be repaired or replaced to keep the system in proper operating candifian. If the mound component fails to accept wastewater or begins to discharge wastewater to the ground surtace, it will be repaired or replaced in its' present location by increasing basal aea 'rf tce leakage occurs or by removing bidogically clogged absorption and dispersal media, axi related piping, axt replacing said components as deemed necessay to brim the system into proper operating condition.
