Real propetty This letter confirms zoning approval according to the plans you have submitted for
715-386-4677 filling and grading an area of approximately 5000 square feet within 300 feet of the
Ordinary High Water Mark (OHWM) of an unnamed pond to construct a driveway,
Reci cling single-family dwelling, and private on-site wastewater treatment system (POWTS) on
715-386-4675 the property referenced above in the Town of St. Joseph. Staff finds that the proposed
project meets the spirit and intent of the St. Croix County Zoning Ordinance and
Shoreland District with the following findings:
1. Filling and grading less than 10,000 square feet in area less than 300 feet from
the OHWM on slopes less than 25 percent is allowed with a land use permit in
the Shoreland District pursuant to Section 17.29(2)(c) of the St. Croix County
Zoning Ordinance;
2. The filling and grading required for excavating the foundation of a single-family
dwelling, the driveway, and POWTS are located within 1000 feet of the OHWM
and subject to Shoreland zoning requirements. The proposed driveway, single-
family dwelling, and POWTS meet all required setbacks and dimensional
standards provided in the St. Croix County Zoning Ordinance;
3. The applicant submitted an erosion control and storm water management plan
featuring retention areas for roof runoff from the garage and house. With
conditions to install appropriate sediment control measures e.g. silt fence or
sediment "logs" below the construction area, to maintain erosion control
measures until self-sustaining permanent vegetation is established on all
disturbed areas, to prohibit the use of phosphorous fertilizer to establish and
maintain a lawn, to implement the storm water management plan, and record a
maintenance and monitoring agreement against the property for the infiltration
basins, negative impacts to the water quality of the wetland will be minimized;
4. The shoreline of the pond is currently unimproved. With conditions for
maintaining the current level of native vegetative cover along the shoreline to
preserve wildlife habitat and provide further filtration of storm water runoff from
I the site consistent with the standards in the Shoreland District, the natural
beauty and ecology of the pond will be protected;
5. The St. Croix County Land and Water Conservation Department has reviewed
the plans and recommends approval of the land use permit with conditions that
an affidavit for the stormwater management plan be recorded against the

6. a Wisconsin Department of Natural Resources has reviewed the plans and commented that a
rmit will not be required for land disturbance less than 10,000 sq. ft. and outside the 75' OHWM

management plan submitted, and as provided in the conditions below.

7 areas, on the construction site and the pond's OHWM for control of contaminated runoff.
~lg4 3. The sanitary permit issued for installation of the POWTS will require compliance with all

stay ide the 75' OHWM setback.
4. The shall maintain all erosion and sediment control measures until permanent, self-
sust g vegetation is successfully established on all disturbed areas of the site.
5. No phosphorous fertilizers shall be used on the disturbed areas of the site, unless a soil test
'confirms that phosphorous is needed for establishing permanent vegetative cover.
%-6. The current level of native trees, shrubs, and groundcover along the shoreline shall be
maintained.
7. Within 30 days of completing construction, the applicant shall execute and record an affidavit
against the property with the Register of Deeds referencing the storm water management plan and
required maintenance and monitoring for the rainwater retention structures, and provide the Zoning
Administrator with a recorded copy of these documents.
8. Within 60 days of completing construction, the applicant shall submit to the Zoning Administrator
photos of the completed project as viewed from all angles. The photographs must also show all
storm water management measures and vegetation on the site.
This apbroval does not allow for any additional construction, structures or structural changes,
grading,; filling, or clearing of vegetation beyond the limits of this request. Your information will
remain on file in the St. Croix County Planning and Zoning Department. It is your responsibility
to ensure compliance with any other local, state, or federal rules or regulations, including
obtaining a building permit from the Town of St. Joseph.

We, Jason and Lisa Haverly, plan to build a 4 bedroom 3.5 bathroom single family home at 477 Perch
Lake Rd. The home will be a 1.5 story home with a footprint of 24' x 44' with a 6' x 18' bump out to
the south. A 6' x 24' front porch, west end of the home and a 10' x 24' covered deck on the east end
of the home.
The garage will be a 24' x 30' detached garage. The driveway will measure roughly 150' in length and
have a turn around area to the west of the garage.
A conventional, gravity fed septic system will be installed to the north of the garage, with a lift pump
for the basement bathroom. A well will be drilled to the south-east of the home.
Two rainwater collection areas will be developed to handle 1.5" of rainfall. A rain water collection
area on the south side of the home will be 10' x 35' x 6". Another collection area will be north east of
the garage with dimensions of 10' x 15' x 12".
Construction Sequence
1. Install silt fence as designated in drawing
2. Install 50' of gravel driveway prior to entering the road.
3. Strip topsoil and save, excavate for house
4. Strip topsoil and save, excavate for garage
5. Install septic system, upon completion reseed with native grasses
6. Strip topsoil and save, excavate for driveway
7. Excavate for rainwater collection area for rainwater runoff as specified in drawing
8. Restore any bare ground from excavation within the 75' setback from the OHWM
9. Lay footings and foundation walls
10. Pour slab for garage
11. Pour slab for basement
12. Parge coat foundation walls
13. Install french drain system and Water proofing (Delta -MS)
14. Backfill foundation and complete to 5% grade away from house.
15. Construct house and garage
16. Install gutters and drainage to rainwater collection area
17. Replace topsoil and finish grade
18. Seed lawn
