Dave,
I have reviewed your land use permit application for Clear Wire's co-location of (3) antennas, (3) microwave dishes and an outdoor
equipment cabinet on the northern most tower located at 1372 Main Street In Houlton WI.
The following items need to be provided or addressed before I can issue this permit;
1.) An insurance policy for the transmission facility for personal injury, wrongful death, and property damage claims.
2.) The old tower structure that is laying on the ground must be removed from the property and properly disposed of.
3.) A certification that the transmission facility will not cause interference with previously established public safety
communication systems.
4.) A written description of the type of consumer services that this co-location will provide.
5.) A written acknowledgement by the landowner that he will abide by all applicable terms and conditions of the land use and
special exception permits, including the restoration and reclamation requirements of section 17.83 (6) of the wireless
communication service ordinance.
PLEASE NOTE: The Town of St. Joseph was mailed a copy of this application on March 29th and has until 5:00 pm on April 8th to
comment on this application.

Microwave Dishes and an outdoor equipment cabinet proposed at: 1372 main Street,
Houlton, WI 54082. Parcel Identification Number: 030-2048-95-000.
Buell Consulting Inc. is working on behalf of Clear Wireless LLC, a Nevada limited liability
company commonly known as "Clearwire" to perform site development services which entails
leasing, zoning and permitting for the communication facilities needed for the expansion of the
Clear Wire network in the Midwest Region. Clear Wireless LLC is requesting approval for a
Land Use Permit in order to install (6) six antennas and an outdoor equipment cabinet within the
existing tower compound owned by Chuck Levake of Leques Towers Inc. The proposed site is
located at: 1372 main Street, Houlton, WI 54082.
Clearwire is specifically proposing to install (3) panel antennas and (3) Directional Microwave
Dishes at 155' AGL of the existing self support tower owned by Chuck Levake of "Levques
Towers Inc.". Clear Wire proposes to install an outdoor equipment cabinet (50" H X 25" D X
25" W) located within the existing Levques Tower Compound located at: 1372 Main Street,
Houlton, WI 54082. I have attached construction drawings and antenna/equipment specifications
for your review as well.
