Land Iniormarion & This letter confirms zoning approval according to the plans you have submitted to the
pla= ng Planning and Zoning Department to replace and add telecommunications equipment on an
715-3864674 existing wireless communication tower located on property listed above. Staff finds that the
project meets the applicable tower provisions in Sections 17.82 17.85, 17.86, and 17.87, of the propo Real

Reocling 1. Approval of this land use permit includes the addition of telecommunications
715-386-4675 equipment on a 290 foot existing American Tower. A list of these replacements and
additions is on the page T-1 of the plans dated November 7, 2012 and page 2 of the
structural analysis report, dated January 23, 2013. The report shows that the tower will
support the added equipment.
2. The installation plans were prepared by Raphael Mohamed, a professional engineer.
3. A copy of the lease has been submitted.
4. A certificate of liability of insurance has been submitted.
5. This is the only tower located on this property.
6. Co locating on a tower is a permitted with a land use permit in the Ag-Residential
Zoning District.
7. A letter from Garrett Lysiak, P.E. has been submitted verifying this facility will not

ri ; ✓ cause interference with public safety communication systems.
This permit has been issued to Sprint with the following conditions;

1. The applicant shall contact the Planning and Zoning Department at least

prior to commencing installation of the proposed antennae and equipment.
2. The applicant shall inform the Zoning Administrator when construction is complete
b and submit photos of the completed antennae and equipment.
3. The applicant shall have one (1) year from the issuance of this land use permit to
complete construction. If the permit expires, the applicant will be required to secure a

new land use permit.
4. The applicant shall notify the Zoning Administrator of any change in name or
ownership of the approved facilities and provide new contact information for the

y purpose of keeping the file updated.

5. Any minor change (or addition) in the project shall require prior review and approval
by the Zoning Administrator. Any major change and/or addition to the originally
approved plans will require the submittal of a new land use permit application.
6. Within 12 months of cessation of operation or use of the wireless facilities, all
unused antennas and equipment shall be removed at the applicant's expense. After
the facilities are removed, the site shall be restored to its original condition or an
improved condition.
This approval does not allow for any structures (temporary or permanent), activities,
or development beyond the limits of this request. Your information will remain on file in
the St. Croix County Planning and Zoning Department. Please contact the Town of
Glenwood to obtain the necessary building permits for the proposed construction. It is your
responsibility to ensure compliance with any other local, state, or federal rules or
regulations.
